Nimitz Highway, Suite 209 Honolulu, HI December 2008 TO: FROM: SUBJECT: All Participants Residing on the Big Island of Hawaii, Maui and Kauai and enrolled in the Indemnity Prescription Drug Plan for the Hawaii Teamsters Health and Welfare Trust Board of Trustees INDEMNITY PRESCRIPTION DRUG PLAN CHANGES The following information is very important changes to the Indemnity Prescription Drug Plan. PLEASE BE SURE TO READ THIS INFORMATION CAREFULLY. By using the programs as described below, you may lower your out-of-pocket costs for prescription drugs. Effective January 1, 2009, the Indemnity Prescription Drug plan benefits will change as follows: For Long Term (Maintenance) Prescriptions (medications for chronic conditions such as diabetes, high cholesterol, high blood pressure; those taken for more than 15 days), you may obtain your prescriptions through the Central Fill Program which is available at designated pharmacies on the Big Island of Hawaii, Maui and Kauai. (Please refer to attached listing of designated pharmacies on your Island) For Short Term (Acute) Prescriptions (drugs such as antibiotics or those taken for up to 15 days), you may obtain your prescriptions through any Catalyst Rx participating pharmacy under the Point-of-Service (POS) Program. (Please refer to the attached listing of participating Point of Service pharmacies on your Island) Central Fill Program **LONG TERM PRESCRIPTIONS** The Central Fill Program has been designed to provide up to a 60-day supply of maintenance drugs to treat conditions such as diabetes, high blood pressure and high cholesterol. Under the Central Fill Program, you may fill your long term prescriptions at a of the designated pharmacy on your island by following the steps below:

2 Step 1: Step 2: Step 3: Obtain a prescription from your doctor. Go to a designated pharmacy and present your HMA/Catalyst Rx identification card. If this is the first time that you are taking this drug or dosage of the drug, the pharmacy will fill your prescription for an initial fill of 15 days at the following copayments: 15-Day Supply Limit (1 st Time Fill) Generic Drugs, Insulin & Supplies $ 5.00 Brand Name Drugs $15.00 If your doctor decides to continue your prescription for this drug and dosage, you may obtain a Refill for a 60-day supply. Step 4: Call the pharmacy refill telephone number at least three (3) days before your prescription supply runs out and request a refill. Step 5: Pick up your refill for a 60-day supply and the following copayments shall apply 60-Day Supply Limit (Refills) Generic Drugs, Insulin & Supplies $ 8.00 Brand Name Drugs $24.00 The Central Fill Program services are only available at the designated pharmacies on the attached listing for the Big Island of Hawaii, Maui and Kauai and not at any other store locations. Point of Service (POS) Program **SHORT-TERM PRESCRIPTIONS** Under the Point of Service (POS) Program, you may obtain your short-term prescriptions for up to a 15-day supply from any Catalyst Rx participating POS pharmacy on your island at the following copayments:

3 15-Day Supply Limit Generic Drugs, Insulin & Supplies $ 5.00 Brand Name Drugs $15.00 Prescriptions will no longer be available for a 21-day or 30-day supply under the POS program, however, prepackaged prescription drugs that can only be dispensed in unbreakable packages (e.g. creams, ointments, certain inhalers), the days supply limit shall be equivalent to the package size days supply, not to exceed a 30 days supply, and the applicable 15-day copayment shall apply. For prescriptions requiring more than a 15-day supply, please use the Central Fill Program as described above. Other Programs There is no change to the Direct Member Reimbursement Program or the Mail Order Program through Walgreen s. Please refer to pages 56 and 58 of your Summary Plan Description dated May 2007 for detailed coverage information of these prescription drug benefit programs.
