To the memory of my Mentor and friend Dr. Edward Delagi And to My granddaughter Laura Adriana and to my grandson Martin Alejandro

PREFACE As with the earlier editions, this Fourth Edition of The Anatomical Guide for the Electromyographer incorporates few new muscles not previously described. It presents, however, a substantial amount of kinesiological information, which I consider the most important addition. This information has been added in the Comments section of each muscle. It is my belief that knowing the functional way in which each muscle acts will help the Electromyographer to analyze and understand better the electromyographic findings. The technique for the insertion of the electrode has also been described in greater detailed in order to avoid common pitfalls in this phase of the procedure. The technique described for the study of the diaphragm is not based on my personal experience, in contrast with all other muscles in this book. It is based on the experience of Dr. P. Saadeh who described the technique in 1993. Finally, a number of bibliographic sources have been added to this fourth edition. A. O. Perotto October 2004 vii

ACKNOWLEDGMENTS First and foremost, I would like to thank the late Dr. Edward Delagi for his steadfast support over many years. I consider myself very fortunate to have had the special privilege of spending 30 years of my professional life working together with him. As a teacher, a colleague and a friend, he was the most honest, intelligent and generous person I have ever known. His passing has left a profound vacuum in the medical community and in my personal life. This edition is dedicated, with my gratitude, to his memory. Once again, I owe special thanks to my wife, Phyllis, for her love, patience and encouragement throughout the long process of revision. To my son, Oscar, for his editorial work, and finally, my thanks go to the Department of Rehabilitation Medicine of the Albert Einstein College of Medicine for their support. ix
