1 Drug Testing Policy October 2016 To: All Students Participating in Intercollegiate Athletics Subject: Drug and Alcohol Education and Testing Program Substance abuse, either alcohol or other drugs, is a serious problem in today s society. University campuses, including Purdue University Northwest, are not immune to this problem. We recognize that student athletes are no more or less likely than other students to participate in the use or abuse of alcohol and other drugs. The special importance of physical and mental fitness to athletic success should provide a strong incentive to studentathletes to avoid use of these substances. However, it is statistically predictable that some student-athletes will have experimented with such substances prior to arriving on campus or may be prone to do so during their collegiate years. Some student-athletes may be attracted to use the so-called performance enhancing drugs such as anabolic steroids, in misguided efforts to increase their strength and endurance or recover from injury. In light of the above considerations, serious attention is given to drug usage and related problems in intercollegiate athletic circles throughout the country, including Purdue University Northwest. The National Collegiate Athletic Association (NCAA) requires that all student-athletes annually sign a statement in which they submit information concerning their eligibility and related matters, and also consent to be tested for the use of various drugs, which are prohibited by NCAA regulations. These NCAA requirements and the NCAA forms will be explained in detail to all Purdue Northwest student-athletes after arrival on campus by members of the Intercollegiate Athletics staff. Many universities, including Purdue Northwest, have their own drug and alcohol education and testing program. Purdue Northwest s program is summarized below and a complete copy of the program is enclosed.

2 We are proud of Purdue Northwest s athletic programs and are sincerely interested in the overall development and personal well-being of our student-athletes. Purdue Northwest s Chancellor, Thomas L. Keon, shares these interests. Our program has been carefully designed and is administered with the full support of the university. The program is applied to all intercollegiate sports teams. It is intended for all studentathletes, whether or not they hold athletic scholarships or grant-in-aid. The drug and alcohol education and testing program is intended to be an integral part of our overall physical and mental conditioning program. Its purpose is intended as a medical diagnostic aid in identifying substance abuse related problems to protect the health and safety of student-athletes, to deter student-athletes from using alcohol and drugs, and to safeguard the integrity of athletic competitions by assuring student-athletes, other schools, and the public that fair competition is an overriding principle in athletics. If a student-athlete has evidence of banned substances in his/her system, he/she must be made fully aware of the physical, mental, and emotional harm which can result from continued drug usage. Since continued drug usage or an alcohol problem obviously is inconsistent with participation in intercollegiate athletics, the program includes appropriate procedures for dealing with such problems. Enclosed is a copy of the Purdue University Northwest Department of Intercollegiate Athletics Drug and Alcohol Education and Testing Program for your information, along with a current list of NCAA banned substances in Table 2. Please read these carefully. Also, be aware that the list of NCAA banned substances is not exhaustive and may be updated from time to time. The most current list is always posted on the NCAA s website, ncaa.org. Also, before consuming any nutritional/dietary supplement product, review the product with your athletics department staff. Dietary supplements are not well regulated and may cause a positive drug test result. Any product containing a dietary supplement ingredient is taken at your own risk. The program will be explained in detail and consent forms will be signed after your arrival on campus this fall. 3 PURDUE UNIVERSITY NORTHWEST DEPARTMENT OF INTERCOLLEGIATE ATHLETICS DRUG AND ALCOHOL EDUCATION AND TESTING PROGRAM October 2016 For the mutual best interest of Purdue University Northwest and all Purdue Northwest studentathletes, the following drug and alcohol education and testing procedures will be in operation. The purposes of the program are to inform student-athletes about the serious physical, mental, and emotional harm caused by illegal/banned substances, serve as a diagnostic aid for the identification of individuals affected by substance abuse or alcohol in order to protect the health and safety of the affected individual and other student-athletes, deter the use of such substances, and safeguard the integrity of athletic competitions by assuring student-athletes, schools, and the public that fair competition is an overriding principle in athletics. The goal at Purdue University Northwest is to maintain an athletic environment free of substance abuse that is consistent with the high standards of the university and with the overall development and education of its student-athletes. The program will be administered in a manner consistent with the personal rights and privacy interests of the studentathletes. Substance abuse (whether disclosed through drug testing or otherwise) is inconsistent with academic and athletic excellence, and will not be tolerated. Procedures for implementation of this program are outlined below. 1) As a part of the orientation process for each intercollegiate sport, the head coach will arrange for all squad members to attend a drug and alcohol education and testing orientation, which will be administered by the Athletic Trainers. During the orientation program for individual sports, the department s drug and alcohol testing program will be explained and consent forms will be distributed to all student-athletes who have not previously signed a consent form for the current academic year. The athletic trainers will request student athletes complete the appropriate forms listing all prescribed medications. The drug and alcohol education and testing program will apply to all student-athletes participating in intercollegiate sports. 2) Purdue University Northwest has selected The National Center for Drug Free Sport (Drug Free Sport) as an Independent third-party Administrator of the Drug and Alcohol Education and Testing Program. Drug Free Sport services include professional and confidential sport drug-testing sample collections, specimen analysis by World Anti-Doping Agency (WADA) certified laboratories, 3 P a g e

4 educational programming, full program audits, policy design, development and review, and all phases of program management, from random selections to confidential results reporting. 3) Educational programming is an integral part of the drug-use deterrence model. To empower student-athletes to make responsible and healthy decisions Purdue Northwest, in partnership with Drug Free Sport, will offer the following educational programming and resources: a) myplaybook - myplaybook is a collection of online lessons that uses evidence-based strategies to promote the health and well-being of student-athletes. Specific strategies include social norms, behavioral expectancies, behavioral intentions, as well as bystander, decision making, stress management, goal setting and harm prevention skills. myplaybook s interactive programming engages student-athletes using state-of-the-art instructional design and includes content related to: (1) Alcohol (2) Tobacco (3) Marijuana (4) Performance-Enhancing Drugs (5) Dietary Supplements & Sports Nutrition (6) Prescription & Over-the-Counter Drugs (7) Life Skills (8) Sexual Assault (9) Transitioning Out of College b) On Campus Educational Speakers - Expert speakers from Drug Free Sport offer a variety of on-site educational opportunities for coaches, administrators and student athletes. c) Resource Exchange Center (REC) - Drug Free Sport offers a subscription based service for athletes called the Resource Exchange Center (REC). Student-athletes can gain access to experts online, via phone or for questions about supplements prior to consuming a product that may contain a banned ingredient. Access to the REC is available 24 hours a day, seven days a week, online at All inquiries to the REC may be made anonymously and all are kept strictly confidential. 4) The testing program will be administered by an Independent Third Party Administrator (Drug Free Sport), under the terms of the consent forms signed by all participating student-athletes. The testing program is intended primarily to test for the presence in the body of street drugs such as amphetamines, cannabinoids (substances contained in marijuana), cocaine and other controlled substances, but may also include procedures to detect anabolic steroids and other performance enhancing drugs. A non-exhaustive list of NCAA banned substances is included in Table 2. This list is subject to modification, and the most current list can be found on the NCAA s website at ncaa.org. Also, before consuming any nutritional/dietary supplement product, a student-athlete should review the product with his or her athletics department staff. Dietary supplements are not well 4 P a g e

5 regulated and may cause a positive drug test result. Any product containing a dietary supplement ingredient is taken at the student-athlete s own risk. 1 5) Testing will be done: a) on a random basis several times per year and more frequently if requested by the Athletic Trainers or Head Coaches, based on reasonable suspicion that a student-athlete is under the influence of alcohol and/or illegal/banned substances based on (but not limited to) factors such as: (1) Unusual or erratic behavior or athletic performance, (2) Behavior, speech, body odor, or appearance indicative of the use of alcohol or illegal/banned substances, (3) Observing a student-athlete using, possessing, selling or purchasing alcohol at a time when it may affect athletic performance or illegal/banned substances at any time, (4) Discovering that a student-athlete has taken action to interfere with or defeat his or her test results or the test results of another student-athlete; and b) because of a positive test and entry into Categories I-IV explained below and in Table 1. Testing will occur under direct observation and through chemical analyses of split-sample urine specimens ( urinalysis ) using appropriate chain of custody procedures (see Appendix A Drug Free Sport Urine Collection Guidelines). The Independent Third Party Administrator will generate a random selection of student athletes to be tested. Once the Associate Athletic Director for Compliance has received the list of student-athletes who will be tested from the Independent Third Party Administrator, the Associate Athletic Director for Compliance (or designee of the Athletic Director) will begin the notification process. Student athletes will be notified of the upcoming test no more than 24 hours prior to the test. The Head Coaches of the selected student athletes may also be contacted. Student athletes must report to the compliance office the day of the notification to fill out the designated paperwork prior to their test the next morning. Student athletes will report at the designated place and time to provide a testable specimen, i.e., urine. Confidentiality will be maintained by code numbers known only to the Associate Athletic Director for Compliance and Independent Third Party Administrator. The analyses will be performed only through Substance Abuse and Mental Health Services Administration ( SAMHSA ) or WADA certified laboratories. If the primary specimen tests positive, based on cut-off levels established by the lab, the lab will perform a confirmation test of the primary specimen. If the confirmation test is positive, the lab will report the positive result to the Associate Athletic Director for Compliance describing which substances were detected. The Associate Athletic Director for Compliance will inform the Athletic Director of positive tests. The Associate Athletic Director for Compliance will arrange a meeting with the student-athlete, Athletic Trainer, Head Coach, and Athletic Director to discuss the positive test result. The studentathlete may provide an explanation for the positive test result, and the Athletic Trainer may request verification for any prescription medications. 6 consultation with the Independent Third Party Administrator, will determine whether a valid medical explanation exists for the positive test result. At the time of being notified of a positive result, a student-athlete may request to have the remaining portion of his or her split-specimen tested. The results of the second test will be final. The Associate Athletic Director for Compliance will report the final results to the Athletic Director. 6) A student-athlete will be placed in one of five categories within the department s drug and alcohol testing program: Category 0, I, II, III and IV. All student-athletes enter the program in Category 0 (i.e., assumed to be drug-free and/or have never had a positive test). A positive test will result in entry into one of the other four categories as defined on Table 1. For purposes of testing, a plea of guilty or a judgment of guilty for an alcohol or banned/illegal substance federal, state, or local law violation, a violation of a University rule or policy involving alcohol or banned/illegal substance, or a positive test from another athletic association, such as the NCAA, is deemed to be the same as a positive test. Further, refusing to sign a consent to testing form, or any other required form, failing to appear for testing at the designated time, failing to provide a urine sample in accordance with protocol, or attempting to alter the integrity or validity of the urine specimen and/or collection process will constitute a positive test result. All drug testing violations are considered cumulative for the student-athlete s career. 7) The Athletic Director will recommend professional counseling through the Purdue University Northwest Counseling Center to any student-athlete who tests positive on one or more occasions. In addition, a student athlete with a positive test result will be required to seek medical clearance to participate in athletic activities from the Health Services Center. However, unlike the situation where a student-athlete is injured while participating in his/her sport, the University may not assume financial or other responsibility for continuing medical treatment and rehabilitation of student-athletes with drug or alcohol usage or dependency problems if the student uses or requires services outside of the University. The University will encourage and be supportive of studentathletes who participate in such medical or rehabilitation programs but will not assume any responsibility for these programs or for any physical, mental, or emotional damage or disability suffered by a student-athlete related to alcohol or drug usage. 8) Purdue University Northwest financial grant-in-aid forms contain the following provision: a) This tender may not be renewed if I am suspended from athletic competition or dismissed from an athletic team for participating in the use, sale, or distribution of any narcotic drug or controlled substance. Even where formal suspension or dismissal from an athletic team has occurred, NCAA regulations would require that a student-athlete be given the opportunity for a hearing, within the Division of Financial Aid, if the University proposed not to renew any athletic scholarship or grant-in-aid previously awarded to the student-athlete. 9) The Office of the Dean of Students will not receive any student-athlete s test results or use any information or evidence as to such test results for any disciplinary purpose whatsoever. The Department of Intercollegiate Athletics will make every effort to keep all such test results confidential except to the limited extent otherwise provided in Table 1, and except as required to be disclosed by law. The results will be considered part of the student-athlete s medical record and are, therefore, confidential by law. 6 P a g e

7 10) All Drug, Alcohol, Tobacco, and Banned Substance violations are considered cumulative for the student-athlete s career. Any violations of the Student Code of Conduct for manufacturing or distribution of any illegal drug, controlled substance, narcotic or prescription drug, except as expressly permitted by law, will cause a student-athlete to be terminated from athletic participation. 11) Hearings, Appeals, and Reinstatement a) Hearings (1) Student-athletes who test positive under the terms of this program will be entitled to a hearing with the Athletic Director to appeal the results of the findings prior to imposition of any sanction. A written request for such a hearing must be made within 24 hours of notification that a sanction is being imposed. The hearing will be held no later than 96 hours after the student-athlete request. An extension of time for the hearing may be granted upon a reasonable request. A student-athlete requesting such an extension may be placed on interim suspension during the extension. (2) The Athletic Director will notify the student athlete of his or her decision within 24 hours after the hearing. b) Appeals (1) Determinations made under section Table 1 may be appealed to the Athletic Appeals Committee. Such request for appeal must be made within five business days of the determination in question. (2) The Athletic Appeals Committee shall adopt hearing procedures. The procedures shall include an opportunity for the student athlete to present evidence and have an advisor of choice accompany him or her at any hearing on the matter. Procedures shall also provide for confidentiality. The decision of the Athletic Appeals Committee is final. 12) Reinstatement a) An individual sanctioned under the testing protocol procedures may be reinstated on recommendation of the PNW Counseling Center Mental Health Professional, Athletic Trainer, Head Coach and with the concurrence of the Director of Athletics. Such reinstatement may be subject to specific conditions, including periodic testing to ensure compliance. This policy was developed at Purdue University Northwest with input from the athletic administrators, athletic trainers, coaches, Dean of Students Office, Counseling Center, Purdue legal counsel, and staff at the National Center for Drug Free Sport. The policy was reviewed with the Chancellor. 7 P a g e

Category I: Qualifications 1) At least one positive test for alcohol or banned/illegal substance Resulting Actions: 1) Student-athlete will meet with a Mental Health Professional provided by Purdue University Northwest Counseling Center for a mandatory two-session substance use screening and intervention. This must occur within two weeks of the positive test result. 2) Student-athlete may be required to participate in more frequent testing at unannounced times. If a student-athlete fails to participate in the additional testing procedure, the student-athlete will be suspended from competition until he/she participates in the additional testing procedure. 3) Test results confined to Athletic Trainer for that sport, Counseling Center Mental Health Professional, Head Coach, Associate Athletic Director for Compliance and Athletic Director. 4) Parents/guardian may be notified and kept informed of progress. Category II: Qualifications 1) Any student-athlete who tests positive while in Category I 2) Failure of a student-athlete to comply with any of the requirements of Category I. Resulting Actions: 1) Student-athlete will meet with a Mental Health Professional provided by Purdue University Northwest Counseling Center for a mandatory two-session substance use screening and intervention. This must occur within two weeks of entering Category II. 8 P a g e

9 2) Student-athlete may be required to participate in more frequent testing at unannounced times. If a student-athlete fails to participate in the additional testing procedure, student-athlete will be suspended from competition until he/she participates in the additional testing procedure. 3) The student-athlete will be penalized 20% of his or her total playing season. This penalty could extend into the next playing season to meet the 20% requirement. The student-athlete will continue to practice and attend all other required team activities but will not be allowed to compete during this time. 4) Test results confined to Athletic Trainer for that sport, Counseling Center Mental Health Professional, Head Coach, Associate Athletic Director for Compliance and Athletic Director. 5) Parents/guardian will be notified and kept informed of progress. Category III: Qualifications 1) Any student-athlete who tests positive while in Category II; or 2) Failure of a student-athlete to comply with any of the requirements of Category II Resulting Actions: 1) Student-athlete will meet with a Mental Health Professional provided by Purdue University Northwest Counseling Center for a mandatory two-session substance use screening and intervention. This must occur within two weeks of entering Category III. 2) Student-athlete will undergo more frequent testing at unannounced times. 3) The student-athlete will be penalized 50% of his or her total playing season. This penalty could extend into the next playing season to meet the 50% requirement. The student-athlete will continue to practice and attend all other required team activities but will not be allowed to compete during this time. 4) Parents/guardian will again be notified of progress. 5) Test results confined to Athletic Trainer for that sport, Counseling Center Mental Health Professional, Head Coach, Associate Athletic Director for Compliance and Athletic Director. 9 P a g e

10 Category IV: Qualifications 1) Any student-athlete who tests positive while in Category III; or 2) Failure of a student-athlete to comply with any of the requirements of Category III Resulting Actions: 1) The student-athlete may be dismissed from the team, and a recommendation will be made to and/or by the Athletics Director that his/her financial aid not be renewed, consistent with all institutional, conference and NCAA rules and regulations. 2) The student-athlete and his/her parents or guardians will be informed in writing of the decision within 7 days. 3) If there are extenuating circumstances and the student-athlete is not dismissed from the team, further requirements of that individual will be made on a case-by-case basis in conjunction with recommendations from the Athletic Training staff, University Counseling Center, the Head Coach and the Athletic Director. These requirements will include, at a minimum, increased frequency of testing and continued involvement of the parents/guardians and may result in temporary suspension from the team as well as other penalties as determined by the Athletic Director 10 P a g e

11 Table NCAA Banned Drugs It is the student athlete s responsibility to check with the appropriate or designated athletics staff before using any substance. The NCAA bans the following classes of drugs: 1. Stimulants; 2. Anabolic Agents; 3. Alcohol and Beta Blockers (banned for rifle only); 4. Diuretics and Other Masking Agents; 5. Street Drugs; 6. Peptide Hormones and Analogues; 7. Anti-estrogens; and 8. Beta-2 Agonists. Note: Any substance chemically related to these classes is also banned. The institution and the student-athlete shall be held accountable for all drugs within the banned drug class regardless of whether they have been specifically identified. Drugs and Procedures Subject to Restrictions: 1. Blood doping; 2. Gene doping; 3. Local anesthetics (under some conditions); 4. Manipulation of urine samples; and 5. Beta-2 Agonists permitted only by prescription and inhalation. NCAA Nutritional/Dietary Supplements Warning: Before consuming any nutritional/dietary supplement product, review the product with the appropriate or designated athletics department staff. There are no NCAA approved supplement products. 1. Dietary supplements, including vitamins and minerals, are not well regulated and may cause a positive drug test result. 2. Student-athletes have tested positive and lost their eligibility from using dietary supplements. 3. Many dietary supplements are contaminated with banned drugs not listed on the label. 4. Any product containing a dietary supplement ingredient is taken at your own risk. Check with your athletics department staff prior to using a supplement. 11 P a g e

12 Examples of NCAA Banned Substances in Each Drug Class Note to Student-Athletes: There is NO complete list of banned substances. Do not rely on this list to rule out any label ingredient. 1. Stimulants: : Amphetamine (Adderall); caffeine (guarana); cocaine; ephedrine; methamphetamine (DMAA); methylphenidate (Ritalin); synephrine (bitter orange); methylhexaneamine, "bath salts" (mephedrone); octopamne; DMBA; phenethylamines (PEAs); etc. Exceptions: phenylephrine and pseudoephedrine are not banned. 2. Anabolic Agents (sometimes listed as a chemical formula, such as 3,6,17-androstenetrione): Androstenedione; boldenone; clenbuterol; DHEA (7-Keto); epi-trenbolone; etiocholanolone; methasterone; methandienone; nandrolone; norandrostenedione; ostarine, stanozolol; stenbolone; testosterone; trenbolone; SARMS (ostarine); etc. 3. Alcohol and Beta Blockers (banned for rifle only): Alcohol; atenolol; metoprolol; nadolol; pindolol; propranolol; timolol; etc. 4. Diuretics (water pills) and Other Masking Agents: Bumetanide; chlorothiazide; furosemide; hydrochlorothiazide; probenecid; spironolactone (canrenone); triameterene; trichlormethiazide; etc. 5. Street Drugs: Heroin; marijuana; tetrahydrocannabinol (THC); synthetic cannabinoids (e.g., spice, K2, JWH-018, JWH-073). 6. Peptide Hormones and Analogues: Growth hormone (hgh); human chorionic gonadotropin (hcg); erythropoietin (EPO); IGF-1; etc. 7. Anti-Estrogens: Anastrozole; tamoxifen; formestane; ATD; clomiphene; SERMS (nolvadex); Arimidex; clomid; evista; fulvestrant; aromatase inhibitors (Androst-3, 5-dien-7, 17-dione), etc. 8. Beta-2 Agonists: Bambuterol; formoterol; salbutamol; salmeterol; higenamine; norcoclaurine; etc. Additional examples of banned drugs can be found at Any substance that is chemically related to the class, even if it is not listed as an example, is also banned! 13 Purdue University Northwest Drug Education and Testing Consent Form I have received, and have read and understand, the attached copy of the Purdue University Northwest Department of Intercollegiate Athletics Drug and Alcohol Education and Testing Program ( Program ). I have also had the opportunity to ask any questions I may have about the Program, and my questions have been answered to my satisfaction. I agree to participate in said drug and alcohol education and testing program and to be subject to its terms. I understand that Drug Free Sport, an Independent Third Party Administrator, will obtain urine specimens from me for the testing and analysis (including testing and analysis for the possible presence of alcohol and any NCAA banned substances in my system), will maintain confidential records and results of such tests, and will perform related activities as set forth in the said Program. I agree to cooperate in furnishing my urine specimens as requested. I understand that the results of these tests will be maintained in the athletic training office as a confidential medical record. I further consent to and authorize the disclosure of my records and test results, only to those persons and only under the circumstances described in the Program or as required by law. 14 Appendix A Drug Free Sport Urine Collection Guidelines 1. Only those persons authorized by the institution will be allowed in the collection room. 2. When arriving to the collection room, the student-athlete will provide photo identification or a client representative will need to identify the student-athlete. The student-athlete will then print his or her name and arrival time on the Roster Sign-In Form. 3. The student-athlete will select a Custody & Control Form (CCF) from a supply of such and work with the institutional collector to complete the necessary information before proceeding with the specimen collection process. 4. The student-athlete will select a specimen collection beaker from a supply of such and will be escorted by the institutional collector (same gender) to the restroom to provide a specimen. The student-athlete will rinse his or her hands with water (no soap) and then dry their hands. Then the student-athlete will place a specimen barcode from the Custody & Control Form onto the beaker. 5. The institutional collector will directly observe the furnishing of the urine specimen to assure the integrity of the specimen. 6. The student-athlete will be responsible for keeping the collection beaker closed and controlled. 7. Fluids and food given to student-athletes who have difficulty voiding must be from sealed containers (approved by the institutional collector), opened and consumed in the collection room. These items must be free of any other banned substances. 8. If the specimen is incomplete, the student-athlete must remain in the collection room until the sample is completed. During this period, the student-athlete is responsible for keeping the collection beaker closed and controlled. 9. If the specimen is incomplete and the student-athlete must leave the collection room for a reason approved by the institutional collector, the specimen must be discarded. 10. Upon return to the collection room, the student-athlete will begin the collection procedure again. 11. Once an adequate volume specimen is provided; the institutional collector will escort the studentathlete to the specimen processing table. 12. The specimen processor will instruct the student-athlete to closely observe the specimen processing steps and will then measure the specific gravity. 13. If the urine has a specific gravity below 1.005, no value will be recorded on the CCF and the specimen will be discarded by the student-athlete with the institutional collector observing. The student-athlete must remain in the collection room until another specimen is provided. The studentathlete will provide another specimen. 14. Once the specimen processor has determined the specimen has a specific gravity above the sample will be processed and sent to the laboratory. 15. If the laboratory determines that a student-athlete s sample is inadequate for analysis, at the client s discretion, another sample may be collected. 14 P a g e

15 16. If a student-athlete is suspected of manipulating specimens (e.g., via dilution, substitution), the institutional collector will collect another specimen from the student-athlete. 17. Once a specimen has been provided that meets the on-site specific gravity, the student-athlete will select a sample collection kit from a supply of such. 18. The specimen processor will open the kit, demonstrate to the student-athlete the vials are securely sealed, open the plastic and open the A vial lid. The processor will pour the urine into the A and B vials and close the lids. The specimen processor should pour urine into vials above the minimum volume level (35 ml in A vial; 15 ml in B vial) and pour as much urine as possible into the vials using care not to exceed the maximum levels (90 ml in A vial; 60 ml in B vial). 19. The specimen processor will securely close the lids on each vial and then seal each vial using the vial seals attached to the CCF; assuring seals are tightly adhered to the vials with no tears or loose areas. 20. The specimen processor must then collect all necessary signatures (collector, donor, witness, and collector/specimen processor) and dates/times where indicated on the CCF. 21. The specimen processor will place the laboratory copy of the CCF in the back pouch of the plastic bag and the vials in the front pouch of the same bag. The bag should then be sealed. The sealed bag with vials will then be placed in the sample box. The box will then be sealed. 22. The student-athlete is then released by the institutional collector. 23. All sealed samples will be secured in a shipping case. The collector will prepare the case for forwarding. When two split samples are collected and packaged, care must be taken to assure one sample is placed in the shipping container for shipment to the drugs of abuse laboratory and one sample is placed in the shipping container for shipment to the anabolic steroids laboratory. 24. After the collection has been completed, the samples will be forwarded to the appropriate laboratory and copies of any forms forwarded to the Sport Drug Testing Department. 25. The samples then become the property of the client. 26. If the student-athlete does not comply with the collection process, the institutional collector will notify the appropriate institutional administrator and Drug Free Sport. 15 P a g e
