2 Our Mission WI Cancer Council Reducing disparities in colorectal cancer screening August 21, 2018 Presenter: Sarah Franҫois Director of Fund Development & Marketing Progressive Community s exists to improve the health and quality of life of the community by providing culturally competent services that address identified needs. What is a Community? Located in a federally designated medically underserved area Offer comprehensive primary health care and other services to facilitate access to care, such as transportation and interpretation Governed by a board of directors composed of at least 51% consumers Cannot refuse care to those unable to pay offer sliding fee scale Must participate in federal reporting requirements True or False? Federally Qualified s are the same as Community s. TRUE. There are three types of FQHCs: Community s, Homeless and Migrant Health Centers, and Tribal s. True or False? True or False? Most Community s are entirely funded by the federal government. FALSE. As non-profit entities, s must leverage additional resources to support operations. The federal contribution is intended to be only a portion of the agency s overall budget. Most providers and support staff at Community s are volunteers. FALSE. s typically employ their providers and support staff. Patients and their care team establish long-term relationships in a medical home environment. 2

Outcomes EBI #5: Professional Development Hosted CME workshop for providers and clinical support staff EBI #6: Patient Navigation CHW follows-up with patients who have not completed screening (assesses barriers) Overall agency CRC screening rate increased from 46% (July 2016) to 61% (June 2018) Ranked #1 in CRC screening statewide as of Q Added third CRC screening site (Kilbourn) Distributed nearly 400 incentive gift cards In year-by-year comparison survey, more than double # of patients reported awareness of CRC screening Outcomes (cont.) Winner: Poop to the Hoop Challenge! First annual contest sponsored by American Cancer Society Goal = distribute the most FIT kits during the month of March We distributed 53 kits as part of the campaign the most of any participating agency!
