REVISED: June 2015 REVISED: December 2014 REVISED: August 2016 Quitline Fax Referral Forms (for Health Care Providers) This form is designed for use by health care providers to refer clients for counselling and follow up through the Quitline. Tear off pads are available in English, with laminated translated copies available for clients who are unilingual. Quit Kits Contain motivational material intended to help prepare tobacco users for their quit attempt. They contain a water bottle, silly putty, food container, yoyo, herbal tea, bracelet and pin all inside a lunch bag. Tobacco Quit Guide Coiled booklet with lots of great information on how to quit smoking using plenty of pictures and Inuit imagery. Available in Nunavut s four official languages. 3 Chew Cost Calculator *Lesson plan available for health workers* This new tool is targeted to the smokeless (chew, snuff) tobacco user to demonstrate the financial cost of chew use. Turn the wheel to the number of cans of chew that is used per week and the patient will quickly see how much they save in 1 week, 1 month, Eng/Inuk Inn/Fr 1 year, 5 years, 10 years and 20 years by quitting. Quit Calendar This calendar guides the tobacco user through a dayby-day quit starting two weeks before a planned quit with daily motivational messages. What is Second-Hand Smoke Double-sided info sheet on second-hand smoke and what action can be taken to avoid it.

4 Weight Gain and Quitting Worry about weight gain when quitting smoking can be a major barrier for tobacco users. This doublesided info sheet provides the truth about weight gain when quitting and what can be done to prevent it from happening. Rumour Busting There are rumours (myths) about quitting smoking that get spread through word of mouth that are not true. This double-sided info sheet provides the facts to help dispel the rumours (myths) in order to get tobacco users on the tobacco-free journey. Health Effects of Smoking This double-sided info sheet shares the effects of smoking and some frequently asked questions. It will be helpful for tobacco users who are still needing more information before they consider quitting.

5 Smokeless Tobacco This double-sided info sheet outlines the basic facts about smokeless tobacco including: what it is, how it affects a persons health and how to get help to quit using. 6 Say No To Tobacco Colouring Book All languages in one (for young Nunavummiut) A colouring book (with crayons) for Kindergarten to Grade 3. REVISED: June 2015 Smokey Sue Smokes for Two Demonstrates the effects of smoking on the unborn baby to prenatal groups. Available at Canadian Prenatal Nutrition Programs (CPNP). Game of a Tobacco-Free Life Boardgame A made in Nunavut board game that discusses how tobacco affects the baby and mother, and what supports are available for quitting and reducing tobacco use. Available at Canadian Prenatal Nutrition Programs (CPNP). Smokerlyzer Carbon Monoxide Monitor This handheld battery operated testing device demonstrates the level of carbon monoxide in a persons body from smoking or second-hand smoke. Available from the Community Health Representative.

7 D-Pieces for Smokerlyzer These plastic pieces are needed to operate the Smokerlyzer and should be replaced every 30 uses or if contaminated. A new Steribreath mouthpiece is inserted into the D-piece before each use. 12 per box Steribreath Mouthpieces for Smokerlyzer The smaller Steribreath mouthpieces look like a straw and must be replaced every time someone blows into the Smokerlyzer. They must be inserted into the D-piece. Clem s Phlegm Shows the amount of phlegm brought up by someone with Chronic Obstructive Pulmonary Disease (COPD) every two weeks. COPD is a disease that is caused by smoking. 250 per box Mr. Gross Mouth This educational model shows the effect that smokeless tobacco (chew/snuff) has on the mouth.

8 Smokers Lungs Demonstration Kit Shows the effect smoking has on the lungs. One set of lungs is a non-smoker while the other set is a 20- year smoker of a pack per day. Available through the Community Health Representative. A good tool to use with youth in schools or at health fairs. Humectant for Smokers Lungs Propylene glycol solution to preserve and keep pigs lungs moist. Varenicline/Bupropion (for Healthcare Professionals) Information about the smoking cessation pills that are available in Nunavut, how they work, side effects and dosing.
