2 1. Introduction and Purpose of Document The Scottish Government has decided that healthcare and forensic services for people in police care should, from 1 April 2014, be provided by NHS Scotland within joint NHS / Police Scotland local and regional partnerships. The provision of healthcare in custody suites, and for children and adults in police care is a statutory responsibility of NHS Boards. The provision of forensic services is, on the other hand, a statutory responsibility of Police Scotland. In practice, a clear separation of the provision of healthcare and forensic medical services is often difficult and impractical. The Scottish Government has therefore decided that NHS Scotland should provide appropriate forensic medical services, which require forensic clinical skills, on behalf of Police Scotland within joint local/regional NHS / Police Scotland partnerships. Responsibility for forensic medical services will remain with the police even although delivery will be a matter for the NHS. This is a significant change over previous arrangements in which the 8 Scottish Police Forces procured healthcare and forensic medical services for people in custody from a variety of sources including independent Forensic Physicians. The management of the change requires robust governance, service involvement and project management throughout to ensure successful implementation. Close working between NHS Scotland and Police Scotland is essential. After implementation, a vehicle to support networking across geographical, multi agency and professional boundaries will be needed in support of local partnerships to ensure equity, mutual support and service co-ordination across the different agencies and professional groups involved. The purpose of this document is to clarify the overall network structure; its governance arrangements; the membership and the function, role and responsibilities of each proposed group, and of individual members within groups / subgroups; the reporting structure between groups; and the escalation procedures. 2

National Co-ordinating Network for Healthcare and Forensic Services for People in Police Care 2.1 Remit The overall remit of the Co-ordinating Network is to: facilitate and help to ensure partnership arrangements are in place for the delivery of forensic and healthcare services for people in police custody in all NHS Boards/Regional Collaboratives by 1 April 2014 and that the responsibility for delivery is with the NHS Boards by this time. oversee the transfer of funds from Police Scotland to NHS Scotland by 1 April extend the work in local / regional partnerships to improve health, reduce care inequalities, and integrate care within community pathways. extend partnerships to encompass areas beyond healthcare and forensic services for people in custody suites, to include other people in police care for example child sex and serious physical abuse, adult sexual assault. support the development of the models of care to achieve a cost effective, high quality service at local, regional and national level. establish quality standards for the delivery of care in appropriate facilities. establish consistent data collection, audit and research that will develop an evidenced informed approach. develop the workforce, training and education based on competency frameworks. form appropriate links with other relevant Networks such as the National Prisoner Healthcare Network and the Forensic Mental Health Network, and to ensure minimal duplication of effort. 2.2 Ways of working The Network will achieve its remit through the establishment, co-ordination and support of a network structure which allocates specific areas of responsibility to a range of topic specific, and geographical subgroups, while providing overall strategic direction through a national Governance Board; and co-ordination of the work of the network through a National Operational Support Group. This will work in a collaborative/joint working approach. 3

4 2.3 Network Governance Structure The proposed structure is illustrated in the diagram below. It comprises: - A high level strategic Co-ordinating Network Board involving senior representatives of NHS Scotland, Police Scotland, Scottish Police Authority, Crown Office Procurator Fiscal Service, Social Work, Scottish Government Health and Social Care and Justice Directorates - An Operational Support Group bringing together the chairs of the network sub groups along with clinical / professional representation - 3 regional collaborations overseeing and co-ordinating implementation of the change and ongoing provision of services through local NHS / Police Scotland partnerships at NHS Board level - A range of standing delivery support groups to provide ongoing support and co-ordination of specific aspects of service delivery and to lead and support a process of continuous quality improvement to improve the quality of healthcare and forensic services and maximise health gain - A range of topic specific workstreams commissioned for a short period to address a specific task 4

Roles and Responsibilities of the Various Network Groups This section sets out the remits, roles and responsibilities, membership, meeting cycles, current status, reporting and escalation arrangements of the standing groups within the network. In addition a range of topic specific workstreams are required on a short term basis to resolve issues to support implementation of the transfer of responsibility. 3.1 National Co-ordinating Network Board for Healthcare and Forensic Medical Services for People in Police Care Remit The remit of the Co-ordinating Network Board is to: provide national strategic direction to the Co-ordinating Network; monitor progress against the overall objectives of the network and direct action to ensure the achievement of network objectives (set out in section 2 above); monitor service delivery and ensure that it meets needs, and is delivered clinically and cost effectively, equitably across Scotland; monitor the quality of services delivered and their impact on health and community safety and provide direction on service improvement; monitor the development of the workforce, training and education based on competency frameworks; ensure that appropriate links are made with other relevant Networks such as the National Prisoner Healthcare Network and the Forensic Mental Health Network to avoid duplication of effort; ensure that, collectively, individuals are getting access to the appropriate health and care services, as required Meeting Cycle The Co-ordinating Network Board will meet quarterly. 8 3.2 Operational Support Group Remit The remit of the Operational Support Group is to: support the Co-ordinating Network Board by: co-ordinating and overseeing the work of the network subgroups to implement the decisions of the Network Board and the achievement of Network objectives; providing a forum for interchange and linking between the Network subgroups; assisting in the design and continuing delivery of services; providing national advice and consistency of approach, as required; supporting the local partnerships in respect of delivery; monitoring and reporting on progress in implementing Network objectives; monitoring and reporting on service quality, cost effectiveness, health and community safety improvement; gathering and presenting information to the Network Board; sourcing and providing advice for the Network Board and providing the secretariat for the Board. form appropriate links with other relevant Networks such as the National Prisoner Healthcare Network and to ensure minimal duplication of effort. 10 Most members attend in the capacity as chairs of topic specific or service delivery subgroups or regional collaborations. In this capacity, members are expected to report on the work (and raise issues of concern) of their subgroups / regional collaborations to the Operational Support Group and to work together with other members to agree joint coordinated action within subgroups to achieve Network objectives. Members are responsible for ensuring collaborative, co-ordinated, focussed action across all subgroups and regional collaboratives. Operational Support Group members are collectively responsible for agreeing work plans and priorities for each subgroup. Subgroup chair members are appointed for 2 years (or the duration of their term of office as chair of the groups they represent if shorter) see right hand column above. Each member is responsible for ensuring two way communication with the groups they are representing to ensure that they can represent relevant interests effectively. Professional Advisers are appointed in a personal capacity for a 2 year term of office to provide expert advice. Status The Operational Support Group is now established and is meeting every quarter. Reporting and escalation of issues to: The Co-ordinating Network Board, with governance oversight by the Co-ordinating Network Chair. Regular updates in the form of a newsletter will also be provided to all participants in the Network from NHS Boards, Police Scotland, Crown Office, Social Work Departments and Scottish Government (Health and Social Care and Justice Directorates). 10

11 3.3 Forensic Medical Services Groups Specialist Paediatric Forensic Service Delivery subgroup Remit The Specialist Paediatric Forensic Service Delivery Group will cover severe complex abuse. The remit of the subgroup is to: Provide strategic support to local NHS / Police partnerships to deliver joint specialist paediatric forensic services for children (up to the age of 16, or those under 18 who have additional needs or who do not have capacity) who may have been sexually abused or have suffered complex abuse, in line with NHS Quality Strategy. Work with the three Regional Managed Clinical Networks (which deliver services for child sexual abuse) to develop a standard Scottish wide service specification for child sexual abuse (CSA) and complex physical abuse in order to support the delivery of NHS/ Police Partnerships.This specification should outline best practice, documenting standard protocols for processing referrals, equipment requirements, process for investigations and minimum training and education requirements for Paediatricians and Forensic Physicians Support the implementation of the recommendations of Forensic Paediatrics A Scottish Government report by the Short Life Working Party 2013 as well as any other relevant publications. Support the NHS across Scotland to develop new models for the provision of trained clinicians; Paediatricians and Forensic Physicians, with the relevant qualifications, knowledge and experience, in agreement with COPFS and Police Scotland Consider developing a national quality improvement programmes for specialist paediatric forensic services Develop specialist paediatric forensic services in line with the Children and Young People Act using multi-agency care planning around Getting It Right for Every Child (GIRFEC) Principles Outputs Quality Indicators Update to National Guidance Meeting Cycle The Specialist Paediatric Forensic Service Delivery Group will meet every 3 months. 28 3.11 Cross Cutting Workstreams Adastra IT Users Group Remit To ensure that the healthcare and forensic medical system for people in police care (Adastra) is maintained and further developed to support the needs of the users. To do this, the group has responsibility for three key areas: A. Change requests The Group will: Assess and evaluate change requests Approve Change Request prioritisation, seeking advice and further information from relevant user groups where necessary Consider the issues and risk associated with change requests Approve change request classification Assess and evaluate change requests and consider their impact on all aspects of the system and programme Provide the relevant oversight group (tbc) with recommendations and prioritisation of change requests where there is a cost associated with the change Provide a recommendation regarding the priority of the change request in order to influence and guide the relevant oversight group (tbc) where there is a cost associated with the change Submit change request matrix to the relevant oversight group (tbc) for the changes that required funding. B. Monitor performance of the Adastra system Ensure that significant incidents or system errors are reported and discussed, sharing lessons learned across local programmes. Report issues Collate and evaluate risks and issues to the system and their impact upon the programme. C. Develop an Adatra User Manual Develop a User Handbook for the use of Adastra with the objective of ensuring consistent use in case question sets across Scotland. 31 3.12 Cross Cutting Workstreams DRAFT Telehealth Short Life Working Group Remit & responsibilities of the group To scope the feasibility of developing telehealth services for the delivery of healthcare and forensic medical services across Scotland. The group should look at links between healthcare professionals only. Police Scotland staff, whilst a key partners to support and enable healthcare delivery, should not be involved in the delivery of healthcare. To do this, the group should consider the following questions: 1. Is there a potential need to use telehealth to support the delivery of police custody healthcare across Scotland? 1.1 If so what sites are the priority? 1.2 What would this look like in practice? Eg Nurses and Drs with expertise in healthcare in police custody linking in to other less experienced healthcare professionals in custody suites? From other Boards or other sites within a Board? 2. Is there a potential need to use telehealth to support the delivery of forensic medical services across Scotland? 2.2. If so what areas of forensic medical services would be appropriate? 2.3.
