2 Preparedness Vision Vision The San Francisco Department of Public Health and planning partners will work towards having the most efficient, integrated and coordinated public/private health disaster response system in the country. Mission To prepare the public health, pre-hospital and healthcare facility disaster response system for a mass casualty incident or infectious disease event, whether the result of human action (e.g. terrorism) or natural event (e.g. earthquake) by:

3 Preparedness Mission Mission Coordinating a comprehensive healthcare surge system which integrates acute, sub acute, long term care and pre-hospital provider response. Leading and coordinating infectious disease control, prevention planning and policy development. Developing a robust environmental health response. Anticipating the need for a comprehensive behavioral health response. Planning for the special needs of various communities and individuals including maintaining culturally competent services during disaster response.

4 Preparedness FY Fiscal Year was marked by the emergence of a new strain of influenza and the large-scale response to this public health emergency. Two phases of this outbreak were most relevant to public health. The first phase was the discovery of the new flu strain, H1N1, and the rapid response required by SFDPH to a novel virus with an unknown severity of disease. The second phase was the continuation of initial response activities in addition to planning for and implementation of H1N1 vaccination clinics.

5 Comprehensive Surge Volunteer Health Professionals Template Outlines guidelines for using volunteer healthcare professionals in clinical settings. Lays the groundwork for a coordinated government and healthcare facility response. Adapted nationally as a best practice. Outpatient and Long Term Care Working Groups Outpatient working group launched in 2009 in partnership with the San Francisco Clinic Consortium. Long Term Care working group launched in Both groups are focused on the primary preparedness levels: personal preparedness and continuity of operations.

6 Comprehensive Surge Mass Fatality Planning Template for San Francisco hospitals to work in partnership with the Office of the Medical Examiner to respond to a mass fatality incident. The template provides both policy direction and operational direction. This template also updates the 2006 Mass Fatality EOP.

7 IDER Communicable Disease Control and Prevention Section (CDCP) was awarded a NACCHO grant to create a toolkit for businesses and organizations across the nation to work with local health departments to dispense antibiotics to employees and/or clients in the instance of a bioterrorism emergency. This toolkit provides multiple tools and training materials for interested jurisdictions.

8 IDER Advanced Practice Center In summer 2009 SFDPH was selected as an Advanced Practice Center by NACCHO. SFDPH CDCP will be developing two toolkits for health departments across the country. The first creates a template infectious disease response plan for other jurisdictions modeled on our own response plan. The second creates a pandemic influenza vaccination plan for future mass vaccination operations.

9 Environmental Health Water Contamination Planning EHS worked in partnership with SF PUC/Water Department to create an ongoing collaboration in relation to the Consequence Management Plan. This plan includes the Cryptosporidium Detection Action Plan and creates an infrastructure for future joint planning and exercises.

10 Behavioral Health Community Programs Training OPP in partnership with Community Programs and the SF Paramedic Association offered five classes for clinic staff to learn about the role of the Disaster Service Worker and receive certification in First Aid and CPR. 250 staff from throughout Community Programs and the Clinic Consortium participated in this training opportunity.

11 Vulnerable Populations 72hours.org Distribution Community Programs continued the annual distribution of 72hours.org preparedness education materials to client throughout the SFDPH clinical system. This year the event was undertaken in April and information was distributed to over 62 sites. The distribution included the following languages: 2,667 English, 804 Spanish, 898 Chinese, 96 Russian and 182 Vietnamese. Big Rumble In partnership with DEM, GSA and Committed 2 Community, SFDPH cohosted an event series aimed at providing the residents of San Francisco with the information and knowledge to prepare, respond and recover in the event of a disaster. The Big Rumble offered meaningful opportunities for hands-on experience to help make the concept of disaster preparedness and recovery more tangible to the SF community.

12 H1N1 Swine Flu H1N1 Swine Flu Surveillance SFDPH rapidly developed several surveillance systems to monitor the occurrence of H1N1 swine flu and influenza-like illnesses after the appearance of the novel H1N1 influenza strain in April Staff from CDCP led the development of these systems, trained epidemiology staff from other sections in SFDPH, and coordinated with external partners to share data. H1N1 Swine Flu Information and Guidance A success of the response was the development and dissemination of information and guidance for the following groups: Health care providers, The public, Schools and universities, Long term care facilities, Congregate living situations including jails and shelters, religious institutions.

13 H1N1 Swine Flu H1N1 Vaccine Receipt and Distribution It is estimated that 470,000 doses total of H1N1 vaccine were distributed throughout San Francisco and that 80% of that vaccine was administered. SFDPH received about 70,000 of those doses to use in primary and urgent care, within SFGH and Laguna Honda and to use in public and special clinic settings. SFDPH sponsored 27 public clinics in late October, resulting in 18,000 people being vaccinated. H1N1 Vaccine Receipt and Distribution SFDPH conducted one mass vaccination clinic in December wherein over 400 volunteers worked two different shifts and over 1,000 people were served per hour, resulting in an additional 9,000 vaccines given. Supplemental clinics for H1N1 vaccine also took place at Community Oriented Primary Care (COPC) Sites, Project Homeless Connect, homeless family shelters, domestic violence shelters, and the Adult Immunization and Travel Clinic (for immunizing active CCSF employees).
