3 The AliveCor System The AliveCor Heart Monitor is a FDA-cleared, portable, single-channel ECG recorder that snaps on to the back of most smartphones. Through the AliveECG app, you can record, display, store and transfer singlechannel electrocardiogram (ECG) rhythms. Each recording takes only seconds to set up, enabling your patient to capture arrhythmias as they occur - ideal for patients with infrequent episodes or for those in post-op care where accurate data is most critical. To record an ECG, your patient simply rests his or her fingertips on the electrodes (metal plates) of the AliveCor Heart Monitor. The patient s ECG and heart rate will display on the AliveECG app. After 30 seconds, the ECG recording is stored directly on the smartphone and sent automatically to our secure database. Patients can take as few or as many ECG recordings for as long as needed, while noting valuable information about their activities and symptoms. The Heart Monitor is easy to use and can easily become part of your patients daily routine. AliveCor Online Dashboard Patient Management AliveCor has made it easy for you to manage all your AliveCor patients in a simple to use online dashboard. Optimized for viewing on your PC or tablet, AliveCor.com is your fully secure, HIPAA compliant, web interface to all of your patients ECGs. Protected health information (ECGs) and personally identifiable information are encrypted. Each time your patient records an ECG it is automatically sent to this online dashboard for review at your convenience.

4 AliveInsights Example Reports AliveInsights Streamlined Support for Your Patients If your patients would like a quick evaluation of their ECG, AliveInsights provides access to two levels of ECG analysis. Patients have several expert analysis options to choose from and all are available 24 hours a day, 7 days a week. All communication with the review services is encrypted for security and HIPAA compliance. Your patient can share these reports with you as part of their normal care appointments or they can be reviewed by logging into AliveCor s secure web interface. This streamlined flow of information lets you more effectively manage your time and provides valuable data so you can maximize each patient appointment. Clinical Review The Clinical Review, performed by a U.S. Board-certified Cardiologist, returned within 24 hours, consists of an in-depth report including cardiac status with recommendation and finding, and an overview of your patient s activity levels and symptoms at the time of the recording. The reviewing Cardiologist may recommend further consultation with you, the patient s cardiologist. Technical Review A Technical Review sends your patient s ECG reading to a U.S. based Cardiac Technician who will return to them a report within 30 minutes or 24 hours depending on the level of service your patient chooses. This report contains a preliminary finding along with related definitions.
