1.0 Purpse This Prcedure establishes the minimum requirements fr perfrming lckut/tagut (LOTO) acrss all CncPhillips Canada (CPC) peratins. This Prcedure des nt apply t the fllwing situatins: Minr equipment adjustments r service activities (e.g. testing r trubleshting) that meet all f the fllwing criteria: It is essential that the equipment cntinues t run. Alternate prtectin methds are used (machine guards r a lcal cntrl switch) ensuring wrkers are nt expsed t surces f energy. A hazard assessment r task analysis has determined that there is n risk f injury t the wrker r ther wrkers nearby. An apprved, written business r functinal unit prcedure is utilized. Wrk n crded r plugged equipment (includes quick release air r hydraulic lines) that meets all f the fllwing criteria: Unplugging the equipment results in cmplete islatin frm all energy surces. The plug r cnnectr is kept in sight and within easy reach s that n ne else can accidentally plug in the equipment. The plug r cnnectr is under the exclusive and immediate cntrl f the wrker at all times. A hazard assessment r task analysis has determined that there is n risk f injury t the wrker r ther wrkers nearby. An apprved, written business r functinal unit prcedure is utilized. Wrk n electrical systems perfrmed by certified persnnel as permitted by the Electrical Wrk Prcedure. Unexpected activatin r mvement f equipment 3.0 Prcedure Specific Rles and Respnsibilities 3.1. Supervisrs Ensure wrkers are cmpetent and trained t perfrm energy islatin and LOTO activities. Ensure a cmpetent CPC Representative is designated t supervise the equipment islatin and lckut. Prvide adequate islatin devices and lcks t wrkers. Prvide authrizatin fr remval f a wrker s lck when required. 3.2. CPC Representative 3.3. Wrkers 4.0 Prcedure Ensure interlcking systems are nt invlved and equipment is prtected against this type f peratinal start up. Assess the ptential impact f the islatin and lckut/tagut n additinal equipment r prcesses. Determine, apprve and dcument the islatin methd. Inspect all lcks and tags prir t signing ff the Islatin Certificate. Ensure the Islatin Certificate is cmpleted and attached t the permit/hazard assessment. Inspect the wrk area and adjacent areas t determine they are in a safe cnditin. Ntify affected wrkers in adjacent wrk areas, including panel peratrs where applicable, when equipment is remved frm r returned t service. Fllw the islatin and lckut/tagut methd as determined by the hazard assessment and/r site specific prcedure. Attach their persnal lck(s) with an identifying tag and keep cntrl f the key(s) in their pssessin at all times. The fllwing requirements are applicable t all islatins and lckuts. Infrmatin specific t the type f lckut being perfrmed is included in the fllwing sectins. Wrk must nt prceed until the absence f energy and/r harmful substances has been verified. Wrkers must stay ut f the line f fire when testing fr absence f energy. If electrical wrk is t take place, islatin must be verified using a meter r apprved testing device as per the Electrical Wrk Prcedure. When an energy islating device is lcked ut, the lck must nt prevent access t ther energyislating devices that culd cause injury t wrkers. A permit and hazard assessment must be cnducted fr all wrk cmpleted under an islatin and lckut/tagut unless specifically exempted under an Exclusive Cntrl (Sectin 4.4). An Islatin Certificate must be prepared fr all islatins unless specifically exempted under an Exclusive Cntrl (Sectin 4.4). This Islatin Certificate must be referenced n all additinal permits r hazard assessments where wrk is being perfrmed under that islatin. When practicable, reference the applicable P&ID, electrical single line drawing, pipeline maps, r gathering system drawings t identify all islatin pints. All permits/hazard assessments and Islatin Certificates must be available at the wrk site. All wrkers directly invlved with the wrk, r wrking at ther lcatins that may be affected by the lckut, must be infrmed f the lckut and the islatin pints. All persnnel wrking n islated equipment have the right t verify the islatin pints. Changes in wrk cnditins r apprved wrk scpe require a revalidatin f the islatin methd and cmmunicatin t affected wrkers prir t cntinuing with the wrk. 4.1. Additinal Equipment Required 4.1.1. Additinal Persnal Prtective Equipment (PPE) Refer t the permit r hazard assessment and applicable Business Unit prcedures fr apprpriate task PPE. 4.1.2. Additinal Equipment Determine additinal equipment requirements based upn the permit/hazard assessment and applicable Business Unit r functinal grup prcedures. Where required, electrical testing equipment fr verifying electrical islatins (this equipment may nly be used by qualified individuals). 4.2. Lckut Devices 4.2.1. Islatin Lcks 4.2.2. Persnal lcks May be individually keyed r keyed alike if used in multiple lck sets fr grup lckuts. Cmbinatin lcks may nt be used fr islatin lcks. Islatin lcks are t be used fr islatin purpses nly. Each wrker wrking under an islatin must have an individually keyed persnal lck unless specifically apprved under an apprved cmplex grup lckut plan. Each wrker must maintain exclusive cntrl f their key at all times. Must be traceable t the wrker wh is using the lck. Cntractrs must supply persnal lcks fr their wrkers. Cmbinatin lcks may nt be used fr persnal lcks. 4.2.3. Islatin Securing Devices May include valve cvers, chains, cables, breakers cvers, etc. and must render the equipment inperative and hld the energy islating device in its ff r safe psitin. 4.2.4. Tags Devices must be strng enugh t withstand inadvertent pening withut the use f excessive frce, unusual measures r destructive techniques. Must be attached t each islatin pint. May nt be used as a substitutin fr a lck at an islatin pint. The minimum infrmatin required n the cmpleted tag includes: The reasn fr the lckut. The full name (printed) and signature (nt initial) f the persn wh perfrmed the lckut (r ther tracking identificatin if electrnic systems are utilized). The date the lckut was perfrmed. The standard tag fr use in Canada is attached in Appendix B. Integrated Safe System f Wrk (ISSW) tags may be used in place f these tags. 4.2.5. Lck Bxes 4.3. Preparatin Must be able t securely cntain all islatin lck keys and number f lcks need fr the jb task. 4.3.1. Equipment Shutdwn When the equipment has cmpletely stpped, the energy surces must be islated with the apprpriate energy islating device. This may invlve turning ff electrical pwer, blcking mvable parts, venting trapped vapr pressure, bleeding hydraulic lines, retracting nuclear surces, r releasing spring tensin energy. There may be mre than ne energy surce r surce f hazardus materials that may be present. 4.3.2. Islatin Verificatin Prir t starting wrk, the CPC Representative will ensure the machinery, equipment r piping system has been de energized r de pressured and the energy islating devices have been rendered inperative. Perfrm a bump test n all equipment that has the ptential t actuate, start r engage. Remte start systems, remte switches, panel systems r permissive circuits must be evaluated prir t cmpleting a bump test If a manual switch r bypass is nt available t verride the abve when perfrming a bump test, the islatin must be verified by a qualified individual (as per the Electrical Wrk Prcedure). 4.4. Exclusive Cntrl Where breakers are used as part f an islatin (e.g. electric mtr islatin t wrk n a pump, etc.), the absence f vltage des nt need t be verified when a successful bump test has been perfrmed. Ensure piping systems are de pressured and islated. 4.3.2.1. Electrical Islatin Verificatin When perfrming electrical wrk requiring an electrical islatin: Cnduct a test t validate the absence f vltage; dcument test n the Islatin Certificate. Test must be cnducted by a qualified individual prir t starting wrk. The absence f vltage must be revalidated after a change in cnditins r after an extended wrk break where care and cntrl f the islatin has nt been maintained. It is nt required that revalidatin tests be dcumented n the Islatin Certificate. When a designated wrker(s) maintains sle exclusive cntrl f the islated piping r prcess system, the requirement t attach lcks and cmplete an Islatin Certificate will nt apply if all f the fllwing criteria are met: The wrker(s) is a full time equivalent emplyee. The wrker(s) has cntinuus cntrl f each energy islating device and each energyislating device is immediately accessible and identifiable t the wrker. The absence f energy has been verified. The energy islating device is secured in the islated psitin against inadvertent mvement. The task is cvered under an apprved hazard r risk assessment (e.g. Task Risk Assessment, etc.). A prcedure is available that includes the prcess fr securing the energy islating devices. The wrker(s) des nt leave the islatin unattended. Wrk in adjacent areas is nt creating additinal risk. 4.5. Individual Lckuts (Single r Multiple Wrker) Each energy islatin device (e.g. discnnect switch, circuit breaker r valve) must be lcked ut by all wrkers invlved in the task. Each wrker must attach an individually keyed padlck(s) and tag(s) t the islatin securing device fr all energy r prduct surces (e.g. circuit breakers, switchbxes, valves and cntrls). When multiple wrkers lck nt an islating securing device with a hasp r scissrs, the CPC Representative must attach the first lck and tag at each islatin pint. The number f lcks used at each islatin pint must equal the number f persns wrking n the jb Each lck must be identified n the islatin certificate. The wrker attaching their padlck is respnsible fr testing and cnfirming that the energy surce(s) has been effectively islated and there exists a zer energy state. This is accmplished by bump testing the equipment r verifying the absence f pressure. When there are multiple wrkers lcking nt an islatin pint, the CPC Representative will test and cnfirm, n behalf f all wrkers, that the energy surces have been effectively islated as abve. When the wrk is cmpleted, the CPC Representative must be the last persn t remve their lck/tag frm the lckut scissrs at each islatin pint when multiple wrkers are invlved. Each wrker must maintain exclusive care and cntrl f their key(s) at all times. 4.6. Grup Lckuts (Using Lck Bxes) Used when a number f wrkers are wrking n machinery r equipment at the same time, particularly, if a large number f energy islatin devices must be lcked ut. The CPC Representative must attach an islatin lck with a tag n the islatin securing device fr each energy islating device. Recrd the lck numbers fr each energy islating device n the Islatin Certificate. The key(s) fr each islatin lck will be placed int a secured lck bx. If using keyed alike lcks in a lck set, all keys fr the lck set must be accunted fr in the lck bx A secnd cmpetent wrker must verify that the energy surces have been effectively islated. The CPC Representative will place the padlck keys fr each islatin pint int a lck bx that is lcked by all wrkers. Nte: In British Clumbia, there must be a secnd qualified wrker wh will independently attach a secnd lck t each energy islating device when using a lck bx. This wrker s name must be identified n the Islatin Certificate as the persn attaching the additinal lcks. Once effective energy islatin has been tested and befre starting the wrk activity, each wrker invlved in the wrk will then attach their padlck with a tag t the lck bx. The number f lcks n the lck bx must equal the number f wrkers perfrming the task. Each persnal lck applied t the lck bx must be dcumented n the Lck Bx Register. The CPC Representative must be the last persn t remve their persnal lck frm the lck bx. 4.7. Cmplex Grup Lckuts Used fr larger prjects, facility turnarunds, r when intercnnected equipment r systems can be adversely affected. A written lckut prcedure and wrk plan that prvides an equivalent level f safety must be develped t ensure a level f prtectin equivalent t that prvided by the grup lckut prcess. The cmplex grup lckut prcedure and wrk plan must be apprved by a Directr f Inspectin (in Alberta) r The Bard (in British Clumbia). 4.8. Securing Remtely Cntrlled Lcatins Where remtely cntrlled systems cntrl the peratin f machinery, equipment, piping, a pipeline r prcess system that cannt be islated using the prcedures listed abve, prcedures must be put in place that result in a level f islatin and prtectin equal t that prvided by nrmal prcedures. 4.9. Electrical Islatins Ensure all surces f electrical supply are identified (see single line diagram r panel schedule). De energize (turn ff) the identified circuit. When functining a breaker, ensure bdy psitin is away frm the line f fire and facing away. Utilize ne f the fllwing methds t secure the energy islating device: Attach an islatin securing device t the energy islating device, r Emply the lck n the panel dr r attach a lck hasp n the panel dr, as lng as the lck des nt prevent access t ther energy devices that culd cause injury t wrkers. Tape ver the breaker and cardbard signs hanging ver the breaker panel ntifying thers are nt permitted. Nte: In British Clumbia, lcking the cver dr is nt permitted as the lck must nt prevent access t ther energy islating device (i.e. circuit breakers) supplying machinery r equipment that culd cause injury t wrkers. 4.9.1. Islatin Task Respnsibilities Individual Mtr Starters Equipment Systems Under 750 Vlts Systems Over 750 Vlts Operatins/Maintenance Qualified Electrical Persn (Nte 1) Distributin Panels (120v/240v) Operatins/Maintenance Nt Applicable Facility Main Breakers/Incming Supply excludes pump jacks Qualified Electrical Persn Qualified Electrical Persn (Nte 1) Nte 1: There are currently three sites within WCBU which have ARC RESISTANT SWITCHGEAR These sites can be switched by peratins as the equipment inherently prtects the peratr frm any electrical internal faults. Placing Equipment Back int Operatin Befre remving lcks and returning the equipment back t service, the wrker in charge must: Ensure all equipment guards are prperly replaced. Ntify all wrkers f the intentin t restre energy t the equipment prir t remving the islatin securing r flw stpping devices. Cnfirm that n wrker will be endangered when the energy islating devices are pened r activated t return the equipment t service. Prir t restarting equipment, the Islatin Certificate must be cmpleted t ensure all lcks are accunted fr, all islatin securing devices have been remved and all energy islating devices are in the crrect psitin. 4.11. Shift, Crew Change and Handver When authrity fr an area, facility, r wrk lcatin is transferred t anther wrker, respnsibility fr the lcking and tagging devices must als be transferred. If an effective handver f the islatin/lckut cannt be achieved, the islatin must be revalidated by the incming persnnel. 4.11.1. CPC Representative 4.11.2. Wrkers Ensures that each utging wrker remves their lcks and tags. Cmpletes r transfers the permit and islatin paperwrk. Identifies the lcatin and types f all islatin pints with the incming CPC Representative. Ensures there is an effective transfer f cntrl by remving their padlck frm the lck bx nly after the incming wrker in charge attaches their padlck t the lck bx. Ntifies adjacent wrk areas r lcatins affected by the lckut f the wrk (includes permit) handver. Wrkers n a new shift must attach their lcks as required in this prcedure. 4.12. Bump Test: Attempting t start equipment by using the nrmal starting cntrls fllwing the installatin f the electrical islatin breaker, t verify that the equipment will nt start (includes attempting t start cntrl valves and ther equipment). Nte: mst bump tests are electrical, but bump testing includes all equipment that actuates, starts, engages, r turns. Cautin as the Distributed Cntrl System (DCS) might prevent equipment frm starting, nt the breaker. Bump tests are designed t test that the main electrical breaker has been successfully islated, r that (when nrmal starting practice is fllwed) the equipment will nt start. CPC Representative: Can be any CPC emplyee, cntract emplyee r cntractr assigned by CPC wh is respnsible fr perfrming r verifying islatins. The representative must be deemed cmpetent in the activity being cmpleted. Electrical Wrk: Any activity that invlves cntact with electrical cnductrs r electrical equipment. This will include pening any electrically energized cabinets, Mtr Cntrl Center (MCC) cubicles, mtr junctin bxes, transfrmer enclsures, etc. Electrical wrk als encmpasses discnnecting r recnnecting any electrical equipment, mtrs, switches r distributin equipment. Energy Islating Device: Devices that cntrl an energy surce. These devices may include manually perated electrical circuit breakers, discnnect switches, mtr starter, fuse, line valves, r ther blcking device that blck r islates energy. Push buttns, selectr switches (e.g. tail switches) and ther cntrl circuit type devices are nt energy islating devices. Energy Surce: Any electrical, mechanical, hydraulic, pneumatic, chemical, thermal, r ther surce f energy f ptential harm t wrkers.
