EXAMINATION DATE 14 May 2017 CLOSING DATE FOR BOOKINGS 3 March 2017 R4900.00 COURSE FEE Course fee comprises: Course fee, SAWFA Membership Fee, (Notes, mentorials, refreshments, exam fees and marking etc.) PAYMENT DETAILS CANCELLATIONS COURSE NOTES AND LECTURES CPR PRACTICAL HOURS LOGGED CERTIFICATE WHAT TO BRING WITH YOU Refer to attached course registration form. In case of cancellation by the course participant prior to commencement a cancellation fee of R350 will be charged. Exams have to be written (and passed) within 9 months after course commencement. Failure to attend the exam on the prescribed date will result in a penalty fee of R 400 being charged. All literature will be handed out at registration. All the course manuals & notes and lectures will be in English although help in Afrikaans will be available. The exam paper will be in English. To obtain the instructors certificate, a valid CPR certificate is required. The CPR certificate has to be valid at all times and the CPR has to be renewed timeously. A CPR or first aid certificate has to be handed in on commencement of the course. All participants have to log hours for attending at least 10 classes (preferably with at least 3 different qualified instructors if possible). Instructor names and log sheet will be provided. A SAWFA Certificate of Instruction will be awarded: On presentation of a valid CPR Certificate Successfully passing a Written Examination (Closed book) and a Practical and Oral Examination with a Practical Written Plan of the Work-out. Presentation of the completed Hours logged sheet Swimming Costumes (at least 2 per day) Towels (at least 2 per day) Pen Coloured pencils Paper (in case you want to make additional notes) Own Lunch
