1 Grassroots Engagement Fund Would you like to hold an event/activity for your local group or community? What are we offering? We are offering funding to local organisations and groups to run one-off activities or events to engage local people in the NHS. A maximum of 750 per event is available but applications for smaller grants are strongly encouraged so we can reach as many people as possible with this money. This fund for the borough of Merton is provided by South West London Collaborative Commissioning and administered by Healthwatch Merton. What can the money be used for? We would like to fund activities and events that would be enjoyable to local people and provide an opportunity for them to engage with the local NHS. Examples could include tea dances; pottery classes; fun days; drop in sessions; pizza and movie afternoons, cultural activities etc. Agreed with you in advance - people from the local NHS will attend these events/activities to talk to local people about their experience of services and collect information and views. Why are we funding this? Healthwatch Merton is working with the NHS in South West London, and Surrey Downs, to help find out what local people think about local health services. We are keen to encourage people who do not normally get the chance to express their views about local services to engage with the NHS and are looking for you to run such events and activities. How to apply Please complete a short application form which explains: what activity you would like to run; who you will be inviting/reaching, how many people you think will attend and how your application will meet our funding priorities; the date and time when the activity is likely to take place; how NHS staff will be able to engage with your attendees. Funding Priorities Applications that help us to reach people who do not regularly get to share their views with the NHS from the following groups will be prioritised children and young people pregnant women and those people from different races people with learning disabilities who have recently given birth and of different religions carers people with mental health conditions people with physical disabilities socio-economically deprived communities older people people for whom English is not a first language transgender people LGBT communities working age men and women who use NHS care in Merton We will prioritise applications that demonstrate how the NHS will be able to engage meaningfully with people they do not normally reach and demonstrate good value for money. 1

2 Decisions Applications that meet a minimum standard will be reviewed by the Panel who may ask for further information before making a decision on whether or not to provide funding. The decision of the panel is final and there is no appeal process. Successful applications will also have to run at times where it is possible to provide NHS attendance so please be as specific as possible Who can apply? Local groups and voluntary sector organisations who work with or support our community at the grassroots. We encourage applications from community groups, self-help groups as well as charities, businesses and social enterprises who can demonstrate that the funds will be spent on community engagement on a not for profit basis. When is the funding available? We are accepting applications from 23/5/16 until 1/12/2016 (or once the all the funding has been allocated). Applications should be made at least one month before the proposed event to ensure that South West London Collaborative Commissioning can make arrangements for local NHS professionals to attend. When will the activity need to be held? All events/activities need to be held by the end of December Money will be allocated on a first come first served basis once the funds have gone they ve gone. How will the NHS engage with my session? We realise that the activities will be varied, as will the attendees. Therefore the ways that the NHS captures information will need to be appropriate to the activity that you run and the people who take part. For example, if it s a coffee morning, a slot at the beginning to hear about people s experiences of the NHS might be appropriate. If it s a fun day, you could provide a stall for NHS staff to speak to people on a one to one basis. The NHS representatives don t need to give a presentation just have some way of speaking to people either in a group or individually. Please bear this in mind when planning your activity or event. The NHS will also want to collect feedback and information from the people at your event including their contact details, whether they want to hear more or take part in the future and their views and experiences of local health and social care. Any questions please contact: Dave Curtis, Manager, Healthwatch Merton - dave@healthwatchmerton.co.uk 2

3 Funding opportunity: Would you like to hold an event/activity for your local group or community to talk about the NHS? Name of the individual, community/voluntary group or organisation making this application: Name of main contact (including position if applicable): Address: Telephone number: Charity number (if applicable): Company Number (if applicable):. Please provide a brief description of your proposed activity/event: what you want to do date and time you would like to hold it (please be as exact as possible) Who it is aimed at? (please see Funding Priorities for groups we would like to target) How many people do you expect to take part? How will you promote the event or ensure that people attend? 3

4 Please explain how the NHS will be able to seek the views of people during this event. Please recommend the most suitable way to speak to this community given the nature of the event. (e.g. time for a presentation/focus group/one-to-one basis) Are there any NHS issues that your group is particularly interested in? How much is the total grant you are applying for? (Max 750) Please give a detailed breakdown of how you would spend this money (e.g. Room hire, refreshments, equipment, staff, resources etc.) Cost heading Total Does your organisation have the following in place?
