3 We are the MTE Community Care Sector Group Our companies supply medical technology to all parts of the healthcare system, e.g. hospitals, homes, primary care etc. Because we make products for both hospitals and the home, we are able to see clearly the additional value to patients and healthcare systems when people are treated in their communities We believe there is an untapped opportunity to move towards patient centric community level care, delivering better outcomes and better value care
4 Community care is care received out of the hospital Community care has no hard definition. Generally, it is seen as treatment provided outside of an acute care setting, such as the hospital. Community care brings care closer to where people live and spend most of their time. Full independence and ability to work Treatment is delivered in the home, either in full autonomy or with the help of family and/or visiting carers Treatment is delivered by a local GP or specialist doctor Treatment is delivered in a community centre Treatment is delivered in an hospice, care home or retirement home Treatment is delivered at the hospital Community Care

9 Anyone, at any age, can benefit from quality care in the community thanks to innovation in medtech Victor, 25, undergoes peritoneal dialysis at home everyday saving 1-hour trip to the closest hospital. Juan, 44, is a teacher. He is on home haemodialysis. He uses this time to correct his students homework from home! Maria, 72, lives with her son who helps her with her Alzheimer s. She also needs continence care, but thanks to high-quality products, it has never been easier for her and her son to manage this condition. Nellie, 23, was treated at her local GP for a wound on her hand. Thanks to medtech advances, she didn t have to go to the hospital! Samira, 32, has a temporary ostomy bag after she received lifesaving surgery for Crohn s disease. New generation of products means that she can still go to the gym without fearing pain or leakage. Jack, 40, needs urine retention care after he suffered a bladder trauma. Innovative devices allow him to continue doing what he likes most: work as a lawyer. André, 55, received pressure wound care at home after staying in hospital for 3 months. This treatment allowed him to return home earlier than envisaged and lowered his risk of contracting HAI. Terrence, 65, was saved from colon cancer thanks to surgery. His ostomy bag is adapted to his body shape and can be managed by Terrence himself.

11 Smart strategies to improve spending efficiency and reduce pressure on healthcare budgets are essential Earlier and more accurate diagnosis, improved remote monitoring / management of chronic diseases, and better efforts to avoid unnecessary hospitalisations can help reduce the reliance on expensive institutionalised care and contribute to more dignified and independent living.

13 Dialysis
14 Dialysis is a lifesaving procedure for chronic kidney disease Chronic kidney disease is mainly attributed to: Diabetes Hypertension These conditions are on the rise due to an ageing population 0.5m end-stage patients in Europe Dialysis therapy is required once end stage chronic kidney disease occurs, caused by many conditions including diabetes and high blood pressure Whilst most patients have their dialysis in a hospital setting, the clinical and lifestyle benefits of home dialysis mean that many more patients are now considering this option Sources: Tennankore et al. 2014, ERCPA 1 in 10 Europeans has some degree of Kidney Disease
15 Advances in dialysis technology means more patients can be treated at home Whilst the majority of patients had dialysis in hospital in the past, home based dialysis is an option today Advances in dialysis mean patients can be treated from home or at a centre in their community Treatment at home can be a safe and efficient therapy and can lead to an improved quality of life As much as 55% patients prefer to be treated at home Remote monitoring technology supports the management of patients and may help to prevent hospital admissions Source: Jager KJ et al. 2004
16 Incentivising home peritoneal dialysis and haemodialysis could increase patient quality of life and bring cost efficiencies Centre Haemodialysis Requires patients to go to a center 3 times per week for 3-4 hours per treatment Home Haemodialysis Allows patients to have a flexible dialysis schedule. Patients can stay on the machine longer or more frequently which is closer to the natural function of kidneys Home Peritoneal Dialysis Can be used independently after training, accompanied by remote monitoring or regular medical visits Patient can be treated while sleeping Better outcomes at a reduced cost: 10% increase in home dialysis for a period of 5 years would generate saving of 1 billion in Europe Patient empowerment to manage their own condition and increased employment opportunities people with end-stage renal disease between are currently twice as likely to be unemployed Incentives for physicians to prescribe home treatment need to be improved Sources: NHS UK, Joble J et al. 2010, Liu F X et al., RSIN Annual report 2012
17 Continence care
18 There are many reasons a person needs continence care and their needs are different too There are 2 types of continence care: incontinence care, providing absorbing devices for people with involuntary loss of urine/faeces; and urine retention care, providing devices for urine collection for people unable to empty their bladder. Incontinence care Incontinence is the involuntary leakage of urine and/or faeces Affects all ages, but increases as people get older (e.g. caused by dementia, stroke, or other comorbidities) Urinary incontinence affects both genders but is more common among women 4-8% of the population is affected Urine retention care Urine retention is the inability to empty the bladder completely Affects all ages and genders Caused by conditions such as spina bifida, multiple sclerosis, bladder traumas and spinal chord injuries Sources: Abrams, et al. 2013, Wagg et al. 2014

20 Selecting the right products for the right people for more efficient and customised care With the right products for the right patients, more people can work longer and/or contribute to society with confidence and dignity To avoid premature institutionalisation, which is very expensive to society and detrimental to patient well-being: Premature institutionalisation Address stigma and raise awareness to improve in-time and proper diagnosis Differentiate between patient needs in tendering and other reimbursement settings to offer different types of products Use internationally recognised standards for product selection (ISO 15621, ISO )

22 Acute and chronic wounds are increasing due to an ageing population and chronic diseases Acute wounds (e.g. from trauma, surgery etc.) are most commonly thought about However, chronic wounds, caused by conditions like diabetes are increasing Elderly people are more susceptible In the EU, million have acute or chronic wounds, and 4 million individuals will develop new wounds annually Chronic wounds (e.g. bed sores, diabetic foot, pressure ulcers, venous ulcers) Acute wounds (e.g. trauma, surgery, burns) Up to 50% of hospital beds are occupied by patients with a wound (many which develop while at hospital) 1 in 5 patients in European hospitals have a pressure ulcer, 50-80% are hospital acquired Source: Posnett, J et al.
23 Advanced wound care helps people heal faster and increase comfort Wound care has dramatically evolved in the recent years, benefiting patients: Moist dressings Innovative materials such as foam, hydrocolloid, alginate, film, hydrogel and collagen (including anti-microbials) mean that new advanced wound care devices are more impermeable, maintain appropriate humidity and thermal insulation and allow adequate breathing of the wound External devices Portable Negative Pressure Therapy (NPWT) devices allow patients to return home to manage chronic wounds and heal faster Patients benefits 1. 24 Incentivising better care stops wounds from becoming worse, saving healthcare systems time and resources Wound care involves all three main healthcare disciplines: nursing, surgery and general practice Best practice sharing and healthcare provider education are essential for good quality wound care Appropriate wound care minimises treatment costs by: Encouraging funding and reimbursement for advanced wound care products and services, outside of acute settings, improve cost efficiencies Reducing wound complications and infections Decreasing time spent in hospitals and use of specialist services Reduced time spent by nurses / care givers in institutions (e.g. elderly care homes) on changing dressings

27 Preventing leakage and increasing discretion: two easy ways that give people with ostomy the confidence to live a normal life Leakage Remains the number one concern, with still more work to do despite major advances Discretion Modern products have much better designs which allow increased discretion for patients 1 to 2 in three people with an ostomy are impacted by skin conditions Ostomy should be tailored for individual patients. Many skin and health complications can be easily avoided with tailored ostomy care Technological advances allow ostomy care to become increasingly patient specific and adapted for better fits to the body Sources: Bosio et al. 2007, Meisner et al Just treating additional skin conditions costs 25.4 Million to the French health system alone
28 Ostomy care in the community enhances patient lives and reduces healthcare costs linked to complications Both patients and healthcare systems could benefit from more advanced ostomy care in the community reducing complications and hospitalisation through: Better access levels to innovation (currently, access is inconsistent due to budget reductions and restrictive reimbursement policies) Patient-centric, personalised and tailored approach to ostomy care (currently, ostomy care categorisation and reimbursement policies are inconsistent and highly countryspecific) Increased number of suitably-trained specialist nurses in the community (currently, ongoing ostomy care is generally hospital-centric)
