3 When the vein system is not working The vein muscle pump helps the blood back to the heart. If the vein system does not work sufficiently, or the vein muscle pump is not active (for instance by wheelchair users), it will result in an accumulation of liquid in the legs. If this condition continues for a long period of time, it can cause damage to skin and tissue and potentially result in wounds. Compression treatment is the cornerstone for treatment of venous insufficiency and wounds When treating oedema and/or wounds caused by venous insufficiency, compression therapy is essential treatment. The choice of compression treatment depends on the condition of the patient and the wound. Initially, it can be necessary to attend/cleanse the wound on a daily basis while reducing the oedema. In this instance, short stretch or long stretch bandages would be suitable. These bandages can be washed and re-used several times and should be repositioned every day. Treatments with short and long stretch bandages can be very efficient when following simple rules. Short stretch bandage The short stretch bandage is produced from 00% cotton and contains no elastic fibres. It stretches up to 90%. It provides a high working pressure and is very efficient when the patient walks and uses the vein muscle pump. The more the patient walks, the more efficient the treatment. The short stretch bandage has a low resting pressure and can therefore be worn at night Long stretch bandage The long stretch bandage is produced from a mixture of cotton and elastic fibres and stretches more than 20%. It provides a high working pressure and a high resting pressure and is efficient whether the patient is mobile or not and whether the vein muscle pump is used or not. Due to the high resting pressure it is recommended not to use the bandage during the night. 3

4 Applying short stretch bandages Apply padding first in order to protect the skin and avoid pressure to the bone protrusions. 2 Start at the base of the toes - with two turns, preferably firm, to avoid front foot oedema. 3 Depending on the length of the foot and the width of the bandage apply 2-3 turns on the foot, followed by one turn around the mid-heel. 4 One more turn around the foot and again low around the heel. Repeat around the foot and enclose the heel by applying the bandage high. Continue upwards over the ankle. 5 Continue applying the bandage in a spiral around the leg - with 50-75% overlap - depending on the required level of subbandage-pressure. 6 Fixate bandage before starting with bandage 2. Continue around the leg with bandage 2. It is important to apply the bandage firmly to secure optimum compression. 7 Continue bandaging up to the back of the knee. 8 Finish with a single circular turn under the knee. Cut off excess bandage and secure with tape. 4

5 Applying long stretch bandages Start at the base of the toes - with 2 turns, preferably firm, to avoid front foot oedema. 2 Depending on the length of the foot and the width of the bandage apply 2-3 turns on the foot. 3 Next turn around the mid-heel. 4 One turn around the foot and low around the heel. Repeat around the foot and up to the heel at a high spot to ensure that the heel is totally enclosed. 5 Continue over the ankle and start turning in a figure of 8 or a herringbone pattern. 6 Secure bandage and continue with bandage 2 in herringbone turns upwards on the leg with a 50-75% overlap, depending on the required subbandage-pressure. 7 Continue bandaging up to the back of the knee. 8 Finish with a single circular turn under the knee. Cut off excess bandage as extra turns under the back of the knee can cause stasis. Secure with tape. 5

6 Application of compression bandages Apply 2 bandages on each leg either 2 pcs of 0 cm or pc of 8 cm and pc of 0 cm (starting with the smallest) Apply from the toes to the knee. Angle the foot at 90 to avoid too much bandage over the instep. Apply the bandage firmly. Short stretch bandages can be applied with very firmly, as the pressure will drop after a few hours. Padding To avoid pressure injuries on the skin it is recommended to use padding under the bandage. In areas with a lot of oedema extra padding can be useful, for instance using pressure pads behind the malleolus. If the leg is very irregular and difficult to bandage the irregularity can be evened out with padding. In treatments with ointments/creams it can be useful to apply a tube gauze under the padding. This will protect the bandages and enhance their durability. If the gauze is cut in double length it can be used for the fixation of the bandage after application. Fixation Do not use fasteners for fixation as they can damage both skin and bandage. Use fixation tapes instead. Good advice: Wash the bandages after each use. Use hand washing or machine washing, for the latter use a washing bag. Use a non-perfumed, mild washing powder, avoid softener. TThe bandages must air-dry laying flat, but not stretched. When rolling the bandage up avoid stretching. Short stretch bandages of cotton can be washed at 95 C.
