3 Surgical Technique (Shortening Osteotomy of P1) 1 Identification of the anatomic dimple and insertion of the proximal drill Insert a 10/10 Kirschner wire perpendicular to the long axis of the phalanx. Then, insert the proximal cannulated drill over the K-wire, making sure that you drill both cortices 2 Selection of the appropriate staple width and bone cuts Determine the appropriate staple width and set the drill guide accordingly Place the drill guide over the proximal drill Mark the position of the proximal cut with the oscillating saw, midway between the two guide bushings and parallel to the proximal drill The drill guide is removed to perform the distal cut Then the proximal cut is made 3 Placement of the axial pin After the bone segment has been removed, carefully realign the phalanx and hold it firmly while introducing a 10/10 K-wire along its longitudinal axis, across the osteotomy, underneath the dorsal cortex MAT Medical Advanced Technology 3

4 4 Distal drill placement The drill guide is repositioned over the cannulated drill and set at the value determined during step 2 The distal hole is drilled using the non-cannulated drill Remove both proximal and distal drills, leaving only the axial pin in place 5 Staple selection (standard or offset) Add at least 1 mm to the length determined with the measuring device to ensure bicortical purchase 6 Staple insertion Grasp the selected staple with the holding forceps or with standard surgical forceps: the central eye of the staple is captured within the jaws of the forceps Introduce the staple into the phalanx until it makes contact with the medial cortex Compression is achieved by closing progressively the handle of the forceps, which produces approximation of the bone fragments If necessary, complete insertion of the staple with the impactor The axial pin is removed 4 MAT Medical Advanced Technology

6 Material Stainless Steel (ISO according to biocompatibility ISO ) Indications Osteotomies of the first phalange and small fragments in view of a reaxation Principle and characteristics For the simplicity of its shape, its very limited obstruction and the ergonomics of its instrumentation, this implant is ideal for the realization of osteotomies and reaxation of small fragments. Owing to the choice of two angulations and two different widths, this staple meets best with the shape of the cortices. Operation Technique Perform the osteotomy observing to preserve the second corticalis. Proceed to the ablation of the osseous segment-and then reduce the phalange by reaxation. In order to mark the two entry points, introduce and impact slightly the staple by using the inserter-impactor (straight or oblique according to the chosen staple), then remove the staple. Make a pre-hole at the level of the entry-point distal. Use a Kirschner wire 10/10 mm. Introduce and impact the staple with the adapted inserter-impactor. Complete impaction by using the monobloc impactor. Note: Use the Varisation staple 26 in case of basic osteotomy.
