2 Welcome The Kensington and Chelsea and Westminster Memory Service is a specialist service for people experiencing memory difficulties. Memory difficulties can be frustrating and worrying. We offer a comprehensive assessment of your memory. We provide useful strategies and treatments to help you to minimise your memory difficulties and to help you live independently and safely. We are a team of health and social care professionals all with expertise in working with people with memory difficulties and dementia. The team includes an occupational therapist, nurse specialist, social worker, psychologist, psychiatrist and dementia advisor. We also have close links with the Admiral Nursing Service. Who is this service for? Residents of the City of Westminster and of the Royal Borough of Chelsea and Kensington. People with concerns about their memory People with memory difficulties/ dementia who would benefit from the services we offer.
3 You may be finding it difficult to remember things on a day-to-day basis and you may be experiencing some of the following things: Forgetting dates and appointments Getting in a muddle with your finances, paying bills etc. Losing things around the house Finding it harder to remember names Difficulties following conversations What do we offer? We offer an assessment of your current memory abilities. When this is completed we will give you feedback on what we have found. We then offer you a wide range of support and interventions to help you minimise the difficulties you are having so that you can live as independently and safely as possible. We also help you to plan for the future. How do I get an assessment? You can contact your GP and ask to be referred to us. Your GP may want to take a blood test at this point to rule out any other health concerns. or You can contact us directly to refer yourself (see back page for our contact details). We may still ask you to see your GP to rule out any other health concerns.
4 What happens next? We will phone you to make an appointment at a convenient time for you. We will then visit you at home to complete our assessment. We can arrange for you to come to see us if you prefer. We always ask that you have a relative or friend with you when we visit so that they can support and help you during the visit. We will ask you about your health, how you are currently managing and what your concerns are. We will also assess your memory using a standard set of questions. After this we may request that you have a CT or MRI scan and a blood test to help us make a diagnosis. When we have all the information we need, we will make a diagnosis. We will explain this to you and give you lots of opportunities to ask questions. We will work closely with you and your family to prioritise what you need and what you would like to achieve.
5 We can offer: Strategies to help you manage your memory on a day-to-day basis. Talking therapy to explore your worries, concerns and feelings. Groups where you can meet other people in a similar situation and can share tips and ideas. Some people may benefit from medication which we can prescribe. Information, advice and guidance that can help you now or in the future. We also work closely with family and carers to provide support, advice, information and education on memory problems and dementia. This is both practical in nature and supportive to help you deal with your feelings and emotions.
