1 COLLEGEWIDE COURSE OUTLINE OF RECORD RESP 101, ASSESSMENT AND CARING FOR A RESPIRATORY PATIENT COURSE TITLE: Assessment and Caring for a Respiratory Patient COURSE NUMBER: RESP 101 PREREQUISITES: Program Chair Approval SCHOOL: Health Sciences PROGRAM: Respiratory Care CREDIT HOURS: 6 CONTACT HOURS: Lecture: 4 Lab: 4 DATE OF LAST REVISION: Fall, 2015 EFFECTIVE DATE OF THIS REVISION: Fall, 2016 CATALOG DESCRIPTION: Presents an introduction into respiratory care including evaluating data in a patient s record and recommending laboratory tests. Includes the process of interviewing and assessing the patient through inspection, palpation, percussion, and auscultation. Covers performing and evaluating a patient non-invasively. Includes topics on oxygen therapy, oxygen analyzers, use of specialty gases, humidity therapy, bland aerosol, medicated aerosol delivery devices and environmental therapy. Basic lung expansion therapy, airway management, including a variety of airway clearance techniques will be covered. Students will be presented information regarding infection control, how to provide care during cardiopulmonary resuscitation and obstructed/lost airway. Assembly and troubleshooting respiratory equipment will be required. MAJOR COURSE LEARNING OBJECTIVES FROM 2015 NBRC MATRIX: Upon successful completion of this course the student will be expected to: 1. Evaluate Data in the patient record: a. Patient history (admission data, orders, meds, DNR, social history) b. Physical exam and trends relative to the cardiopulmonary system and trends (e.g. vitals, physical) c. Lab data (CBC, electrolytes, C&S, Sputum) d. Summarize major legal and medical consideration in medical records documentation 2. Interview patient to assess: a. Level of consciousness and orientation, emotional state, and ability to cooperate. b. Level of pain c. Presence of dyspnea, sputum production, and exercise tolerance d. Patient and family medical history (e.g. smoking history, environmental exposure) e. Advanced directives f. Activities of daily living g. Learning needs (e.g. literacy, learning style, culture) 3. Performing inspection to assess: a. General appearance b. Determine airway patency (HTCL, jaw thrust) c. Cough, sputum amount and characteristics d. Recommend sputum for gram stain, culture and sensitivity Ivy Tech Community College 1 RESP 101

2 4. Palpating to assess: a. Pulse, rhythm, force b. Accessory muscle activity c. Asymmetrical chest movements, tactile fremitus, crepitus, tenderness, secretions in the airway and tracheal deviation 5. Performs diagnostic chest percussion 6. Auscultate to assess: a. breath sounds b. blood pressure 7. Recommend diagnostic lab procedures e.g. blood tests, for example electrolytes, CBC, Sputum gram stain, Sputum culture and Sensitivity 8. Perform and evaluate noninvasive monitoring for example pulse oximeter: a. Assemble and troubleshoot noninvasive monitoring devices for example pulse oximeter b. Recommend diagnostic procedures e.g. noninvasive monitoring for example pulse oximetry c. Trends in noninvasive monitoring (e.g. pulse-oximetry) 9. Demonstrates knowledge of oxygen therapy: a. Indications and hazards of oxygen therapy b. Classify oxygen therapy devices c. Assemble and troubleshoot oxygen devices d. Assemble and troubleshoot gas delivery, concentrator, liquid, cylinder, flowmeter, regulators, blenders, air compressor e. Perform quality control procedures on flowmeters f. Support oxygenation by initiating and adjusting high and low flow oxygen therapy g. Ensure modifications are made to respiratory care plan by recommending initiation of treatment h. Ensure modifications are made to the respiratory care plan by recommending changes in oxygen therapy based on HR, RR, SpO2, color, general appearance and LOC i. Ensure modifications are made to the respiratory care plan by discontinuing treatment 10. Demonstrates knowledge of specialty gases: a. Indications, hazards for helix, NO, CO2 11. Demonstrates knowledge of humidification therapy (including HME): a. Indications, contraindications and hazards of humidity therapy (including HME) b. Assemble and troubleshoot humidifiers c. Manage a patient s airway by maintaining adequate humidification d. Ensure modifications are made to respiratory care plan by recommending initiation of treatment e. Ensure modifications are made to the respiratory care plan by recommending changes in humidification therapy f. Ensure modifications are made to the respiratory care plan by discontinuing treatment 12. Demonstrates knowledge of bland nebulizers: a. Indications, contraindications and hazards of aerosol therapy (including bland aerosols and tents) b. Assemble and troubleshoot nebulizers. Ivy Tech Community College 2 RESP 101

3 c. Ensure modifications are made to respiratory care plan by recommending initiation of treatment d. Ensure modifications are made to the respiratory care plan by recommending changes in aerosol therapy (change aerosol output, temperature of aerosol) e. Ensure modifications are made to the respiratory care plan by discontinuing treatment (HR, RR, BS, sputum) f. Perform, evaluate and recommend sputum induction for sputum for gram stain, culture and sensitivity 13. Demonstrates knowledge of oxygen analyzers: a. Assemble and trouble shoot gas analyzers 14. Demonstrates knowledge of maintaining an airway: a. Positions patient to maintain a patent airway b. Establishes and manages a patient s airway (nasopharyngeal, oropharyngeal) c. Assemble and troubleshoot artificial airways d. Assist a physician/provider in performing intubation e. Assemble and troubleshoot resuscitation devices f. List types and characteristics of manual resuscitation devices g. Properly assess patient and modify resuscitation based on response (VT, rate, flow) h. Manage a patient s airway by performing tracheostomy care 15. Performing airway clearance: a. Lists indications, hazards of suctioning and modifies suction pressures b. Suctioning for example, nasotracheal, oropharyngeal, and artificial airways c. Assemble and troubleshoot suctioning equipment, e.g. regulator, canister, tubing, catheter d. Assisted cough, for example quad and huff cough e. Ensure modifications are made to respiratory care plan by treatment termination for life threatening adverse events f. Ensure modifications are made to the respiratory care plan by discontinuing treatment 16. Demonstrates knowledge by performing lung expansion techniques: a. Lists indications, hazards and contraindications for IS b. Coaches patient in proper breathing techniques for IS c. Assemble and troubleshoot lung expansion equipment for IS d. Ensure modifications are made to respiratory care plan by recommending initiation of treatment. (HR, RR, BS, SpO2, sputum, CXR) e. Ensure modifications are made to the respiratory care plan by discontinuing treatment f. Ensure modifications are made to respiratory care plan by treatment termination for life threatening adverse events 17. Demonstrate and apply knowledge of pharmacology: a. Explain and perform procedure to gather clinical information (peak flow) b. Evaluate procedure results (peak flow) c. Ensure modifications are made to respiratory care plan by recommending starting treatment, modifying therapy and/or discontinuing treatment d. Administer medication (aerosolized preparations MDI, SVN, DPI, soft mist inhalers, endotracheal instillation) Ivy Tech Community College 3 RESP 101

4 e. Assemble and troubleshoot equipment (MDI, spacers, valved holding chambers, DPI, soft mist inhalers) 18. Demonstrates knowledge of infection control: a. Suggests using high level disinfection techniques b. Selects appropriate agents and techniques for surface disinfection c. Ensure infection control by monitoring effectiveness of sterilization procedures d. Ensure infection control by proper handling of biohazard materials e. Adhering to infection control policies and procedures, e.g. standard precautions, isolation 19. Provide Respiratory Care Techniques in High risk situations for cardiopulmonary emergencies for example, cardiac arrest, and obstructed/lost airway COURSE CONTENT: Topical areas of study include Laboratory data Patient Assessment Oxygen Transport Oxygen Therapy Regulating devices Compressed gas storage Oxygen Analyzers Specialty gas therapy Humidity therapy Bland nebulizer therapy Medicated Aerosol Devices (SVN, MDI, DPI, Slow Mist) Environmental therapy Airway management (naso, oro and tracheostomy care) Manual resuscitators Airway clearance (suctioning, directed cough) Lung expansion techniques (IS) Infection Control Cardiopulmonary emergencies Evaluations Procedures: 1. Hand Washing 2. Patient Interview and History/Communication 3. Medical Records (Review of and Charting) 4. Physical Exam of the Chest (Inspection, Palpation, Percussion, Auscultation) 5. Vital Signs Including Pulse Oximetry 6. Cylinders, Regulators and Flowmeters 7. Oxygen Therapy Devices (NC, SM, Partial and NRB Masks,VM and Membrane Cartilage) 8. Standard Precautions/Isolation Techniques 9. Humidification 10. Environmental Therapy 11. Oxygen Analysis 12. Large Volume Nebulizers (Bland) 13. Medicated Aerosols (SVN, MDI, DPI and Slow Mist) 14.
