1 Newsletter Autumn 2016 Introduction As we move into Autumn and a traditionally busy time of year for us all, I felt it important to reflect some of our successes to date and highlight some of the commissioning initiatives we are undertaking as a CCG and how we are working with our key partners. One area that we have been focusing on is the launch of our Fit for Frailty project which you can find an in depth feature on in this issue. As a CCG we value the contribution that our stakeholders and the public make on each piece of work that we do. I am really pleased to report that another of our CCG practices has been rated as outstanding in their recent CQC inspection report. It really highlights that the work our practices are doing locally is some of the best in the country. Thanks to everyone who provided feedback on our commissioning intentions which we launched during the last Have Your Say Day. We recently approved the content of these and they are now available on our website. Have Your Say Days give us an opportunity to go out and meet the very people that we commission services for and help us to gain valuable insight into the work that we are doing. 2 South Warwickshire GPs achieve outstanding scores in patient satisfaction survey GP practices across South Warwickshire have bucked the national trend across all criteria in a recent GP patient satisfaction survey. Local doctors were recognised for delivering exceptional patient satisfaction over the period July 2015 to March 2016, with an overall GP surgery experience result of 90%, five points ahead of the national average. Most South Warwickshire practices were ahead of the average, with Lapworth Surgery at the top of the class with an extraordinary 100%! Over half of the South Warwickshire practices are leading the way in ensuring patients can get an appointment, with improvements in their scores recorded over the last three years. Patients are particularly pleased with the ease of getting through to a GP surgery on the phone, which has remained consistently strong over time. Other practices who are making great strides demonstrated by their high performances include the Arden Medical Centre in Stratford-upon- Avon where 99% of their patients agreed that the surgery receptionists were helpful. This was twelve points ahead of the national average (87%). An astounding 96% of patients at St Wulfstan Surgery in Southam said they did not have to wait too long to be seen, considerably higher than the national average of 58%. Patients rated 100% confidence and trust in GPs at several south Warwickshire practices, including the New Dispensary Surgery in Warwick, Trinity Court in Stratford-upon-Avon and Hastings House in Wellesbourne. The overall South Warwickshire result for level of confidence with their GP was a resounding 98%, pushing on further from an already high level of 97% since And at the same time another two practices in south Warwickshire have been awarded an Outstanding by the Care Quality Commission. St Wulfstan in Southam received their rating following a report published in September and Waterside Medical Centre following a November report. They join Shipston Medical Centre, The Castle Medical Centre and Trinity Court Surgery who have all been rated Outstanding bringing the south Warwickshire total to five. 3 Fit for Frailty - July 2016 & beyond Building on a combination of the success of last year s Over 75 s scheme and best practice evidence from the British Geriatric Society the CCG is proud to launch the Fit for Frailty programme. The programme comprises of three key components: - Prevention and early intervention - Acute care management - Managing our most poorly patients. All of these components are supported by foundation projects including training and education for all involved, ensuring accurate and timely reporting and the core principle of effective care planning. The Fit for Frailty programme is also underpinned by some core principles: - Focus on patient care by reducing the administration burden on clinicians and GP practices - Collaboration across health and social care and the voluntary sector to deliver joined up services for patients - Robust analysis to support the measurement of success - Ultimately align with the Out of Hospital programme as per the CCG Strategic Plan, Commissioning Intentions and the Sustainability & Transformation Plan. So what are we doing and what are we planning to do? The diagram below sums up our work plan for the next 21 months. Medication review i.e. STOPP START criteria. Implementation of EPaCCS Agreed KPI s and evaluation GP support/resource. Training to support identification and patient management Agreed assessment tools - My Care Plans - (routine, escalation and advance) Shared Records inc. My Care Plans 3
4 For the mild frailty component we have just provided data to all our GP practices on their performance for universal screening and immunisation programmes. In the moderate frailty component we have once again launched three pilots and this part of the programme is based on last year s Over 75s scheme. Making some changes to the pilots from the lessons learnt, this year it applies to any patient over the age of 18 that is considered to have frailty. Alcester Health Centre has joined forces with Pool Medical Centre to run a local scheme using a care co-ordinator to support their moderately frail patients. Arden Medical Centre, Meon Medical Centre and St Wulfstan Surgery are continuing with their work which is very much targeted at reducing social isolation and continue to build on the relationship established with Silverline. The remaining 31 practices supported by the South Warwickshire GP Federation (SWGP) are continuing to provide targeted clinical checks, (which are now 30 minutes) and are working with Age UK to deliver any non-medical interventions for this patient group. The largest change, when compared to last year, comes in the form of the severe frailty component. 19 of our practices are signed up to participate in a local scheme to support our most poorly and complex patients. A comprehensive 60 minute review to produce MY Care Plan will be undertaken with the GP and then used as the basis to co-ordinate care from all the other appropriate organisations. The remaining practices are continuing to provide an alternative service through a national scheme. All those participating in the Fit for Frailty programme are very keen to improve patient care and co-ordination between health and social care and feedback on the Fit for Frailty projects will become a regular element of future newsletters. 43
5 Commissioning Intentions Much has been achieved over the last 12 months but the CCG recognises that much more work is required to deliver the desired transformational change as laid out in our strategic plan. We have recently, following a period of consulting with partners, patients and the public, completed our commissioning intentions. The commissioning intentions for 2017/18 not only progresses our current local strategy, but puts in place the building blocks for a simplified system in Coventry and Warwickshire. This means we will not only be addressing the triple aim for our local population but we will be contributing to the sustainability and transformation of the Coventry and Warwickshire system. Our ambition is to move to jointly commissioned arrangements with Warwickshire County Council for learning disability services and long term mental health conditions. To ensure that our most expensive and limited resource is used as efficiently as possible and we deliver the best outcomes in the most cost effective ways We will continue to make sure that the standards and processes are in place to ensure timely access to care. We will deliver continuous improvement in quality and patient outcomes. 6 Health and Wellbeing Boards join forces The Health and Wellbeing Boards of Warwickshire and Coventry - whose membership includes the areas councils, clinical commissioning groups, NHS Trusts, voluntary and patient associations, emergency services and others - came together at a special event at the University of Warwick on Thursday 13th October to sign the new Health and Wellbeing Alliance Concordat. At the heart of the agreement is a focus on enabling people living in Coventry and Warwickshire to pursue happy, healthy lives by focusing on what s best for the public rather than individual organisations, streamlining decision making and creating a health and wellbeing system that is easy to understand. This commitment will form the basis for a new, joined up approach to service delivery. It signals the commitment of all the partner ozrganisations to the transformation of services and an unprecedented need for greater integration of services across Coventry and Warwickshire over the next five years. Councillor Izzi Seccombe, Chair of the Warwickshire Health & Wellbeing Board and Leader of Warwickshire County Council, said: This is a momentous step for Coventry and Warwickshire working together around the health and care needs of our people and our shared place and I am excited to be going forward together. Councillor Kamran Caan, Chair of the Coventry Health & Wellbeing Board and Cabinet Member for Public Health and Sport at Coventry City Council, said: People and communities are at the heart of everything we do and creating a partnership like this is going to help shape better futures for those that we want to support. This agreement allows us to work closer together to create a better system that improves the health, wellbeing and overall happiness of people and families across Coventry and Warwickshire. 7 CAMHS - redesign and transformation Children s and Adolescent Mental Health Services (CAMHS) across Warwickshire will benefit from a redesign that is being led by a joint group of commissioners and stakeholders including Coventry and Warwickshire CCGs, local authorities, public health, and parents. A Redesign Project Board has been established and has overseen a comprehensive co-production process between all stakeholders which has led to the development of a new outcomes framework for children and young people s mental health service for Warwickshire and Coventry. The process was led by Young Minds who engaged with young people and their families. Over 750 young people, parents, carers, and professionals who either provide or refer into CAMHS contributed to the development of the new outcomes framework. There was overwhelming consensus as to what the local children and young people s mental health system should aim to deliver and this included; 1. A focus on prevention and early intervention 2. Building the resilience of children and young people 3. Integration between mental health and other services, particularly education and schools 4. Working systemically with the family and network around the child and young person 5. Providing child and young people s mental health services from birth up to 25 years 6. Delivering a tier-less service 7. Ensuring support for vulnerable children and young people Since March the team have been busy developing joint commissioning agreements under section 75, finalising the service specification for a redesigned service and ensuring that the financial envelope is robust. The service is now out to tender with the potential new contract go live date of 1st August
8 Warwickshire MASH The Multi Agency Safeguarding Hub (MASH) is a partnership between Warwickshire County Council, Warwickshire Police, National Health Service (NHS) and other key partner agencies working together to safeguard children, young people and adults. The MASH was launched on 7 May 2016 and mirrors a practice that is being rolled out elsewhere in the country. The MASH was launched on 7 May 2016 and mirrors other models being rolled out elsewhere in the country. The children s service is now live and the adult s service will launch on the 1 September. The MASH will be based at the Saltisford building in Warwick and will see the sharing of information in relation to safeguarding made easier and will facilitate a far quicker response. In terms of safeguarding, this is a tremendous stop forward. As a result, the agencies will be able to act quickly in a co-ordinated and consistent way, ensuring that children and adults are kept safe. The MASH will involve representatives from the Warwickshire County Council Social Services, Police, Health, Probation and other key agencies working together in the same location. 9 Annual General Meeting We held our third annual general meeting on 21 September 2016 at our offices at Westgate House, Warwick. The meeting is designed to give all our stakeholders an opportunity to hear about our achievements over the last year and to learn about our plans for the year ahead. The event is also an opportunity for attendees to engage with local partners and providers who offer a range of services that are commissioned by the CCG. This year we held a market place event to allow a number of services to engage with the public. This included Myton Hospice, Rethink Mental Health, Age UK Warwickshire, South Warwickshire GP Federation and Healthwatch Warwickshire. Ahead of the meeting Dr David Spraggett, Chair of NHS South Warwickshire CCG and Kenilworth GP, said: I am keen to meet the public during our third AGM. We have undertaken a rigorous programme of work over the last twelve months and we have much more to do over the coming year. I am looking forward to sharing details of our achievements and plans at this event. The theme of the day was Commissioning In Action and the highlight was a ten minute video featuring a number of our partners talking about work that we have commissioned over the last twelve months. Primary Care Awards The first ever awards celebrating success in primary care was held at the Stratford Manor hotel with over 200 attending the memorable occasion. The awards included seven award categories to showcase care across south Warwickshire and were a celebration of care within the primary care setting. Categories included: Excellence in Medical Training Award; Excellence in Prescribing Award for Quality and Safety and an End of Life Care Award. NHS South Warwickshire Clinical Commissioning Group Chair and local GP, Dr David Spraggett and presented an award for the Excellence in Prescribing Award for Quality and Safety said I was honoured to be a part of this inaugural event which champions the great care that is being offered to the public in south Warwickshire. 9
10 Engagement The CCG seeks to continually increase the ways we engage with the public. South Warwickshire Health Champions are invited to participate in a variety of ways and they can play as big or small a part as they wish. The CCG has been taking all opportunities to increase the numbers of Health Champions. So far we have visited Kingsway Community Primary School, Warwick Library, Vision Support Group in Leamington Spa, Southam Library, Royal Priors shopping centre and many more, to encourage people to sign up and get involved. Dr David Spraggett, Chair of NHS South Warwickshire CCG said: I am passionate about the public being involved with their local NHS. It s a service many of us have access to and use, and often have a comment on. By becoming a South Warwickshire Health Champion local people have the opportunity to have your voice be heard and be involved in the important decisions that we have to make. I d encourage everyone in the district to sign up and be heard. Upcoming plans - engagement events South Warwickshire CCG is committed to making sure that primary care maintains its status as the lynchpin of care delivery in south Warwickshire. Therefore, we have planned a number of engagement events over the next few weeks which will help us to develop the future model for primary care. We are keen to know views in primary care and now and how it could look in the future. A requirement of this is that we will engage with our population to understand what they want and expect from primary care both now and in the future. From that engagement we will produce an independent report that enables us to understand the key requirements of primary care from the perspective of our Member Practices and population.
