By completing a series of healthy actions, you can receive $ incentive reward at the end of the contract period. 3 Step Healthy Fit $60.00 NetCare pays up to $60.00 fitness rewards per Contract Period at either a participating or non-participating gym/fitness facility, any gym anywhere. To qualify for the reward, you must be a current NetCare member eighteen (8) years and older. Sign up directly with one of our participating gym providers and meet a minimum of eight (8) visits per month. The participating gym provider shall document and submit your monthly attendance to NetCare for recording. Healthy Fit Classes Continue your Health Steps to Wellness journey by participating in FREE Healthy Fit Classes at the Hilton Guam Resort & Spa, Kickboxing, Aqua Aerobics, Mixxed Fit, Morning Run, Pilates, Yoga, Power Yoga, and Zumba classes. NetCare members (must be at least 8 years of age) may participate. Step Healthy Actions Rewards $00.00 NetCare s Healthy Actions Rewards is a critical part of a total wellness solution. It is designed to increase health and wellness engagement for members. Members can start earning rewards when they complete a health behavior. NetCare will reward members up to $00.00 when all criteria s are met for participating in various Health Actions Programs. NetCare members must be eighteen (8) years and older to participate. A. NetCare has contracted with the SDA Wellness Center, Health Services of the Pacific, Dr. Horinuchi Wellness Clinic, Dr. Lombard s Weight management Program and Lohas Chiropractor and Nutrition Clinic to facility and provide a range of wellness and health improvement programs and activities for members. Your covered benefits also include educational and treatment programs for risk factors or conditions such as diabetes, asthma management, hypertension smoking cessation etc that may lead to serious but avoidable complications. Eligible members must satisfy all Healthy Outcome benchmarks for three () quarters within the Contract Period. Members who do not or are unable to attain or achieve health benchmarks must enroll and complete a bonifide NetCare Wellness Program showing progress of all biometric benchmarks. Upon completion of a bonifide Wellness program, members also qualify for their co-insurance reimbursement of the Wellness program.
