At the heart of communities 3
Responding to Change Pharmacy practice has transformed from focusing only on medicines and disease to patient care and quality Large portion of total health care spending is on pharmaceuticals and chronic diseases Pharmacy services aim to achieve the triple aim : Better quality of care Better health outcome Lower costs 4
It all makes sense More trips here mean fewer visits here 5
Trends in pharmacy services Core Pharmacy Services (expertise in medicines): Dispensing (incl. repeat dispensing and homecare); Compounding; Medication management (unit dose packaging, new medicines service, medicines use review); Emergency care (incl. emergency contraception) and minor ailment management. Advanced Pharmacy Services (health care services): Vaccination; Smoking cessation; Measurement of blood pressure, cholesterol, glucose, weight, Chronic disease management; Early screening and testing. 6
Overview of pharmacy services Pharmacy services by number of country in Europe *PGEU Database Medication Review (Type 1) Dispensing Prescriptions Weight measurement Blood pressure measurement Night services Disposal of medicines waste Glucose measurement Cholesterol measurement Smoking Cessation eprescribing/ edispensation Repeat dispensing Medication Review (Type 2) Home Delivery of Medicines Sale/supply of Self-test Kits to Patients/public Asthma / COPD management Diabetes management Hypertension management Needle & Syringe Exchange New Medicine Service Medication Review (Type 3) Vaccination (by Pharmacist) Weight Management Manual Unit-dose Dispensing Tele/online Consultations with Patients/public Cardiovascular Disease Management
Community pharmacist s contributions to public health 8
Prevention 9
Health Checks in a Pharmacy One can measure blood pressure in 27 countries weight in 28 countries blood glucose levels in 23 countries cholesterol in 22 countries in a community pharmacy. 10
Health promotion 11
Public Health Services in a Pharmacy In 21 European countries pharmacies offer smoking cessation services; Pharmacies support number of national and regional public health campaigns: Including European Antibiotic Awareness Day; As part of our daily practice we offer health and wellbeing advice and support for self care 12
Protection 13
Vaccination & needle/syringe exchange services In 12 countries in Europe patients can be vaccinated within community pharmacies In 8 countries in Europe pharmacists provide a needle and syringe exchange service for intravenous drug users. 14
Emergency care All pharmacies in Europe offer: Minor ailment (cough, sore throat, head lice, indigestion, flu, etc.) management, Emergency supply for chronic and acute conditions. 15
What is next for community pharmacy? The case for the pharmacist s contribution to health systems in the form of services, has never been stronger; Evidence confirms that pharmacists interventions improve patient outcomes and they also save money; However, services are unlikely to be sustainable on a large scale without financial support; Our accessibility is the major asset and needs to be preserved along with logistic efficiency ; New services are important, but the dispensing function is crucial to our place in communities; Quality, professionalism and added value are at the heart of what we do. 16
