Thank you for your interest in the Fit For Life program! This 6 week version of our extremely successful 12 week Fit For Life program is designed to give you the opportunity to begin or keep you on your path towards a healthier, happier, you through the Holidays. Some people use the 6 week program as a trial run or preparation tool for the 12 week program which will begin in February Developing real and lasting lifestyle changes is difficult and very personal. This program is designed to use evidenced based exercise and nutrition strategies to help you reach your individual healthy lifestyle goals. Join the community and benefit from the support of a group that shares common goals and struggles! Please answer the following questions carefully and honestly. YMCA of the Northwoods believes in providing membership and program services to all who desire to participate in the YMCA, no matter their financial situation. 2 Fit For Life Questionnaire What value does maintaining good health throughout your lifetime have for you? Why do you seek to be healthier? What are you looking forward to in the near future? How do you want what you are doing right now to influence this event? How do you want to feel during this time? Give some examples of how you have tried to lose weight in the past. In one word, what is your biggest obstacle to maintaining successful lifestyle changes? Please describe the type of support you may receive outside of the program. (Include your home environment, work environment, extended family, etc.) What do you think exercise-related discomfort is, and how well do you manage it? What do you hope to get from the Fit For Life program? What do you expect from your trainers? Other than medical conditions described in the PAR-Q, please list any issues that would affect your ability to fully participate in this program (including physical, mental, or medical conditions). 3 What is Fit For Life? To get a better understanding of what our Fit For Life program is, you must first understand what it is not. The Y Fit For Life Program is not a miracle program. It is not a diet program. The program does not promise that you will lose 100 pounds in the 6-weeks. The trainers do not push you to the point of physical and mental breakdown. They do not force you to participate in any activity you are not comfortable with. The Fit For Life program is built on the idea that weight loss is a result of long-term lifestyle changes, not a 6-week fitness program. It focuses on providing you tools for implementing a healthy and fit lifestyle, while benefiting from the weight loss that comes along with this. The bootcamps offer the benefits of both cardio and strength training. The weekly wellness lectures are designed by Certified Personal Trainers and provide realistic nutrition, fitness, and goal setting tools. The program challenges each participant by offering weekly weigh-ins, workouts and challenges. The professional trainers modify each workout to accommodate all fitness levels. As a potential participant, consider the following description of the Fit For Life philosophy and training technique. When starting a new fitness program, it is important to discuss the program with your Health Care Provider (HCP). Philosophy on Training The Fit For Life program has been developed to give you simple, effective tools for lasting results. The program mission is: To educate, motivate and support you in achieving a healthier lifestyle during the program and beyond. The most successful participants utilize the trainers as coaches. Growing knowledge and skills to make an independent, lifelong commitment to a healthier lifestyle. What can the program do for you? 1. Provide the structure of a regular fitness routine 2. Improve your body s strength and endurance 3. Expose you to a variety of nutrition strategies and training techniques Signing up for the Fit For Life program does not magically give you the healthy body you ve always wanted. YOU are the only one that holds the answers to reaching your goals. The trainers guide and support you to help make your goals a reality! How Do We Train? The 6 week program is based on these main principles: Progression: The workouts at the end of the program look very different from those in the beginning. As your body gets stronger we will increase the difficulty of the workouts. It s YOUR Body: You are in charge of your own body and you are in charge of your own challenge. You know your body the best. Just because we may have developed a workout with 20 squat jumps that week does not mean you must do 20 squat jumps if your body is not able to do that. There are many ways to adjust the workouts to your body s needs. Pushing yourself is good, but pushing yourself towards an injury is not. The YMCA Certified Personal Trainers will show you modifications for the exercises if needed. It s YOUR Challenge: Many of the exercises have limitless potential. The harder you choose to challenge yourself the more challenging the exercise becomes. You become more fit and the exercise allows you to achieve more reps, yielding measurable results, and breaking through those benchmarks and plateaus. What Does a Typical Workout Look Like? Warm-up: 5-10 minutes of low to moderate exercises designed to get blood flowing, muscles warm, and joints lubricated. Theme: Introduce the theme of the workout. Examples are circuit training, interval training, pyramid strength training, cardio endurance training, and etc. Each of these themes is designed to give you a taste of all the different training techniques that are available. You will be able to take these exercises and ideas with you after the program is done. Depending on the theme we may use equipment such as dumbbells, kettlebells, exercise bands, stability balls, Bosu balls, and the step. Other workouts include types of YMCA group fitness classes like Zumba, Yoga, Pilates, Abs/TRX, Tai Chi, Indoor Cycling, Club Moves, Cardio Kick/Core Strength, Tabata Triple Threat, Rhythm & Weights, Chisel, and more. An additional set of classes include the Fitness On Demand (FOD). FOD is a virtual class system available in both studios. It offers over 200 classes from kickboxing to yoga, to dance and cycling classes around the world. Cool Down: 5-10 minutes of low to moderate exercises and stretches designed to transition the body from the workout. Page 3 of 6
4 IMPORTATNT INFORMATION REGARDING ENROLLMENT AND START DATES We will make reasonable accommodations for any of the following elements. You must ask the Wellness Director, Jenny Chiamulera, for approval to any of the requirements below. This program is a commitment and the following are expected of you each week. Please read through them carefully. Wellness Center Orientation Week: October 30 th November 3 rd MANDANTORY Introductory Meeting: Monday, October 30 th 6:00 8:00 PM At this meeting you will weigh in, meet the trainers, confirm bootcamp times, schedule a Wellness Center orientation and officially begin the 6 Week YMCA of the Northwoods Fit For Life Program. Wellness Center Orientation This is a hour long one-on-one session with a Personal Trainer. It is meant to help you get comfortable with the Wellness Center and help the personal trainers get to know you before the program begins. Fit For Life Program: November 6 th December 15 th Bootcamps: o Monday Evenings, 5:30 6:25pm o Wednesday Mornings, 5:30-6:25am *These times are negotiable at Welcome Meeting. We want times to work for YOU! Educational Lectures: o Monday Evenings at 6:45-7:45pm (After Monday Boot Camp) Weekly Workouts, Challenges, and Weigh-Ins Come in any time a Wellness Coach is on duty to weigh in, get your weekly workout, and complete your challenge. o Wellness Coach Hours are as follows Monday-Friday Mornings, 6:00am-10:00am Monday-Thursday Evenings, 4:00pm-9:00pm Friday Evenings, 4:00pm-8:00pm Saturdays, 10:00am-3:00pm PRICE: o Members: $125 o General Public: $225 Application for the program is due Sunday, October 15 th. 5 Physical Activity Readiness Questionnaire (PAR-Q) PAR-Q is designed to help you help yourself. Many health benefits are associated with regular exercise, and the completion of PAR-Q is a sensible first step to take if you are planning to increase the amount of physical activity in your life. For most people, physical activity should not pose any problems or hazard. PAR-Q has been designed to identify the small number of adults for whom physical activity might be inappropriate or those who should have medical advice concerning the type of activity most suitable for them. Please read the questions carefully and answer honestly. Check YES or NO as it applies to you. If you answer YES to any question, please explain. YES NO 1. Has your doctor ever said you have heart trouble? 2. Do you frequently have pains in your heart and chest? 3. Do you often feel faint or have spells of severe dizziness? 4. Has a doctor ever said your blood pressure was too high? 5. Has your doctor ever told you that you have a bone or joint problem(s), such as arthritis that has been aggravated by exercise, or might be made worse with exercise? 6. Is there a good physical reason, not mentioned here, why you should not follow an activity program even if you wanted to? 7. Are you over age 60 and not accustomed to vigorous exercise? 8. Do you suffer from any problems of the lower back, i.e., chronic pain, or numbness? 9. Are you currently taking any medications? If YES, please specify. 10. Do you currently have a disability or a communicable disease? If YES, Please specify. If you answered YES to one or more of the above questions then you need to sit down with your physician and discuss if this program is an appropriate plan of action for you. You must attach your Physician s Clearance to complete your application if there is a YES answer. If you answered NO to all questions there is a reasonable assurance of your suitability for exercise, however, we still highly recommend you talk with your physician before joining the program. 6 Voluntary Participation Agreement I,, desire to voluntarily participate in interviews and tasks (the Program ) for THE FIT FOR LIFE at the YMCA of the Northwoods. In connection with my participation in the Program, I acknowledge, represent, warrant and agree as follows: (Initial each line) I am 18 years of age or older. I represent and warrant that I am in good health and that I have no medical, physical or emotional condition that might interfere with my ability to engage in the Program. I represent and warrant that I am not under the influence of any medication, drugs, or other substance that might impair my physical or mental ability to engage in the Program or that might impair my judgment while engaging in the Program. I will follow all rules made and directions given by the YMCA of the Northwoods and each of their respective agents, representatives and employees (collectively, Trainers ) in connection with the Program. Trainers have no obligation to me whatsoever. Without in any way limiting the foregoing, I acknowledge and agree that trainers are under no obligation to select me to participate in the program, and I understand that I will not be paid any money or given any other consideration for giving trainers the rights listed in this Agreement or for signing this Agreement. I give permission to the YMCA of the Northwoods to use, without limitation or obligation, photographs, film footage, or tape recordings which may include my or my family s image(s) or voice(s) for purposes of promotion or interpreting. MY PARTICIPATION IN THE PROGRAM IS AT MY OWN RISK. I further understand and agree that even if I am not selected for any further consideration as a contestant on the Program, trainers shall nevertheless have the right to use my name, voice, likeness, appearance, biographical information, any information or material provided by me to trainers, and any motion or still pictures or recordings trainers take of me, in any and all media now known or hereafter devised, worldwide in perpetuity, and I hereby release trainers from any and claims, actions, damages, liabilities, losses, costs and expenses arising out of or resulting from trainers exercise of its rights under this paragraph.
