1 Dr. Harvey Manes is the creator of and is presently a practicing orthopedic surgeon in the New York area and was the youngest board certified orthopedic surgeon in the United States. He has specialized in pain management for over 30 years. He has made many contributions to orthopedics including the publication of more than a dozen articles and as a pioneer in joint replacement surgery, he was the first orthopedic surgeon to implant a total elbow replacement and total ankle replacement in the Tri State Area. For over twenty years he was a clinical instructor of orthopedic surgery and taught surgery to numerous resident physicians at Nassau County University Hospital. He also served as a team physician at both the University and High School levels. He continues to practice orthopedic surgery in the New York area, specializing in sports medicine, pain management, arthritis and arthroscopic surgery.

4 RESEARCH Research Article - Dr. Harvey Manes The main stay for treatment for pain and arthritis in the joints and muscles for the past thirty years has been almost exclusively the domain of the NSAIDs. As a result, dozens of drugs in this category were developed during this time. Although these drugs were fraught with serious side effects, they continued to be used. Several years ago, the COX-2 inhibitors were developed and were believed to solve the "side effects" problem of the NSAIDs. However, it was soon discovered that not only did they not eliminate the well known complications but they created a whole new category of side effects, such as cardiovascular events that could lead to death. During this time there were numerous herbal remedies being developed. Unfortunately the research on these substances was frequently anecdotal, and not taken very seriously by the main stream medical community. Now that the conventional NSAIDs have all been tainted, alternative methods of treating pain and arthritis needed to be developed. After an extensive review of hundreds of herbal products listed in the homeopathic and conventional medical literature, six of the most effective substances were combined with the only topical NSAIDs proven to relieve pain. The purpose of the study was to determine the effectiveness of Dr. Harvey's Pain Reliving Cream pain relieving cream and compare it to 1) a top selling topical pain relief cream, 2) a placebo, and 3) the most popular oral NSAID on the market. The investigation was conducted as a random double blind placebo- controlled study in two separate centers. There were a total of 80 patients ranging in age from all of whom suffered with painful conditions involving one joint (knee, elbow, shoulder, or ankle), or muscle group (cervical or lumbar). Patients were randomly picked and divided into four different groups. Those who had other medical problems or who were not considered to be compliant were excluded. Those patients who had more than one area of pain, or were taking other pain remedies or had severe pain (>8.5) were also excluded. The three creams included Dr. Manes' formula, asper cream/sports cream, and a placebo cream. The creams used were indistinguishable as to smell, look, and tactile sense. The patient was told to place a small amount of cream in the palm of their hand the size of a nickel and rub the entire amount into the painful area. The fourth group was instructed to take advil 400 mg. All the patients were told to fill out a questionnaire on an hourly basis for five hours to determine efficiency of treatment. The questionnaire was reviewed by a nurse for accuracy upon presentation. The questionnaire asked the patient to grade the level of pain and range of motion on a scale of 1-10 using.5 intervals. If a pain score was between 4 and 5, then 4.5 was used. 1) Initial severity of pain 2) Level of pain at hourly intervals 3) Initial range of motion 4) Range of motion at hourly intervals

5 Directions: Apply generously and massage into painful sore areas until completely absorbed into skin.
