14 tomas - Anchorage System tomas Indications The tomas pin is used as a temporary skeletal anchorage during orthodontic treatment. The various indications can be classified into three groups depending on the goal of the treatment: Active tooth movement Passive stabilization Preprosthetic treatment The secure function of the tomas pin requires a fixed anchorage in the bone (primary stability) and placement of the head in the region of the alveolar gingiva. The tomas pin can be inserted in the maxilla facially, palatally or on the alveolar ridge. It should be inserted in the mandible facially and in the centre of the alveolar ridge. Insertion on the lingual side of the mandible and in the vicinity of extraction wounds, dental follicles and first teeth is not recommended. 14

15 tomas - Anchorage System Active tooth movement The tomas -pin is used in orthodontic treatment as anchorage for numerous active tooth movement techniques. In some cases movement can be achieved which was previously impossible or only with considerable loss of anchorage. The tomas -pin can be used as reliable anchorage for the following active tooth movement techniques: Distalization of molars in the mandible and maxilla Mesialization of individual teeth or entire tooth groups En-masse retraction Sliding mechanics in Class II Asymmetric Class II Space closure in Class I occlusion Asymmetric occlusal plane 15

16 tomas - Anchorage System tomas Indications Passive stabilization The tomas -pin can be used to prevent the movement of individual teeth or entire tooth groups. If the position of the teeth is not to be changed, they can be easily and gently stabilized by connecting them to tomas -pins. Time-consuming adjustments due to anchorage loss, often necessary after completion of treatment, is no longer required.
