Water lilies and fish, on which the majority of the population survive, will likely become scarce as the rivers and swamps start drying up by January, leading to large food deficits. The joint assessment team concluded that without sustained food and livelihoods assistance and nutrition and health services, the situation is likely to degenerate significantly in the first quarter of After an RRM mission in Koch and Bauw in Unity State in late September, UNICEF was part of interagency missions to the same locations from 9-12 November. The missions were an opportunity for UNICEF to screen and treat children for malnutrition; provide primary health consultations; rehabilitate boreholes and follow up on unaccompanied and separated children identified during the September mission. So far in 2015, UNICEF and partners have provided critical child protection services to 331,217 children, including psychosocial support and family tracing and reunification. 2 Situation Overview & Humanitarian Needs According to the latest information published by UNMISS on 8 October, the estimated number of civilians seeking safety in six Protection of Civilians (PoC) sites located on UNMISS bases is 184,284 including 107,649 in Bentiu, 27,950 in Juba, 45,462 in Malakal, 2,289 in Bor, 700 in Melut and 234 in Wau. An estimated 6,000 people entered Bentiu PoC in October. Despite the signing of the Peace Agreement, the security situation in the country remains volatile. Clashes were reported in Unity State including in the southern areas of Rubkona as well as Nhialdu and Leer. Upper Nile remained calm during the reporting period, though Kodok and Melut have seen an increase in troop build-up. The humanitarian situation in Western Equatoria remains of concern, and despite church leaders mediating with to support a peaceful end to the ongoing unrest there, the situation in Mundri town and Ezo region has not yet stabilized. Meanwhile, the security situation in Eastern Equatoria state is deteriorating, with an increase in crime. UNICEF participated in an IPC joint assessment mission in Unity State which confirmed that an emergency food security situation still exists in central and southern Unity. The joint assessment team concluded that without food and livelihoods assistance and nutrition and health services, the situation will deteriorate significantly in the first quarter of During the reporting period, 12 suspected measles cases were reported (9 in Bentiu, 2 in Torit and 1 in Upper Nile) for a total of 487 suspected measles cases in UNICEF together with partners have developed a measles response plan to conduct a nationwide measles follow-up campaign in Humanitarian leadership and coordination UNICEF actively participates in the Humanitarian Country Team and the Inter Cluster Working Group, which lead strategic and cross-sectoral coordination of humanitarian programmes. UNICEF leads the WASH, Nutrition and Education clusters as well as the Child Protection sub-cluster. Within the Health cluster, UNICEF provides leadership on vaccination, communication and social mobilization. UNICEF also supports the core supply pipelines for the Education, WASH and Nutrition clusters, providing essential humanitarian supplies to all partners. While continuing to focus on the delivery of life-saving interventions, UNICEF is also investing in providing access to education and a protective environment for affected children. Humanitarian Strategy: Rapid Response Mechanism (RRM) As part of the interagency rapid response teams, UNICEF continues to expand activities in remote locations together with WFP and partners. Through 68 missions, UNICEF has reached 882,000 people including 197,000 children under 5. In 2015, UNICEF has deployed 34 RRM missions (including 10 missions in Unity State), reaching 459,000 people, including 82,000 children under 5. After an RRM mission that visited Koch and Bauw Payams in late September, UNICEF was part of interagency missions to the same locations from 9-12 November. The nutrition status in Koch was worse than the September screening with proxy moderate acute malnutrition (MAM) and proxy severe acute malnutrition (SAM) rates of 25.2 per cent and 7.1 per cent. This shows an alarming increase from 4.2 per cent MAM and 0.9 per cent SAM in September. The food security situation in Koch is dire, with pregnant and lactating women and children most affected. In Koch, acute respiratory infection (ARI), malaria and acute watery diarrhoea (AWD) were the main morbidities found from the 227 consultations provided, with 85 being for children under 5. Fifty-four clean delivery kits were distributed to pregnant women who sought antenatal care services during the mission. UNICEF also rehabilitated three boreholes in the market, the primary school and a cattle camp. Bauw was reported to have received 8,000 households displaced from Leer due to an offensive in the town. Based on a rapid survey of 51 households during the latest mission, 70 per cent of the new influx was actually from within Koch County, with more than half of them being returning residents of Bauw, rather than new IDPs from Leer. A total of 1,327 primary health care consultations were conducted with 345 being for children under 5. In terms of nutrition, 2.9 per cent of children were identified with SAM and 16.9 per cent with MAM, with 1,472 children under 5 screened. This is an increase from the September mission where SAM and MAM rates were 0.25 per cent and 5.9 per cent. The reasons for the change include increased displacement and lack of food, with water lilies and pumpkins being the main source of food. 2

3 Summary Analysis of Programme Response HEALTH: Malaria continued to be the most common cause of morbidity, with 25,838 cases reported from the stable counties, PoCs and IDP camps countrywide. This represents a proportionate morbidity of 44 per cent, with Bentiu, Malakal, Warrap, Western Bahr el Ghazal, Northern Bahr el Ghazal and Lakes being the most affected and exceeding the epidemic threshold. So far this year, UNICEF has provided 500,000 doses of antimalarial drugs; 454,170 malaria rapid diagnostic tests; and 452,153 long-lasting insecticide treated nets (LLITNs) for both conflict and non-conflict affected counties. Additional 61,000 doses of antimalarial has been received during this reporting period and will be prepositioned to the states with a high burden of malaria Trends of Priority Epidemic Prone Diseases from week 1 to week 46 of Total Consultation Malaria Diarrhoea ARI Other diseases Malaria, acute respiratory infection (ARI) and diarrhoea continue to be the major causes of morbidity and mortality among children under 5 years in all reporting sites. A total of 21,508 children under 5 were treated for common childhood illness during this reporting period, with malaria accounting for 42 per cent of consultations followed by ARI at 19 per cent and diarrhoea at 17 per cent. Forty deaths occurred among the children under five in Bentiu, Bor and Mingkaman. The majority of hospital deaths occurred within 48 hours of admission; this is attributed to delays in seeking health care which could be associated to mismanagement of patients in the community by private practitioners. Community sensitization with messages emphasizing importance of early health seeking behaviour need to continue. UNICEF supports its implementing partners to ensure timely treatment of the common childhood illness through provision of antibiotics, respiratory timers and malaria drugs for prompt and effective case management. Cumulatively, in 2015, 613,505 people have accessed basic primary health care services through UNICEF support in the conflict affected and less-conflict affected. Integrated Community Case Management (ICCM) services continued in Mingkaman for children under 5 through partnership with Health Link. A total of 751 children under five were treated for malaria, pneumonia and diarrhoea. Since the launch of ICCM in Mingkaman, 18,111 children under 5 have benefited from home management of common childhood illness through community drug distributors. Through the RRM in 2015, UNICEF has provided primary health care services to women and children in 21 counties through 35 RRM missions in Jonglei, Upper Nile, Unity and Western Equatoria States. Cumulative beneficiaries reached through RRM mission from January to date include: 127,644 children vaccinated against measles and 132,854 children against polio; 33,967 pregnant women received at least one dose of Tetanus Toxoid vaccine; and 8,830 pregnant women received clean delivery kits. Additionally, 15,365 people accessed basic health care services. The response to the circulating vaccine derived polio virus (cvdpv2) outbreak, confirmed in October 2014 and May 2015, continues as per outbreak response plan. Two rounds of sub-national immunization days (SNIDs) are ongoing in Warrap, Lakes, Jonglei, Unity and Upper Nile. Warrap and Lakes States have completed two rounds, with the second round in Warrap having reached 518,506 (98 per cent) children and 354,431children (121 per cent) in Lakes. For the three conflict-affected states, Jonglei has completed the first round with 200,675 (103 per cent) children immunized. Upper Nile and Unity are still conducting the first round. Through implementing partners (IMC, MAGNA, IOM, Medair), UNICEF continues to support provision of routine vaccination services in the PoCs and through mobile clinics among IDP populations. Two rounds of routine acceleration outreach campaigns were conducted in Aweil East and Aweil centre, with support from WHO and UNICEF, enabling the state to reach a planned coverage of 80 per cent for Pentavalent 3. Routine immunization in PoCs for children under 3

4 the age of one and new arrivals under 15 years also continued. In the reporting period, 4,519 and 2,669 children have been reached with measles and polio vaccination respectively through in the PoCs and through RRMs. UNICEF continues to support the strengthening of the cold chain system through provision, installation and maintenance of cold chain equipment. In the reporting period, eight solar fridges were installed in Western Equatoria, five in Central Equatoria and two in Warrap, with two also repaired in Central Equatoria. The country received 400,000 BCG vaccines for routine immunization and 4.4 million doses of oral polio vaccine for the National Immunization Days. Delays in security clearance for flights and communication constraints has challenged timely delivery of vaccines. Delivery at health facility still remains low. During the reporting period, 482 women delivered under skilled birth attendance. More efforts continue to be put on to strengthen community networks for health sensitisation, education and referral to health facilities. In terms of antenatal care (ANC) services in the reporting period, there were 2,166 new ANC visits recorded, including 918 women who attended their first and 582 women have attended four or more visits. Additionally, 1,169 pregnant women were counselled and tested for HIV, and 10 were identified as HIV positive and enrolled on treatment. NUTRITION: A nutrition assessment mission was undertaken to Southern Unity during the reporting period, based on the September IPC which estimated that 30,000 people could be facing catastrophe levels of food insecurity. Dablual and Jaguar/Thaker payams recorded the highest proxy global acute malnutrition (GAM) rates at 9.4 per cent and 10.0 per cent through mid-upper arm circumference (MUAC) screening. In Bauw, out of the 184 children screened, 5 were found to be moderately malnourished which represents a proxy GAM of 4.8 per cent. Only four children were screened in Bil which was insufficient to make any conclusion. It should be noted GAM based on MUAC is about half that based on weight for height measurements in South Sudan, strongly suggesting that the actual GAM rates in Dablual and Jaguar/Thanker could be at critical levels. The mortality information collected through key informant interviews showed rates below the emergency thresholds. The assessment confirms that an emergency food security situation exists in central and southern Unity. The team confirmed that the diet being consumed is not diversified as there are limited food sources and markets have been destroyed. Water lilies and fish will likely become scarce as the rivers and swamps start drying up by January, leading to very large food deficits. The joint assessment team concluded that without any food assistance, nutrition and health services and livelihoods assistance, the situation is likely to degenerate significantly in the first quarter of Between January and October 2015, 115,810 severely malnourished children have been admitted for treatment by UNICEF and partners, or 78 per cent of the target for 2015, with 60 per cent reporting for October. The majority of cases have been reported from the conflict-affected states and high burden states. During the reporting period, 2,878 children were screened; 65 children were identified with SAM and 644 with MAM, all of whom were treated. The proxy SAM and proxy GAM rates for the total children screened was 2.3 per cent and 24.6 per cent respectively. UNICEF and partners continue to increase nutrition response activities outside of Bentiu PoC. CARE has re-established three outpatient therapeutic programme (OTP) services in Rubkona and Bentiu town, in addition to one mobile nutrition service outside the PoC. During the reporting period, 12,891 children were screened in Bentiu PoC and surrounding areas of Dera A, Rubkona and Bentiu town, of whom 264 (4.0 per cent) were identified with SAM and 1,273 (9.9 per cent) with MAM. During the reporting period, 495 children were admitted to programmes to treat SAM and 790 for treatment of MAM. SAM admission rates have dropped from 678 in week 38 to as low as 212 in week 45, which is mainly attributed to the reduction in double registration of children in the camps. From January 2015 to date, through the RRM, 78,848 children have been screened for malnutrition and 919 children identified with SAM. All of these children who were not already enrolled in any existing nutrition treatment programme received initial treatment through the RRM. 71,970 children have been reached with Vitamin A supplementation and 59,044 children received deworming treatment. In addition, 35,686 pregnant and lactating women have been reached with key infant and young child feeding messages. Nine of 11 counties in Jonglei State reported screening figures for the reporting period. Based on 21,064 children screened, the state-wide proxy GAM rate was 37.5 per cent and the proxy SAM was 2.8 per cent. Proxy-GAM and especially proxy-sam rates close to 10 per cent in some areas is of great concern due to the lack of Stabilization Centres in Jonglei. There are only 14 functioning Stabilization Centres in the state despite its geographic size and population. 6 No. Of People Served Liter/Person/Day South Sudan SITUATION REPORT 19 NOVEMBER 2015 Since the beginning of the year, 41 Nutrition SMART surveys have been completed of which 38 have been validated. As shown below, the highest prevalence of acute malnutrition was reported in Bentiu PoC (34.1 per cent) followed by Mayom (30 per cent), Gogrial West (29.1 per cent) and Abiemnhom (26.5 per cent). WASH: The average water supply in Bentiu PoC has increased to 15 litres per person per day (L/p/d) through the connection of the Sector 4 network to service Sector 3 and the installation of an automatic chlorination system. The network now serves Sectors 3, 4 and 5. An additional 21 stances of latrines were completed in Sector 5, benefiting 1,050 people. This bring the number of latrines constructed through direct UNICEF implementation to 997, serving over 50,000 people. Solid waste disposal activities are ongoing. A survey was conducted of the Bentiu Town water network to facilitate the rehabilitation of Bentiu and Rubkona water systems to increase water supply for IDPs returning in these locations Bentiu PoC Drinking Water Average L/p/d World Vision and Solidarites continue to carry out their operations in Wau Shilluk. Through these partners, UNICEF is providing essential WASH services to 2,500 people including safe water, hygiene promotion and distribution of essential WASH supplies (soap, buckets, jerry cans, etc.). New taxes are now being imposed along the delivery routes, however, consultative meetings with government authorities have allowed essential supplies to be transported to Wau Shilluk over the weekend with the plans of starting construction of latrines immediately. In the Greater Pibor Administrative Area (GPAA), hygiene promotion activities have been scaled up, including cholera awareness campaigns. In Vertet, 590 people (360 women, 150 men and 80 children) were reached while in Lekuangole 1,600 people (800 women, 500 men and 300 children) were reached through these campaigns. In Kapoeta East, the epicentre of Guinea Worm infection Eastern Equatoria State, 17 new water points (1 motorized and 16 hand pumps) have been constructed in 2015, benefiting over 11,000 people in endemic villages. Cases of Guinea Worm in South Sudan have decreased by 99.9 per cent since 2006 (20,581 cases compared to only 4 from January October 2015). To support safe water supply activities in affected areas, hand pumps spare parts, water treatment tablets and collapsible jerry cans have been provided to support in the operation and maintenance of existing water points and household water treatment. EDUCATION: As of 30 October 2015, UNICEF has provided 249,033 children (42 per cent girls) aged 3-18 with Education in Emergencies (EiE) support. A total of 7,192 (1,835 females and 5,357 males) teachers, Parent-Teacher Association (PTA) members and other education personnel have been trained to effectively support EiE in conflictaffected areas. In addition, a total of 494 temporary learning spaces (TLSs) have been established, including 12 in the past two weeks. UNICEF has surpassed its targets for EiE in 2015, indicating a well-coordinated response among UNICEF, government and partners, as noted during the Joint Education Sector Review held last week but gaps remain in some geographic areas. UNICEF and partners continue to provide essential school supplies to students and teachers. Over 150 tons of school supplies (school-in-a-box kits, recreation kits, early childhood development (ECD) kits, teachers kits and hygiene kits) 6

7 have been procured and prepositioned nationally for distribution to more than 30,000 conflict-affected students during the reporting period. The availability of the education materials has been found to be a key factor in reaching new enrolments as well as keeping children in school. With support from UNICEF and partners, children in the GPAA continue to gain access to education with 11,135 students (27 per cent girls) now in school, 74 per cent of the GPAA Back to Learning (BTL) target for A small decline in attendance of four per cent was seen from September to October for Lekuangole and North Pochalla which was attributed to seasonal movement of children with their family s livestock, however, most of these children are expected to return to school once this is completed. In coordination with the GPAA Ministry of Education and WFP, provision of school meals has started in four schools (Pibor Boys, Pibor Girls, Kondako, and Langachot primary schools) in Pibor County, benefiting 1,547 children (35 per cent girls) out of a targeted of 3,000 children. The meals have contributed to a surge in attendance in GPAA. UNICEF works closely with WFP to identify schools and TLSs for support under the WFP Food For Education programme, as well as providing complementary parental education on infant and young feeding as well as health seeking behaviours. This collaboration between UNICEF and WFP demonstrates the convergence across sectors (education, child protection and nutrition) integral to BTL. In 2016, WFP has planned to include more BTL schools under its school feeding programme for a more significant impact. BTL campaigns continued in schools in Bentiu Town and Bentiu PoC, which further increased enrollment from 13,872 to 17,080 students (42 per cent girls) against the target of 20,000 children. BTL campaigning has been effective in involving communities, PTAs, government and implementing partners to reach out and encourage communities to send their children to school. The campaigns will continue into 2016, including for newly accessible areas of Unity state. UNICEF continues to establish new learning spaces but also re-open existing schools which were closed due to conflict. UNICEF and SMoE reopened Rubkona Primary School in Rubkona town which was closed for the last six months due to the upsurge in fighting. UNICEF and INTERSOS were also able to re-open Wau Shilluk West Primary School after it was also closed for six months, with 753 children (50 per cent girls) registered and 15 teachers and volunteers on board to support. The school infrastructure, including cleaning and fixing of latrines and washing points, were made possible with community support. The UNICEF Education and Child Protection teams jointly supported partner CMD to organize training on life skills, psychosocial support and peacebuilding for 97 teachers (16 per cent female) in 14 schools in Nyirol County, which will benefit 3,952 learners (32 per cent girls). Hygiene promotion trainings were also arranged for in four schools in Nyirol County reaching 2,830 children (36 per cent girls). UNICEF has received reports of two classrooms being used by military in Pulturuk primary school in Nyirol County. The occupation started in mid-october, affecting the learning of 220 boys and 133 girls. UNICEF is combating this issue by working closely with the implementing Distribution of supplies at Bentiu POC School. partner and school staff to continue providing school supplies and psychosocial support to ensure attendance of children and to avoid dropouts. UNICEF also received reports of military and civilian occupation of Pajut and Poktap primary schools in Duk County. School occupations have continued to threaten school enrolments and attendance, while also interfering with school construction and rehabilitation. CHILD PROTECTION: So far in 2015, UNICEF and partners have provided critical child protection services to 331,217 children, including psychosocial support and family tracing and reunification. Twenty per cent of this has been directly implemented by UNICEF. An additional 5,162 people were reached by partners with mine risk activities in the past two weeks, bringing the total to 111,947 this year. 7

8 As of mid-november, the total number of registered unaccompanied, separated or missing children stands at 10,939 (48 per cent girls), of which 58 per cent are separated children and 14 per cent are unaccompanied children. Over the past two weeks, 227 children were reunified with their parents, bringing the total number of reunified children to 2,922. An additional 153 unaccompanied, separated or missing children were registered this reporting period in the Greater Upper Nile Region. Currently, 8,504 cases (78 per cent of the total caseload) remain active and open, requiring regular monitoring visits, family tracing action, and/or post-reunification and reintegration support. Documented follow-up visits for these children continues to increase, reaching 57 per cent this week. While the reintegration of the 1,755 children in the GPAA continues, additional attention continues to be placed on advocacy for the release of further children across the country, in line with the Peace Agreement. UNICEF hosted an advocacy visit by General Romeo Dallaire and the Child Soldiers Initiative, during which he had the opportunity to engage with relevant senior government counterparts to discuss ways to accelerate the implementation of the Peace Agreement and map out areas for potential future collaboration. During the reporting period, UNICEF and partners continued GBV case management, psychosocial support activities, referrals, General Dallaire meets with children released from Cobra Faction in GPAA coordination, risk mitigation and capacity building, which have reached 79,477 people this year, or 99 per cent of the target for In Central Equatoria, UNICEF and partners hosted two trainings on clinical management of rape (CMR) for medical service providers. CMR capacity continues to be a major gap in South Sudan; the trainings were the first of five that will be conducted before the end of The trainings included 45 clinical staff and 10 non-clinical staff (social workers, MOH staff, etc.) of 120 targeted in Joint capacity building activities for social workers and police remain ongoing and partners continue their outreach efforts in IDP settlements and host communities around Juba, reaching a total of 297 people (133 women, 36 men, 93 girls and 35 boys) during the reporting period. In Upper Nile, UNICEF s partners provided case management and psychosocial support services for vulnerable women and girls, collaborated with other sectors on GBV risk mitigation efforts and conducted awareness-raising on GBV through house-to-house visits and open dialogues in schools, hospitals and marketplaces. In total, 4,145 community members (1,712 women, 521 men, 1,194 girls and 718 boys) were reached with GBV awareness raising activities. In Pochalla and Malakal women s centres, 736 women and girls (60 in Pochalla and 676 in Malakal) accessed group psychosocial support services. In Bentiu, UNICEF and partners provided induction training for eight community volunteers and staff and conducted 11 sessions on GBV awareness reaching 301 (200 women and 101 men) people during the reporting period. In Jonglei State, UNICEF and partners formed six new community engagement and action discussion groups, targeting 150 men and women. Each group will participate in 30 meetings to address negative social norms surrounding GBV. Twentyseven community leaders (33 per cent women) were trained on GBV basics. As part of the Communities Care programme, UNICEF and partners conducted training for 20 psychosocial and health service providers on survivor-centered care in Warrap state. Community members in Yei also conducted a round table consultative meeting with county authorities, religious leaders and other influential community members to discuss the effects of forced marriage on girls and how the issue can be addressed. This is part of a larger community-led advocacy initiative by a group of community members who are working towards preventing sexual violence through changing social norms. COMMUNICATION FOR DEVELOPMENT (C4D): The Ministry of Health of South Sudan, with financial and technical assistance from UNICEF, conducted a massive social mobilization and mass media campaign to support the third round of the National Immunization Days in the seven states. This round was combined with the distribution of vitamin A. Over 1,300 social mobilizers have been trained and deployed to conduct house-to-house visits and community announcements through megaphones and to organize orientation meetings with community, political and religious leaders, school teachers and other influencers. In addition, national and state radio stations were engaged by UNICEF to broadcast public service announcement on vaccination campaigns in several languages including English, Arabic, Nuer and Dinka. While the results of the campaign are being finalized, a social mobilization campaign is being prepared 8

9 for the fourth round of NIDs, which is planned for mid-december UNICEF also continued supporting social mobilization for SNIDs against polio in the three conflict affected states of Jonglei, Unity and Upper Nile. Meanwhile, UNICEF hosted a one-week Theatre for Development workshop in collaboration with the Ministries of Culture, Sports and Education, local and regional partners and Search for Common Ground. This is part of a three-year initiative between Zambia-based Africa Directions Theatre Company and UNICEF as one of the proposed Pan-African initiatives to use participatory theatre for community empowerment, behaviour change and social transformation. The initiative aims at influencing positive change and using participatory theatre to wider peacebuilding efforts. In collaboration with Search for Common Ground, UNICEF has already had 18 performances conducted in PoCs and host communities in Juba, Mingkaman and Bor reaching 4,500 people. SUPPLY & LOGISTICS: During the second week of November, six trucks departed Juba for Wau with 5,800 cartons of ready-to-use therapeutic food (RUTF) and 7,460 buckets. In addition, four trucks were dispatched to Bor with 4,065 cartons of RUTF and health supplies to replenish partner stocks. After a month of waiting, nine trucks left Juba on 19 November under escort to Rumbek via Mundri, carrying 30,900 buckets, 800 teachers kits, 360 school-in-a-box kits and 150 tarpaulins. This bring the total number of trucks dispatched during this reporting period to 19. Two charters were dispatched from Juba to Pibor and Yida. Educations supplies for the Back to Learning campaign were delivered to Pibor while the second charter delivered WASH supplies for construction of emergency sanitation facilities in Yida camp. Supplies continue to arrive in Juba from overseas: 14 trucks have been received in Juba containing part of the 52,350 cartoons of RUTF from Food for Peace. Nine of the 14 trucks have delivered 9,900 cartons of RUTF and the other 5 trucks carried 6,328 cartons BP-5 high energy biscuits. FUNDING: Against UNICEF s 2015 requirement of US$ million, the funding received remains at US$ 81.4 million leaving an unmet requirement of US$102.8 million. This week, UNICEF South Sudan signed a multi-year agreement of US$ 5.8 million with the Government of Norway to increase access to quality education for out of school children and adolescents. The signing ceremony which was held in Juba was attended by the Ambassador of Norway and the State Minister of Education along with other dignitaries. A five-member delegation from the US Congress visited South Sudan on 9-13 November under the auspices of the UN Foundation s Better World campaign. As part of their mission, they visited the PoCs in Bentiu and in Juba where the Congressional Delegation had the opportunity to observe UNICEF s programmatic interventions in the areas of nutrition, education, child protection and WASH.
