3 Roles and Responsibilities 1) FAMILY MEMBER/CAREGIVER: Seeks medical treatment for ill family member or friend Is part of the treatment team Educates oneself about brain disorders and medications Keeps records and informs other team members of changes and concerns Attends discharge planning meeting Takes very good care of self 2) FAMILY PHYSICIAN Provides complete medical assessment, diagnosis and treatment Consults with family for collateral information May refer to Psychiatrist May refer to Pediatrician if under 18 May refer client and family to Mental Health & Addictions or to Child & Youth Mental Health Monitors medications and mental status with input from family/caregiver Is team member for discharge planning Provides long term follow up May refer to BC Children s Hospital or Prince George Adolescent Unit 3) PEDIATRICIAN Provides complete medical assessment and treatment if under 19 Consults with family for collateral information May refer to Psychiatrist Refers to BC Children s Hospital or Prince George Adolescent Unit, for assessment when needed Refers child and family to Child & Youth Mental Health 4) PSYCHIATRIST Provides consultation for diagnosis and treatment to client, family physician, hospital staff, mental health staff and family Provides short term follow-up May be team member for discharge planning 5) MENTAL HEALTH & ADDICTIONS Accepts self referral or referral by family/friend or family doctor Provides clinical assessment, group and/or individual treatment and discharge planning including geriatric individuals Provides free counselling and education and support for youth, adults or families around substance use and/or problem gambling Provides psychiatric consultation Consults with family for collateral information Provides information on financial assistance for psychiatric medication through the BC No-Charge Psychiatric Medication Plan (Plan G) Supports and educates families and the public Advocates for client and family

Children s Hospital, Maples Adolescent Treatment Centre or related programs such as residential services Liaises with Mental Health & Addictions for smooth transition between Child & Youth and Mental Health & Addictions

5 11&12) COMMUNITY AND SCHOOL COUNSELLORS Provides counselling Advocates for client and/or family 13) TDCSS ADDICTIONS SERVICES Accepts self referral or referral by family doctor Provides free counselling and education and support for individuals or families around problem gambling. Provides fee for service counselling, education and support for individuals or families around substance use. Provides free counselling, education and support for First Nations individuals or families around substance use. Provides assessment Provides referrals to Mental Health & Addictions when indicated Establishes treatment plan Provides referrals to treatment centre 14) EMERGENCY SHELTER AND HOSTEL Provides short-term housing for adult men and couples, 19 years and older 15) K SAN HOUSE SOCIETY Provides victims services, counselling, emergency shelter for women and children 16) TAMITIK STATUS OF WOMEN /ANTI-POVERTY Provides women's & children's counselling; outreach services for women Provides emergency shelter for women and children Provides Anti-Poverty o Assistance with monthly products - diapers, formula and feminine hygiene o A furniture recycle/exchange program o Angel Tree - a Christmas gift program for children o School Supplies 17) KAIEN ANTI POVERTY Provides help in applying for income assistance and disability Provides help in filling out income tax forms Provides help with residential landlord and tenancy issues Provides a free store 18) FRIENDSHIP HOUSE ASSOCIATION Aboriginal Mental Health Program-Provides referrals, information on community resources, advocacy and workshops Aboriginal Hospital Liason Worker-Provides information on community resources and advocacy 19) NORTH COAST TRANSITION SOCIETY Provides victims services, counselling, emergency shelter for women and children 20) PRINCE RUPERT COMMUNITY ENRICHMENT SOCIETY Provides family skills and support programs

6 21) INCOME ASSISTANCE & DISABILITY ASSISTANCE Client to first complete application for Income Assistance immediately after receiving diagnosis After acceptance on Income Assistance, complete application for Disability Assistance: o one part for the individual to fill out o one part for the doctor to fill out and o one part for Adult Addictions and Mental Health to fill out Mental Health & Addictions or Anti-Poverty can help with application process If a person has been employed previously, they can apply for Disability Benefits through Canada Pension Plan 22) CANADA PENSION PLAN DISABILITY BENEFITS Client may apply if has been previously employed Phone or go on line for disability assistance application kit which includes: o an application form o a questionnaire about work history and medical conditions o a medical report to be filled out by the a doctor and o a consent form for CPP to get additional information This process can take up to 6 months If an individual is unable to apply on their own another person may apply for that individual 23) TRIUMPH VOCATIONAL SERVICES Provides services for clients interested in developing life and work skills, finding a job or volunteering in their community 24) TRANSPORTATION Provides low-cost medical travel for Northern B.C. patients to and from appointments within province 25) BC PARTNERS FOR MENTAL HEALTH AND ADDICTIONS INFORMATION Provides resources on brain disorders and addictions 26) REACH OUT Moderated Online Support Group.
