2 Fort Lauderdale Port Saint Lucie Sarasota West Palm Beach Broward County Environmental Protection and May 13, 2015 Growth Management Department Project No. HD Waste Regulation Section 1 North University Drive, Suite 203 Plantation, Florida Attention: Mr. Steven Schwerstein Subject: Report - Groundwater Quality Analysis s 81 st Quarter Event City of Closure Permit No. SF Dear Mr. Schwerstein: As required by the requirements of the Closure Permit for the above captioned project, the 81 st quarter event of groundwater sampling and analysis has been carried out. Transmitted herewith is one copy of the 81 st quarter event report which contains: 1. A copy of a Specific Purpose Survey for the site prepared by Paul E. Brewer & Associates, Inc. which shows locations of the monitoring wells, provides coordinate information for the wells and furnishes top-of-casing elevations as of May 30, A Groundwater Monitoring Report. 3. Laboratory analysis results for the groundwater samples. 4. A Summary of Analytes Exceeding Florida Department of Environmental Protection (FDEP) Contaminant Cleanup Target Levels per Florida Administrative Code Chapter A groundwater elevation map (surficial aquifer). Also attached are laboratory and field Electronic Data Deliverables (EDDs) that are compatible with the FDEPs Automated Data Processing Tool (ADaPT) software and the ADaPT error logs. The field sampling and laboratory analysis for the 81 st quarter event were conducted by Dunkelberger Engineering & Testing, A Terracon Company, on April 20, Sampling and analytical protocols conformed to those presented in the FDEP Standard Operating Procedure (SOP) DEP-001/01, FS A variable speed peristaltic pump was used to purge each monitoring well at a slow rate until the water level and several field parameters had stabilized. The groundwater samples were then collected using the reverse flow method (volatiles) or through the peristaltic pump (other analytes). The samples were transferred into laboratory supplied sample containers placed in an State of Florida Board of Professional Engineers Authorization No Toll Free (877)
