conclusions Pre-op RT significantly reduces local recurrence Pre-op CRT results in higher rates of pcrcompared to RT alone CRT is associated with higher toxicity c/w RT alone Pre-op RT is associated with better local control compared to post op RT pcrrates are higher when interval from CRT to Sxis more than 8 weeks Longer interval to surgery did not compromise outcomes and was not associated with difference in toxicity rates Hypofractionationwith doses >5Gy appears to increase perioperative mortality and post op morbidity in some reports Pre-op CRT results in better local control compared to post op CRT with no difference in DFS, OS or DM rates Pre-op CRT resulted in significantly fewer complications and higher rates of conversion from APR to LAR

Conclusions: Adjuvant chemotherapy Conflicting data on benefit in patients treated with neo-adjuvant CRT (ncrt) Clear survival benefit in patients treated with surgery Appears to benefit patients with pcr following ncrt Delay in initiation of adjuvant chemo appears to compromise outcomes

W&W conclusions Risk of local regrowth with W&W approach is around 15% and is higher than the risk of regrowth in patients with pcr who had surgery Wait-and-see strategy with strict selection criteria, an appropriate follow-up schedule, and salvage treatments achieved outcomes (DFS, OS, DM) at least as good as radical surgery Robust surveillance with early detection of regrowthsallows a high rate of successful salvage surgery, without an increase in the risk of systemic disease, or adverse survival outcomes More prospective studies are needed to confirm long term safety
