3 Welcome, to the new South Warwickshire NHS Foundation Trusts Continence Formulary. We have tried to make this guide very user friendly. We expect a 100% compliance with the new formulary. If for any reason you need to go off formulary, can you please discuss with the Continence CNS for your area to ensure the appliance is appropriate. Please ensure the patient uses any current stock before switching to the new products to help reduce waste. Always ensure that the sheath is applied with the foreskin forward.
