1 2016 Update COMMUNITY HEALTH IMPROVEMENT PLAN January 2016
2 Dear Barron County, Thrive Barron County has been an evolving partnership between Public Health, Cumberland Healthcare, Lakeview Medical Center, Marshfield Clinic, Mayo Clinic Health System- Northland, and multiple other community organizations. These entities started looking at the health of Barron County residents in In 2013 and again in 2015 Barron County conducted Community Health Assessments to look at health related data. Community members gave input on their health concerns and completed asset mapping worksheets in each of our health concern areas. In 2016 Barron County Public Health, with input from more than 25 different organizations and community members and information received at the 2015 Community Health Assessment, created this update to the Community Health Improvement Plan or CHIP. While our overall health priorities remained the same; what we are working on within those priorities has evolved based on data and community needs. The issues highlighted below are new since the original plan was created in Alcohol, Tobacco, and Other Drugs Tobacco Use Youth Substance Use Prevention Methamphetamine Use Chronic Disease Prevention & Management Pre-Diabetes Hypertension Mental Health Promotion and Stigma Reduction Youth Access Community Health Action Teams (CHATs) along with community organizations are meeting regularly to work on the goals and strategies you will read about in this update. We appreciate and thank the community for its commitment to improving health in Barron County. 4 Vision: Community members and organizations working together to improve the quality of life for everyone in Barron County. Mission: Thrive Barron County will work to engage community members and organizations to focus resources and develop and strengthen partnerships to establish sustainable, safe and healthy communities. Overarching Goals: Assess needs-conduct community survey, seek community input, review and analyze data Evaluate efforts-did the intervention meet the goals. 5 Alcohol, Tobacco, & Other Drug Use & Abuse What are our overall goals? 1. By December 2018, reduce the rate of people using methamphetamine in Barron County. 2. By December 2018, decrease the number of Barron County adults reporting smoking and binge drinking based on the County Health Ranking Data. 3. By December 2018, decrease the number of high school students reporting smoking, marijuana, and alcohol use as measured by the Youth Risk Behavior Survey. Goal 1: By December 2018, reduce the rate of people using methamphetamine in Barron County. Why are we concerned: New since 2012: The use of methamphetamine has increasingly become problematic in Barron County, as reported by the Barron County Sheriff, District Attorney, WI State Crime Lab, and Drug and Alcohol Court (DAC). 7 Strategies to decrease methamphetamine use in Barron County: 1. Work collaboratively with Northwoods Coalition Methamphetamine Workgroup on regional prevention strategies. 2. Educate lawmakers on the impact of methamphetamine in Barron County. 3. Increase community awareness of methamphetamine by hosting Methamphetamine Town Hall events throughout Barron County. 4. Increase community awareness of methamphetamine by hosting the hiding in plain sight interactive display at Barron County schools, businesses, community events. 5. Promote preventative messages through know Meth Barron County initiatives 6. Work collaboratively with law enforcement and community partners to identify treatment and recovery options. 7. Collaborate with other counties to create and publish a report of localized solutions and recommendations collected from the 2016 Regional Stakeholder s Meeting which discussed local challenges and solutions to the meth problem. 8. Share the Hiding in Plain Sight interactive mock room display throughout Barron County. Goal 2: By December 2018, decrease the number of Barron County adults reporting smoking based on the County Health Ranking Data. 8 Strategies to reduce adult tobacco use in Barron County: 1. Promote Kick Butts Day throughout Barron County to encourage tobacco cessation. 2. Encourage local lawmakers to establish smoke free outdoor spaces throughout Barron County. 3. Promote the WI Tobacco Quit Line and resources at health fairs, patient waiting rooms, and when speaking to clients. 4. Conduct general community outreach through health fairs, press releases, social media posts, etc. Goal 3: By December 2018, decrease the number of high school students reporting smoking, marijuana, and alcohol use as measured by the Youth Risk Behavior Survey. Participate and promote National Kick Butts Day throughout Barron County schools and Boys and Girls Clubs. 2. Conduct WI Wins Tobacco Age Compliance checks to prevent youth access to tobacco. 3. Provide Barron County Tobacco Retailers with information, resources, and educational tools to increase their knowledge and prevent the sale of tobacco to minors. 4. Conduct youth alcohol compliance checks at Barron County alcohol retailers to prevent youth access to alcohol. 5. Promote the Parents Who Host, Lose the Most campaign to increase parental awareness of the dangers and legal ramifications of providing a space for youth to consume alcohol. 6. Host an annual youth outreach event to educate Barron County High School Students about an ATODA subject. 7. Conduct general community outreach through health fairs, press releases, social media posts, etc. 8. Barron County schools incorporate ATODA topics in their Health Education Classes. 9. Offer Safe Serve Classes to ensure that Barron County Alcohol retailers are aware of the laws regarding alcohol sales. Barron County Community Health Improvement Plan 2016 Update 9
10 Chronic Disease Prevention & Management What are our overall goals? 1. By December 2018, maintain or decrease the number of Barron County Residents who have pre-diabetes and type II diabetes. 2. By December 2018, maintain or decrease the number of Barron County Residents who report hypertension. Goal 1: By December 2018, maintain or decrease the number of Barron County residents who have pre-diabetes and type II diabetes. Conduct general community outreach through health fairs, press releases, social media posts, etc. 4. Share the Diabetes Risk Test on social media, in press releases, and in our patient waiting rooms. 5. Provide information and resources in our patient waiting rooms about diabetes prevention. 6. 13 Mental Health What are our overall goals? 1. By March 2019 Barron County adults will report fewer poor mental health days. 2. By December 2018 Barron County youth mental health indicators as measured on the Why are we concerned: New since 2012: According to the , Adverse Childhood Experience (ACE) study, almost 21% of Barron County residents 18 or older have experienced four or more ACE s. Barron County receives a high number of child protection access calls compared to counties of similar size. In 2013, Barron County received a total of 971 access calls to Youth & Family services for child welfare and child protection services. Goal 1: By March 2019, Barron County adult mental health indicators will improve. [Adult Mental Health] Indicator 1.1: The number of poor mental health days/month reported by Barron County adults. 16 Mental Health Strategies: Continue to promote mental health issues during May is Mental Health Month, Mental Health Promotion Week in November. Continue to conduct activities that reduce the stigma around mental health. Promote community efforts that promote resiliency and reduce Adverse Childhood Events. Promote trauma informed communities. Continue to develop mental health capacity to provide Trauma Focused Cognitive Behavior Therapy to youth identified with trauma. Increase Mental Health First Aid trainings in Barron County.
