Coronary artery disease occurs when plaque builds up in your arteries and slows blood flow. Plaque is a buildup of fat, cholesterol and calcium in your blood.

2 The Condition: Coronary Artery Disease Coronary artery disease is a form of heart disease that affects your arteries. Your arteries are the blood vessels that bring oxygen-rich blood from your heart to the rest of your body. Coronary artery disease occurs when plaque builds up in your arteries and slows blood flow. Plaque is a buildup of fat, cholesterol and calcium in your blood. Symptoms of a blocked artery can vary from person to person, but common symptoms include pressure or pain in your chest, shoulders, arms, neck, jaw, or back. Coronary artery disease can lead to a heart attack which occurs when blood flow is completely blocked. A heart attack can cause serious health problems or even death. Blocked Coronary Artery

3 Surgery Options Your doctor may suggest medicine and lifestyle changes to ease your symptoms and reduce your risk of a heart attack. If your symptoms get worse, your doctor may suggest surgery, such as angioplasty or coronary bypass. The goal of surgery is to improve blood flow to your heart and ease your symptoms. Coronary Bypass Surgery: Coronary bypass surgery is also called CABG (coronary artery bypass graft surgery) or coronary revascularization. Surgeons use healthy arteries or veins from other areas of your body to bypass (go around) your narrowed arteries using open surgery or minimally invasive surgery. Open Surgery: During open surgery, the surgeon makes a large chest incision and cuts through your breastbone (sternum) to reach your heart. A heart-lung machine is also used. It takes over the function of your heart and lungs during surgery and allows doctors to operate on a blood-free and still surface. A newer type of bypass surgery does not use the heart-lung machine. This is called off-pump coronary artery bypass, or OPCAB. Open surgery allows your surgeon to touch your heart and tissues.

Minimally Invasive Surgery Angioplasty: With angioplasty, your doctor makes a small incision (cut) in your groin and inserts a thin tube with a deflated balloon on the end.

A small mesh tube called a stent may also be placed in your artery to keep it open after the procedure.

4 Minimally Invasive Surgery Angioplasty: With angioplasty, your doctor makes a small incision (cut) in your groin and inserts a thin tube with a deflated balloon on the end. Once it reaches your artery, the balloon is inflated and pushes the plaque outward against the artery wall. This widens the artery and restores blood flow. A small mesh tube called a stent may also be placed in your artery to keep it open after the procedure. The stent may also be coated with a drug that helps to prevent your artery from closing in the future. MIDCAB: During MIDCAB surgery (also called minimally invasive direct coronary artery bypass), the surgeon operates through a few small incisions between the ribs. 6 Risks & Considerations Related to Coronary Bypass Surgery (Mammary to Left Anterior Descending Coronary Artery Anastomosis for Cardiac Revascularization with Adjunctive Mediastinotomy): graft injury or narrowing, blood vessel connection fails requiring another operation, stroke caused by a clot that gets stuck in smaller arteries of the brain, heart failure (heart cannot pump enough blood to the body), tear in the aortic wall causes it to separate, lengthy time on a breathing machine of 48 hours or more, lengthy time for a heart lung machine, extracorporeal membrane oxygenation (outside body technique to provide cardiac and respiratory support), intraaortic balloon pump (mechanical device to increase oxygen to cardiac muscles) or other cardiac assist systems, fluid in the lungs, sudden lack of blood flow to a limb due to a block in the blood stream, valve infection, irregular heartbeat that requires a pacemaker, kidney or lung failure, sac-like cover around the heart becomes swollen and causes a low fever and chest pain for up to 6 months, bleeding disorder in which the blood cannot properly clot, heart attack, memory loss and/or loss of mental clarity, infections which may affect the kidneys, chest, valves or bladder cut in the major artery(ies) that sends blood to the pelvis and legs, pooling of blood between the chest wall and lung, pressure on the heart when blood/fluids build up between the heart muscle and its outer sac.

7 Important Information for Patients: Serious complications may occur in any surgery, including da Vinci Surgery, up to and including death. Risks include, but are not limited to, injury to tissues and organs and conversion to other surgical techniques. If your doctor needs to convert the surgery to another surgical technique, this could result in a longer operative time, additional time under anesthesia, additional or larger incisions and/or increased complications. Individual surgical results may vary. Patients who are not candidates for nonrobotic minimally invasive surgery are also not candidates for da Vinci Surgery. Patients should talk to their doctor to decide if da Vinci Surgery is right for them.
