The consent form re-confirms your agreement for Macau Yinkui Hospital to release certain proof or relevant test result(s) to us in order to calculate AIA Vitality Points. Only minimal relevant information data will be transferred to us for the sole purpose of calculating AIA Vitality Points, AIA Vitality will only use store such information data in accordance with the terms conditions of AIA Vitality Programme AIA Vitality Personal Information Collection Statement. Note: It is voluntary for you to sign the consent form.
