2 Purpose ARFI is a new ultrasound application that enables numerical measurements of the mechanical stiffness of tissue. The aim of the study was to evaluate if the method could be useful in assessing kidney fibrosis caused by glomerulonephritis. Methods and Materials Using Siemens Acuson S2000 Virtual Touch Tissue Quantification 28 patients mean age 45y(±14) with glomerulonephritis confirmed by biopsy and control group of 10 adult healthy volunteers, mean age 33y ( ±12) were examined. ARFI quantification, given as shear wave velocity (SWV) of the renal cortex was performed.the intrarenal blood flow was measured by Doppler US and RI ( resistive index) was calculated.the length of kidneys and cortex thickness was measured. Additionally the liver tissue quantification was performed for each patient. Images for this section: Page 2 of 9

7 Results Two valid measurements per each kidney was performed. RI was calculated from two segmental arteries. For patients with glomerulonephritis the median SWV at median depth 61mm(±12) was 2,5m/s (range:1,42-3,64), RI 0,63 while in healthy subjects SWV at median depth 48mm(±8) was 2,95m/s ( range:1,3-4,88) and RI 0,58.There was a significant difference ( p<0,05 ) in SWV and RI values between two groups. The liver tissue quantification results did not differ significantly in both groups-median SWV was 1,23( range:0,88-1,74) vs 1,09( range:0,85-1,42)m/s. There was not a significant difference in kidney length and cortex thickness.there was a signifant influence of age on SWV and RI values - that may require in future studies the larger groups to compare. 8 Conclusion ARFI quantification gives possibility to measure kidney stiffness.high variations in SWV values in kidneys needs to be discussed and reproducibility of measurements improved.
