1 HEALTHY TOGETHER KNOX Healthy Workers Initiative Workplace Health & Wellbeing Survey A jointly funded initiative of the State Government of Victoria and the Australian Government

2 Invitation to employee Dear employee, We are looking to develop a workplace health and wellbeing program and would like to learn more about what topics and activities you are interested in to improve your health. This survey asks you about eating, physical activity, alcohol consumption and smoking habits and will take you approximately 10 minutes to complete. All information collected will be confidential and will help the organisation to identify changes that it can make to the workplace to support improved health and wellbeing.
