Welcome to Ballarat Pony club for the year 2015-2016 Thank you for joining Ballarat Pony Club! We hope your family enjoys being part of the club and benefits from the many skills gained through participation in the club. The following is a list to help ensure you complete your membership forms correctly. We understand it is a fair amount of paperwork, but this is the one step that ensures your child is fully covered by their 24/7 PCAV insurance, that we can care for them to the highest standard in the event of an accident on our grounds, and that everyone fully understands their commitment to joining the club. So please be diligent in ensuring we have the correct information especially contact and medical information. It is implicit in applying for membership that each family has read and understood the PCAV Code of Conduct form (attached). Please ask at the office if you have any queries. The Volunteer Choices Form is a new idea I hope you will understand the need for it as we plan for the year ahead. Please talk to one of the Committee members if you have any questions or suggestions! Each riding member must have completed: A Club Membership Form for Junior and Associate Riders A Medical History and Emergency Contacts Form At least one parent/guardian must have filled out The Supporter Form The Volunteer Choices Form

The Annual Fee Payments required are as follows: Fees Membership Type PCAV Payment BPC Instruction Affiliation Fees Total Payment Associate 150 193 7 350 Junior 130 193 7 330 Supporter 0 0 0 0 Explanations: Associate: Is 17 on or after July 1 st 2015 Junior: Under 17 on July 1 st 2015 Supporter: A non -riding individual who has signed a Supporter Form. Signing this form gives both insurance cover and voting rights ( if the supporter is 18 or over). It is highly recommended that each family member/friend who may come and help at the Club becomes a Supporter PCAV Payment: Fees include 24/7 Personal Accident Insurance Cover. This insurance enables PCAV riders to claim for injuries sustained whilst engaging in equestrian pursuits at any time of the day. This means riders are covered for Personal Accident outside of Pony Club endorsed activities. BPC Instruction Payment: The money goes directly to paying instructors for our rallies. On average this works out to $4.40/hr a real bargain! Affiliation Fees: Go to PCAV (approx. $1) and Barwon Zone ($6) Please arrange payment by direct debit to: Bendigo Bank Ballarat Pony Club BSB 633000 Account 144990942 Reference: Please use Surname and Riders first initial (or as much as possible) Please insert direct deposit receipt number in box on Membership form (upper left) Thank you, Jo Gell President ballaratponyclub@gmail.com

As this is a very new policy we are working towards it, and would appreciate your assistance. You need to go to www.workingwithchildren.vic.gov.au.
