4 RESTYLANE FERN PATTERN TECHNIQUE Patient Criteria Any patient who requires treatment of the nasolabial fold, marionette folds, mental crease, lip lines, cheek lines, or any other feature of the face with dynamic lines. The Fern Patter Technique involves injection of very small amounts of Restylane in a leaf-shaped pattern to fill dynamic lines or folds. The effectiveness of this techique is based on the product being injected into the dermis as a skin supporter and the gel particles of Restylane, in combination with the collagen in the dermis producing a robust, yet flexible area of skin. The physician and the patient choose whether the lines will be completely invisible after the procedure, or simply reduced by some amount. The technique can also be combined with other techniques, in order to treat static, dynamic and superficial lines. Tips and Techniques Have patients sitting fully upright when taking the before and after pictures, and sitting at a 45-degree angle during the procedure. Inject Restylane at a 90-degree angle, from within the fold itself, and inject while withdrawing the needle; the result is that the injection is sone from the line outward, and slightly downward; reduce the amount inected as the needle is withdrawn to produce the leaf-tip pattern at each injection line. Inject only a very small amount of product with each injection; inject 2-3 mm apart on one side, and then switch to the other side and make injections 2-3 apart, staggered wit the injection sites on the opposite side. When performing the Fern Pattern Technique in the mental line, first bend the needle in order to be able to inject vertically into the dermis (and test the needle to make sure the lumen is still intact) Change the needle once or twice during the procedure. Patients should not use makeup for six hours after the procedure. Products Used Approx. 1 CC of Restylane, and another approx. 0.5 CC in the touch-up session, if necessary. Can use small amounts of Perlane, injected under the Restylane Fern Pattern.
5 PAIN RELIEF Nerve Block To anesthetize the upper lip: feel the gingiva of the upper teeth to locate the canine eminence (corresponding to the socket of the canine tooth). Just lateral to it or between the 3rd and 4th tooth at the mucosal fold you should inject ml of lidocaine 10-20mg/ml. For the lower lip: locate the mucosal fold between the premolars (4th and 5th tooth) and inject as above. nerve diagram? Remember to inject slowly to reduce discomfort.
