5 BACKGROUND Community Food Centres Canada grew out of The Stop Community Food Centre, an innovative community organization located in Toronto s Davenport West neighbourhood. For over 30 years, The Stop has been working to increase access to healthy food in a manner that maintains dignity, builds health and community and challenges inequality. In , spurred by interest from funders and community organizations across Canada, high-level staff started exploring how to scale the model nationally. COMMUNITY FOOD CENTRES CANADA
6 NORWEST CO-OP COMMUNITY HEALTH NorWest Co-op Community Health Centre has a staff of 120 and has served the Inkster community in northwest Winnipeg since NorWest is a forerunner in taking a multi-service, holistic, team approach to health care and employs doctors, nurse, dietitians, community developers, exercise specialists, immigrant settlement workers, cooking class instructors, and early childhood educators. Our Vision: People taking control of their health. Mission: To engage our community in cooperative health and wellness.

8 FOOD INSECURITY & THE IMPLICATIONS FOR HEALTH Adults and adolescents in food-insecure households in Canada have lower intakes of milk products, fruits and vegetables; and are at an increased risk of inadequate nutrient intakes [Kirkpatrick & Tarasuk 2008]. Food-insecure adults report poor mental, physical and oral health, and chronic conditions such as diabetes, heart disease, hypertension, depression, and fibromyalgia [Che & Chen 2001, Muirhead et al. 2009, Guccardi et al. 2009, Vozoris &Tarasuk 2003]. Household food insecurity affects people s ability to manage chronic health problems such as diabetes. As a result, food insecurity takes a significant toll on the healthcare system [Tarasuk et al 2015]. Multiple episodes of hunger among children over time are associated with higher odds of chronic conditions and asthma [Kirkpatrick, McIntyre & Potestio 2010]. Health implications in NorWest s catchment area in Winnipeg: Fastest increase in diabetes prevalence in Winnipeg, 32% increase over last 5 years (Manitoba Centre for Health Policy, 2009). Rates of diabetes at 13.1% in the Eastern portion of the community. Highest rates of hypertension in Winnipeg at 29% (WRHA).

14 JOURNEY TO PARTNERSHIP Community experiencing extensive food insecurity, chronic health and social isolation issues. A building was left vacant left when NorWest moved into a larger clinic. This 4,000 sq. foot space was renovated to become the CFC, drawing on lessons/site design from other CFCs in Ontario. The CFC opened in March 2015 and is part of the community development department at NorWest Programming direction flowed from our community consultation and was guided by best practices from other established CFCs in Ontario. Collaboration with other NorWest departments happens across programs ranging from consultation, to co-facilitation of programs/special events, referrals both to and from the CFC, and hosting primary care services at our site. Through this manner of working we leverage expertise in the areas of social work, primary care, dietetics, youth programming and immigrant settlement and in turn build expertise around food security at the CHC organization as a whole.

18 WHY CHCS AND CFCS WORK WELL TOGETHER Aligned values of social justice, health equity, and a community development approach CHCs use a social determinants of health approach- and food security is a critical determinant of health CHC and CFC services reinforce each other. CFCs are a gateway for people to receiving primary health services (ie- people will come for a meal then get connected to health services) and vice versa. CHCs provide a strong organizational framework and the infrastructure necessary to maintain a CFC CFCs offer opportunities for CHCs to increase their public profile through fundraising and communications channels CHCs and CFCs are committed to delivering the highest quality health and social services and using a strong evaluation strategy to maintain them COMMUNITY FOOD CENTRES CANADA

20 EVALUATION STRATEGY Healthy food access: Kilograms of fresh produce harvested; fruit & vegetable consumption; healthy meals served Healthy food skills and behaviours: Fruit and vegetable consumption; regularity of cooking healthy meals at home; basing food choices on nutritional & other health considerations; perceived physical & mental health Social inclusion: Volunteerism by participants; number of successful referrals; level of social support; new friendships made; sense of community belonging Civic engagement: Feelings of self-efficacy; contacting media and politicians Strengthening the local food economy: Money spent on local, sustainable food COMMUNITY FOOD CENTRES CANADA
21 First Year Evaluation Results NorWest Community Food Centre Impact: -93% of participants identified NorWest CFC as a source of healthy food -78% of participants reported making changes to their diets due to CFC programming -45% have noticed improvements to their physical health -97% felt that they belonged to a community at the NorWest CFC -75% reported improvements in their mental health Socio-demographic information about our participants: -48% of participants worry about not having enough for food after paying rent and bills -55% receive government assistance -60% of participants attend more than one program
22 IN THEIR OWN WORDS Without this centre I would be a very sick, unhealthy person. This is a really good support and people do need that, especially if they're sick. Here, it promotes that kind of nice community with health and good food Without this place I would be at home and not leaving my house. Social anxiety - this has pulled me out of it. We catch the bus here for every lunch. I'm glad we caught on to this place. I leave here feeling so good!" My stress level is down considerably. There's always food here too. When I was unable to work, money was an issue. Going to a food bank, you feel stigmatized like you've done something wrong. It s not like that here.

Offers resources, customized training, grants, and chances to network and promote shared priorities. Working together through a set of shared principles, the initiative connects like-minded organizations across Canada in a collective commitment to achieving a healthy and fair food system. 28 KNOWLEDGE EXCHANGE Almost 4,000 individuals and organizations have accessed the more than 150+ modules and resources on our Pod Knowledge Exchange More than 2,500 people have attended 12 webinars on everything from program evaluation to food justice to nutrition education CFCC and CFC staff gave more than 50 talks and presentations in 2015 Second annual Food Summit in April, 2016 brought together more than 100 CFC and GFO staff from across Canada COMMUNITY FOOD CENTRES CANADA
