Fee cde Custm ear mlds. The assciated fees and business rules fr the abve fee cdes fllw. Hearing instruments will be funded minus accessries. The fllwing and ther accessries shuld nt be issued r authrizatin t issue requested: Dry aid kits. Drying system. Sleeve mlds. Remte cntrls. Easy phne aut t-cil. Battery tester and charger. Oticn streamer. Ear wax remval system. Wax receptr, etc. WCB reserves audit privileges and pst payment recnciliatin where inapprpriate billing has ccurred. Clarificatin may be btained frm the Health s Manager at wh may cnsult the executive f the Saskatchewan Hearing Instrument Practitiners regarding the spirit f the negtiatin that led t the abve fee schedule. Otscpic evaluatin. Pure tne air cnductin testing, and masking when indicated t include the fllwing frequencies: 250, 500, 1000, 2000, 3000, 4000, 6000, and 8000Hz. Pure tne bne cnductin testing t include the abve nted frequencies and masking when indicated. Please nte: The WCB requires all frequencies, including 3000Hz, fr determining and injured wrker s degree f permanent impairment. Speech audimetry including speech receptin threshld testing, determinatin f uncmfrtable levels, mst cmfrtable levels and speech discriminatin testing level. Assessment f the functin the middle ear system r impedance audimetry; and Depending n the results f the impedance testing, referral fr additinal testing, as may be required. A detailed written reprt f the audilgical test results, including recmmendatins regarding apprpriate amplificatin, signed and dated by the individual perfrming the test. May be repeated every *Three years * except where wrker experiences a sudden nset f tinnitus r sudden unilateral hearing lss after illness in a wrker wh has bilateral hearing aids. Where the injured wrker chses an upgraded prduct, the wrker will pay the difference between the aid that was recmmended and the aid chsen; ntwithstanding the ceiling fr fee cde 202. The wrker will nt pay additinal fitting/fllw up r ther fees payable by WCB. Fee where wrker has lst a hearing aid, with pre-apprval. WCB will apprve replacement aids nly where the current aid cannt be adjusted in respnse t a significant change in hearing, where there are signs f inadequate gain available, imprper amplificatin fr hearing lss, imprper fit resulting in feedback, where repair is n lnger cst effective, with pre-apprval. Fee Cde Fee Repair fee 205 Manufacturer s price plus 10% handling fee and actual shipping fee Nt billable while warranty perid in effect. Frm end f warranty perid (end f third year) t end f furth year pst issuance, $300 may be inviced t WCB withut pre-authrizatin. 4 Manufacturer s invice must be included when billing fr a repair fee and stamped Nt fr Payment. Fee payable nce per transactin with the manufacturer (nt the aid). Where a receiver has been replaced, n fee is billable during the warranty perid f the replacement receiver. Fee Cde Batteries 206 $0.75 per $0.77 per $0.78 per $0.80 per $0.81 per $0.83 per Batteries are prvided t injured wrkers t last till the next service visit withut preauthrizatin. Where a wrker runs ut f batteries, mre may be issued withut pre-authrizatin but, N service fee is billable t the WCB. Fee Cde Fitting & Dispensing 213 $525 $ $ $ $ $ Selectin f apprpriate hearing aid device. Ear mld impressin. Prgramming f the hearing aid device. Real Ear Measurements (REM) fr fitting verificatin. 5 Apprpriate training and cunselling regarding usage f hearing aid device. Cunselling injured wrker regarding the realistic expectatin f the benefits during and after adjustment perid. Fllw up with the injured wrker within the hearing aid manufacturer s nted trial perid (fllw up visit t include adjustments, cunselling, repair and re-prgraming if necessary). **Includes the alltted tw service visits within the first year after new aid is issued including labr n repair, cleaning, re-tubing, vacuuming, readjustments and verificatin. Prvide all prduct manuals and warranty infrmatin; and Any warranty wrk required withut additinal handling/shipping fees. WCB des nt fund extended warranty. Fee Cde Per ear service 214 $30 $30.60 $31.20 $31.80 $32.40 $33.00 Includes minr in huse repairs, perfrmance checks, reprgramming, cleaning, vacuuming, re-tubing, and adjustments. Fee includes all incidental parts and prjects required fr regular maintenance f aids including dmes, tubes, micrphne cvers, wind cvers, wax guards, filters, micrphnes and cmpnent parts. Fr all ther parts, see Repair Fee (Fee Cde 205). Wrker will be prvided a six mnth supply f batteries t last till next service visit, billable t WCB withut pre-authrizatin. Limit f tw visits per year withut WCB pre-authrizatin. fees are billable nly after the ne year anniversary f the fitting/dispensing date as the first year service fees are included in the fitting/dispensing fee. Maximum f ne per ear every tw years withut pre-authrizatin. Only payable if there is an urgent medical need fr the repair r if the wrker is nt in that city n a regular basis fr persnal needs. Fee Cde Fee Custm ear mlds 217 Manufacturer s price plus 10% handling fee and actual shipping fee May be issued withut pre-authrizatin where dmes/tips are nt effective and excessive feedback ccurs.
