How can we prevent Edna s skin from breaking down? Consider following these steps to help prevent skin damage from pressure ulcers: Understand why Edna is at risk. Develop a care plan to prevent pressure ulcers, based on her risk factors. Inspect Edna s skin regularly and report any suspicious areas to the nurse or doctor. Provide good skin care and promote healthy eating habits. Help Edna turn and reposition her body on a regular basis to relieve pressure. Publication No OH /2010. This material was prepared by

Pressure ulcers can be treated if they are identified, so it is important to conduct skin inspections. Conduct a head-to-toe skin inspection for all residents on admission, and then on a regular basis. Look closely for any open or reddened areas, and be sure to document and report your findings. Did you know? Tissue damage from unrelieved pressure can occur in as little as 2 hours. Publication No OH /2010. This material was prepared by

Loss of physical functioning can lead to immobility, which can lead to possible skin breakdown. Encourage your residents to participate in daily activities to promote their independence and better skin health. Support independence with ADLs. Publication No OH /2010. This material was prepared by

Make mealtime an enjoyable occasion for your residents by offering smaller, more frequent meals with choices and sufficient time for completing their meals. Eat and mingle with residents, and get to know their likes and dislikes. Food and fun the prescription for healthy skin! Make sure all residents are consuming their supplements and snacks as ordered to help prevent skin breakdown. Operations Manual, Appendix PP. 2009; F-325. Publication No OH /2010. This material was prepared by

Since incontinence is a risk factor for pressure ulcers, it is important to develop and follow toileting plans. When incontinent residents are helped to maintain or improve their bladder control, skin may be less likely to break down. Prevent skin damage from excess moisture and its damaging effects by promoting continence. Source: Centers for Medicare & Medicaid Services. Guidance to State Surveyors for Long Term Care Facilities. State Operations Manual, Appendix PP. 2009; F-315. Publication No OH /2010. This material was prepared by

Keep bed linens and incontinence pads clean and free of wrinkles. Eliminate causes of uneven surfaces (crumbs, extra pads, etc.) that can increase pressure on the skin and lead to pressure ulcers. Do your residents beds pass inspection? Publication No OH /2010. This material was prepared by

Keep the head of the bed at a 30 angle or less for all residents (unless it needs to be higher for medical reasons). Sliding may cause friction or shear, and can damage delicate sacral tissue, buttocks, heels, elbows or spines. Remember the 30 rule. Publication No OH /2010. This material was prepared by

What is a pressure ulcer? Also called a bedsore, a pressure ulcer is a localized injury to the skin and/or underlying tissue. Injury usually occurs over a bony prominence (like ankles, heels, and tailbones), and can be caused by pressure, or pressure in combination with shear or friction.
