1 トラムセット :tramadol( トラマドール ) の効果 (110721) メーカーの置いて行ったパンフレットの中に トラムセット があった トラムセットはトラマドール塩酸塩とアセトアミノフェンを配合した新しい鎮痛剤 トラマドールのμオピオイド受容体作動作用 セロトニン ノルアドレナリン再取り込み阻害作用とアセトアミノフェンの疼痛域値の上昇がトラムセットの作用とされる 非オピオイド鎮痛剤で治療困難な 非癌性慢性疼痛 抜歯後の疼痛に適応がある トラムセット ( トラマドール ) の効果を検討した論文を読んでみることにした コクラン 3 本に目を通 してみる 参考文献 1 は変形性関節症に対するトラマドールの効果を検討している これによると 疼痛は VAS で 12% 減少させている 中等度の改善を認める患者は 6 人に 1 人 The placebo-controlled studies indicated that participants who received tramadol had less pain (-8.5 units on a 0 to 100 scale; 95% confidence interval (CI) to -5.0) than patients who received placebo. This represents a 12% relative decrease in pain intensity from baseline. Participants who received tramadol had a 37% increase (95% CI 1.2 to 1.5) in the likelihood of reporting moderate improvement (number needed to treat to benefit = 6; 95% CI 4 to 9). Participants who received tramadol had 2.27 times the risk of developing minor adverse events and 2.6 times the risk of developing major adverse events, compared to participants who received placebo. Of every eight people who receive tramadol or tramadol/paracetamol, one will stop taking the medication because of adverse events, number needed to treat to harm (NNTH)= 8 (95% CI 7 to 12) for major adverse events.

Pooled results revealed that tramadol was more effective than placebo for pain relief, SMD 0.71 (95% CI 0.39 to 1.02), and improving function, SMD 0.17 (95% CI 0.04 to 0.30). The two most common side effects of tramadol were headaches, RD 9% (95% CI 6% to 12%) and nausea, RD 3% (95% CI 0% to 6%). One trial comparing opioids to another analgesic (naproxen) found opioids were statistically significant for relieving pain but not improving function. When re-calculated, the results were not statistically significant for either pain relief (SMD -0.58; 95% CI to 0.26) or improving function (SMD -0.06; 95% CI to 0.76).

Three of the trials which compared tramadol to placebo (total 303 participants) were combined in a meta-analysis. The number needed to treat with tramadol compared to placebo to reach at least 50% pain relief was 3.8 (95% confidence interval 2.8 to 6.3). There were insufficient data to draw conclusions about the effectiveness of tramadol compared to either clomipramine or morphine. Numbers needed to harm were calculated for side effects resulting in withdrawal from the placebo controlled trials. Three trials provided these data, and the combined number needed to harm was 8.3 (95% confidence interval 5.6 to 17).

4 ( 参考文献 3 より引用 ) 各論文で指摘されていた通り 効果はそれなりだが 副作用の頻度は非常に多い 添付文書で も指摘されているように 副作用の発現率は 81.8% で その中でも悪心が 41.4% 嘔吐が 26.2% 傾眠が 25.9% 便秘が 21.2% 浮動性めまいが 18.9% と多い プラセボよりは効果が高いことは確かだが 他の薬剤と比較して差が無いという論文もあるようなので ファーストチョイスとしては使い難い もちろん 非オピオイド鎮痛剤で治療困難な 非癌性慢性疼痛 が適応なので いきなりトラムセットという選択は少ないと思う 使い方次第では強力な武器になりそうだが 患者と相談しながら使う薬剤になりそうだ
