4 HOME DEPOT FLU PROGRAM OVERVIEW In continued efforts to provide valuable benefits to our associates, we are proud to announce the 2017 Flu Program: The program will open September 1, 2017 and run through October 6, Appointments aren t necessary. Associates can visit one of the 60,000 participating pharmacies during the program. Claiming the vaccine is easy, the associate will simply need to present the pharmacist with the Flu Certificate. Copies of the Flu Certificate will be sent to locations or they can be accessed at livetheorangelife.com/flu. The pharmacist will confirm eligibility in their system using the associate s ID number and their photo ID. Each associate is eligible to receive one free flu vaccine. For more information visit the Frequently Asked Questions & Answers section of this Toolkit or they can be accessed at livetheorangelife.com/flu. 2

5 Find a Par ticipating Pharmacy Appointments aren t necessary. Associates can visit one of the 60,000 participating pharmacies when it is convenient for them during the September 1, 2017 October 6, 2017 Home Depot Flu Program period. HOME DEPOT ASSOCIATE: The Home Depot s 2017 Flu Program entitles you to one FREE flu vaccine during the time period of September 1, 2017 through October 6, There are 60,000 participating pharmacies in the program. To find a participating pharmacy near you, go to livetheorangelife.com/flu. A sample of participating pharmacies includes: You will need to fill out your name, associate ID number and sign this Certificate prior to giving it to the participating pharmacy. Bring a photo ID with you for the pharmacy to confirm eligibility. PHARMACY: Ensure The Home Depot associate has completed the following three fields: printed name, associate ID number & signature. Check the photo ID of the customer and ensure it matches the name in the completed field. Use the associate ID to confirm eligibility in the system. 9 Frequently Asked Questions & Answers 2017 Home Depot Flu Program Frequently Asked Questions & Answers What is the 2017 Home Depot Flu Program? The 2017 Home Depot Flu Program is a Certificate program that will be accepted at 60,000 participating pharmacies across the U.S. and U.S. Territories. Who is eligible for the Flu Program? All full-time and part-time associates are eligible to participate in the Flu Program, including temporary associates. Non-Home Depot associates, such as temporary agency workers and contract workers (not on The Home Depot payroll), vendors, family, friends and customers, are not eligible to participate. Each associate is eligible to receive one free flu vaccine. Can family members par ticipate in the Flu Program? No, family members are not eligible to participate in the Flu Program. However, if the associate has a dependent covered under The Home Depot Medical Plan, the covered dependent can receive a free flu vaccine under the Medical Plan if received from an in-network provider and the provider bills solely for the flu vaccine. How can associates get a 2017 Flu Cer tificate? Copies of the Flu Certificate were sent to work locations. Additionally they can be accessed at livetheorangelife.com/flu. How can associates find where to go to get a flu vaccine? The 2017 Flu Program Certificate will be accepted at 60,000 participating pharmacies, including CVS, Walmart, Sam s Club, Kroger and the Kroger Co. family of pharmacies. To find a participating pharmacy visit livetheorangelife.com/flu. When can associates get a flu vaccine? The 2017 Flu Program will run between September 1, 2017 and October 6, There will be a grace period through November 3, 2017 for any associate who missed the communicated deadline of October 6, If an associate is covered under a Home Depot Medical Plan, they are eligible to receive a free flu vaccine under the Medical Plan if received from an in-network provider during September 2017 March The provider will need to bill solely for the flu vaccine. What do associates need to bring with them to receive a flu vaccine? The associate will need to bring a Flu Certificate (completed with their associate ID number and signature) along with their photo ID. What happens if an associate misses the Flu Program? The 2017 Flu Program will run between September 1, 2017 and October 6, There will be a grace period through November 3, 2017 for any associate who missed the communicated deadline of October 6, If an associate is covered under a Home Depot Medical Plan, they are eligible to receive a free flu vaccine under the Medical Plan if received from an in-network provider during September 2017 March The provider will need to bill solely for the flu vaccine. Do associates have to get a flu vaccine? No. The Flu Program is completely voluntary. Do associates get paid for time spent going to receive a flu vaccine? No. Receiving a flu vaccine is completely voluntary and should be done off the clock. 7

The CDC recommends a yearly flu vaccine for everyone 6 months of age and older as the first and most important step in protecting against this serious disease. 11 Tips for Preventing the Flu 1 Tips for Preventing the Flu Clean your hands. Washing your hands often will help protect you from germs. If soap and water are not available, use an alcohol-based hand rub. Cover your mouth and nose. Cover your mouth and nose with a tissue when coughing or sneezing. It may prevent those around you from getting sick. Avoid touching your eyes, nose or mouth. Germs often are spread when a person touches something that is contaminated with germs and then touches his or her eyes, nose or mouth. Stay home when you are sick. livetheorangelife.com/flu If possible, stay home when you are sick. This will help prevent spreading your illness to others. Get the flu vaccine. Practice other good health habits. Get plenty of sleep, be physically active, manage your stress, drink plenty of fluids, and eat nutritious food. Clean and disinfect frequently touched surfaces at home, work or school, especially when someone is ill. 9
