Glossary WLR BMI WC Maintaining a healthy weight is essential to good health. If, after a verified weigh-in, your Body Mass Index, waist circumference or both are above the recommended healthy range, Vitality will help you select a weight-loss goal. Reach your goal for the first time and enjoy cash back of up to 50% of your HealthyFood spend for 3 months. Eligibility Who is eligible to join WLR? If, after a verified weigh-in, Vitality members aged 18 to 79 years old have a Body Mass Index (BMI) and waist circumference (WC) that are out of range, they are eligible to join WLR. To be out of range, you must have: A BMI greater or equal to 30 OR a BMI between 25 and 29.9 and a waist circumference greater or equal to 94 cm for males, and greater or equal to 80 cm for females. If you meet these criteria, you ll receive a recommendation to join WLR. However, you will not be able to activate it unless you also have: An active HealthyFood policy with bank details for cash back to be paid into An ios or Android smartphone compatible with the Discovery app The benefit is not intended for members who shouldn t intentionally be losing weight, such as those who are pregnant or breastfeeding, or those who suffer from certain conditions (such as uncontrolled depression or anxiety, Crohn s disease or ulcerative colitis, bulimia and/or anorexia nervosa, or any other eating disorder). These members are not excluded from the WLR recommendation, but prompts will advise them not to activate the programme if any of these conditions apply to them, and to end the programme if any of them arise. The benefit is only available to Vitality members.

2 How will I know if I am eligible to join WLR? If you meet the eligibility criteria, Vitality will recommend WLR to you. You will receive a recommendation to join by and SMS. You will not be able to join Vitality Weight Loss Rewards through the call centre. Activation How do I activate WLR? Download the Discovery app on an ios or Android smartphone. Log in, navigate to the Vitality tab and click on Vitality Weight Loss Rewards. If you are eligible to join, follow the step-by-step process from the WLR app screen. To activate WLR, you must first have an active HealthyFood policy with bank details for cash back to be paid into. Official activation starts once you select a goal for the first cycle of your WLR journey How can I join WLR? If you would like to join WLR, you can go for a verified weigh-in by doing any one of the following: A Vitality Health Check at: o A pharmacy in the Vitality Wellness Network o A Vitality Wellness Centre o A Vitality Wellness Day o Your nearest Discovery Store o Your doctor* A stand-alone weight assessment at: o A pharmacy in the Vitality Wellness Network o A Vitality Wellness Centre o Your nearest Discovery Store o Your dietitian* as part of a broader consultation o Your doctor* as part of a broader consultation A Vitality Fitness Assessment at a biokineticist in the Vitality Wellness Network *If you complete your health checks at a GP or other authentic test facilities, you need to send us your official results by submitting proof of these checks on the Claim Vitality points page. Please allow 48 hours for processing, you can then check that these points have been awarded on your Vitality Points Monitor on the Track and earn points page. Once your points have been awarded, and if you meet the eligibility criteria, you will receive a recommendation from Vitality to join WLR. You will then have 8 weeks to activate it. If you don t do so in this time, you ll have to go for a new verified weigh-in to update your data. You must activate Vitality Weight Loss Rewards yourself on the Discovery app as it cannot be activated by a call centre agent. What is BMI and WC, and how are they measured?

3 Discovery Vitality acknowledges that being overweight and obese increases the risk for developing a range of various health conditions. We assess your weight status using a combination of Body Mass Index (BMI) and waist circumference (WC). BMI estimates your body fat and is calculated using your body weight (in kilograms) divided by your height (in metres), squared. Fat around the stomach area increases one s risk of disease and the waist circumference measurement is used to identify excessive fat around the organs. WC is measured by placing a tape measure around the waist. What is a verified weigh-in? A verified weigh-in is when we get your weigh-in data from an approved source. It comprises of a Body Mass Index (height and weight) and waist circumference check, conducted at the approved Vitality partner. We use verified weigh-ins to start and end a cycle. If you choose to do your weekly weigh-in at a verified partner, you will earn 100 Vitality points for that weigh-in. Please be aware that there may be a cost for this weigh-in. I already went for a verified weigh-in this year why do I need to go for another one to be recommended? Your weigh-in data needs to be less than a month old for us to recommend that you join WLR. Recent weigh-in data is important to help us ensure the weight-loss goals we recommend for you are realistic and safe. What is a self-captured weight? Why do I need to self-capture my weight? Research shows that people who have successfully lost weight, and kept the weight off, weigh themselves every week. It s more practical and sustainable for you to hop on a scale at home instead of going to a particular venue for a verified weigh-in each week. We don t advise that you weigh yourself more than once a week, but rather assess your weekly trend in weight-loss. Remember that it s normal for weight to fluctuate daily due to various reasons such as fluid shifts, what you recently ate and how you recently exercised. For women, fluid retention is normal during a menstrual cycle and this will give you a falsely higher weight. To record your weight, go to Log your weight on the WLR home page and enter your current weight. You ll earn 20 Vitality points for logging your weight once a week. Remember, you can only edit your weight once per week (if you made a mistake when entering or your weight changed), but you will not get any additional points for this. Hopping on and off a scale sounds simple, but there are a few factors to keep in mind to make sure you get the most accurate reading: Use the same scale every time, and ensure that the scale rests on a flat, solid surface, such as a floor tile, instead of a carpet or mat. Remove your shoes and excess clothing, and wear similar minimal clothing every time you weigh yourself. Weigh yourself at a similar time of the day each week. If you weigh yourself after exercising, you may pick up fluid losses, so it s best to weigh under the same conditions, preferably before exercising.

4 You will be able to see your weekly progress within your current and past cycles on the History page on the WLR home screen. You can also see the difference between verified and self-tracked weight logs on the History page. What is the difference between a self-captured weight and a verified weigh-in? Self-captured weight: A self-captured weight is the weight you record weekly by weighing yourself at home. Weigh yourself each week, record your weight on the Log your weight tab and track your progress on the History page (found on the WLR home screen). Verified weigh-in: A verified weigh-in comprises of a Body Mass Index (height and weight) and waist circumference check, conducted at an approved Vitality partner or accredited facility. You will need to go for a verified weigh-in at the end of each cycle, and if you choose, on a weekly basis. Go to What is a verified weigh-in to find out more. Do I need HealthyFood to activate WLR and how do I add my bank details? Yes, to activate WLR, you must first have an active HealthyFood policy with bank details for cash back to be paid into. You can activate HealthyFood on and navigate to Vitality > Nutrition and weight management > HealthyFood. Follow these steps to add cash back bank details: If you are the main member or spouse on the Vitality policy, log into and navigate to Vitality > Nutrition and weight management > HealthyFood to add or update your HealthyFood cash back bank details. If you are a dependant on the Vitality policy, ask the main member or spouse to go online and supply bank details for you. What are the medical reasons I need to be aware of before activating WLR? WLR is not intended for members who shouldn t intentionally be losing weight, such as those who are pregnant or breastfeeding, or those who suffer from certain conditions (such as uncontrolled depression or anxiety, Crohn s disease or ulcerative colitis, bulimia and/or anorexia nervosa, or any other eating disorder). These members are not excluded from WLR recommendation, but prompts will advise you not to activate the programme if any of these conditions apply to you, and to end the programme if any of them arise. If you are eligible to join WLR, you must accept the terms and conditions at the start of each cycle. If you unsure about whether you should participate, please consult your doctor. How long do I have to activate WLR after going for a verified weigh-in? Once you receive a recommendation to join, you have exactly 8 weeks to activate WLR. If you miss the activation deadline, you ll have to meet all the eligibility criteria as well as have a new verified weigh-in to receive a new recommendation.

5 Engagement in WLR What is a WLR journey and what is a WLR cycle? A WLR journey refers to the period of time it takes to achieve your recommended healthier weight. It can be anything from 6 months to a year, depending on the amount of weight to lose. A WLR journey ends when you have reached your recommended healthier weight. Each WLR journey is made up of a number of 12-week cycles. Each time you start a WLR cycle, you ll be asked to select a personalised weight-loss goal for that cycle. A WLR cycle ends when you have reached your goal (or your 12 weeks have ended). You can complete as many cycles as you need to reach your recommended healthier weight, and thus your WLR journey. What is my journey starting weight? This is the weight at which you begin your WLR journey, recorded by your verified weigh-in. What is a recommended healthier weight? During activation, you are set a recommended healthier weight, which is the required weight for you to achieve over the course of your WLR journey. If your Body Mass Index (BMI) is less than 30, your recommended healthier weight will be the required weight to get your BMI in range, or 5% weight-loss. If your BMI is greater than or equal to 30, your recommended healthier weight will be 10% to 15% weight-loss. How are my goals for each cycle determined? At the beginning of a WLR cycle, you need to select a goal weight from a safe but clinically-relevant range. Three different weight levels within the range are offered Level 1, 2 and 3. Each one is associated with a different level of reward (up to Vitality points and up to 50% HealthyFood cash back). These levels, and corresponding rewards, take into account your starting journey weight, your current weight (as per your latest verified weigh-in) and the weight-loss you achieved in your previous two cycles. However, the maximum weight-loss goal Vitality will ever set (in terms of kilograms to lose) in a 12-week cycle is 11 of weight loss, because clinical guidelines state that weight loss of over 1 a week can be unsafe. Below is an example of how the goal levels would work if your starting weight was 100 :

6 Example: Starting weight 100 Level 1 Level 2 Level 3 You can choose a cycle goal within any of the three levels that you re comfortable with The Vitality points you earn for achieving your goal weight in the cycle The additional cash back you earn for achieving your goal weight in the cycle 10% 15% 25% Each cycle has the ultimate aim of helping you reach your recommended healthier weight. Once you do so, you ll move into the final cycle of your WLR journey. The goal for this final cycle will always be to maintain your recommended healthier weight. I have only been given one goal to maintain my weight why? There could be two reasons why you ve been given maintenance as a goal: 1. If you ve had rapid weight-loss in the previous two cycles, you may only need to maintain your weight to earn rewards for your next cycle. 2. If you achieve your recommended healthier weight, you will move into the final cycle of your WLR journey. The goal for this final cycle will always be to maintain your recommended healthier weight. You must do so for one cycle in order to successfully exit the WLR journey. How are rewards for each goal determined within a cycle? By losing weight, you get rewarded with Vitality points and additional HealthyFood cash back. 1. Earn up to Vitality points for every cycle of the journey you complete, depending on the amount of weight you lose: Points for successfully achieving Level 1, 2 or 3 goals for a cycle, as detailed in the table below: Rewards level Level 1 Level 2 Level 3 Vitality points earned Earn additional HealthyFood cash back the first time you achieve your goal You also earn additional HealthyFood cash back the first time you successfully achieve Level 1, 2 or 3 weight goals for a cycle, as follows: Rewards level Level 1 Level 2 Level 3 Additional HealthyFood 10% 15% 25% cash back You can earn additional HealthyFood cash back for your first successful cycle (i.e. the first cycle where you achieve a Level 1, 2 or 3 goal) once in your lifetime only (see exception below). EXCEPTION: If you start WLR with a BMI greater or equal to 30 /m 2 and you successfully complete the full journey (and so earn the additional HealthyFood cash back), but your BMI is still

7 out of range, then you will be eligible for additional HealthyFood cash back one more time, if you activate a new WLR journey. The additional HealthyFood cash back will be paid for three months after the WLR cycle ends i.e. the additional HealthyFood cash back will be applied to the three cash back payments that follow the end of the successful qualifying cycle. This means that once you ve used your additional 3- month HealthyFood cash back, you ll just receive Vitality points for successful cycles going forward. The additional HealthyFood cash back for a particular cycle will expire after it has been paid for three consecutive months. Your additional HealthyFood cash back through WLR is calculated based on the same rules we apply to calculate the HealthyFood cash back (i.e. based on the entire family s HealthyFood spend). Existing HealthyFood caps, payment limits, and cash back terms and conditions will apply, including our right to wait to pay cash back if it is below R50. See the HealthyFood benefit guide for more details on these rules here. The main member on the policy will receive notifications about HealthyFood cash back amounts. You can view your additional HealthyFood cash back on the Discovery app or if you log into How do I know what level of rewards are associated with different goal ranges? When you are selecting a goal on the app, you ll be able to slide across the goal range to see what rewards are associated with each weight-loss goal. Once you have selected your goal for the cycle, you can view the Vitality points and additional HealthyFood cash back you can earn for various levels of weight-loss by going to the 'Points earned' and 'Additional HealthyFood cash back' sections on the WLR home screen. What if I select a goal at the beginning of my cycle and change my mind? Or made an error? The goal you select at the start of the cycle serves as a target for you to aim for. You will be rewarded based on the amount of weight you actually lose at the end of your cycle, no matter what goal level you selected at the start. This means that you can t change your goal mid-cycle, but it doesn t really matter because you will be rewarded for the amount of weight you lose. Ending a cycle When do I go for a verified weigh-in? You can go for a verified weigh-in any time within your window period (weeks 10 to 12 of each cycle). Look out for WLR prompts from your Discovery app. Where do I go for a verified weigh-in? You can go for a verified weigh-in by doing any one of the following: A Vitality Health Check at: o A pharmacy in the Vitality Wellness Network o A Vitality Wellness Centre o A Vitality Wellness Day o Your nearest Discovery Store A stand-alone weight assessment at:

What happens if my home scale is different from my verified weigh-in? Your home scale is used for self-captured weigh-ins. Your verified weigh-in will be used to determine your rewards. Can I go for a verified weigh-in more than once within the verified window period, or I went for a verified weigh-in but think I can lose more weight before the window closes can I go again? Yes, you can go as many times as you wish before the cycle closes. We will use the latest verified weigh-in we receive to determine your reward, as long as it was completed within the verified window period (between weeks 10 and 12 of your cycle). What happens if I did not go for a verified weigh-in within the window period? If you go for a verified weigh-in before the verified weigh-in window opens (i.e. between weeks 1 and 9 of your cycle), the data will only count as a self-captured weigh-in. If you go after your window period for the cycle has closed, the data will not contribute towards determining your reward for that cycle. We will therefore be unable to determine your rewards for this cycle and you will need to restart your WLR journey. There is a problem with my verified weigh-in, how can I resolve it? We can help you. Submit your query online here, or visit the Help page on I went for a verified weigh-in but my cycle closed before the full 12 weeks. Why? If you go for a verified weigh-in and achieve the goal you selected at the beginning of the cycle, the cycle will automatically close, you will get your rewards and progress to the next cycle immediately. What happens if I do not achieve any weight loss, or if I gain weight? There are two possible scenarios: 1. If you don t achieve a minimum amount of weight-loss or weight maintenance to stay in your cycle, or if you didn t go for a verified weigh-in before your cycle closes, your cycle will be considered as failed. This means you will not be rewarded and will have to start a new journey to continue with WLR.

9 2. You get 3 opportunities to maintain your recommended healthier weight in your final maintenance cycle. If you fail at your third attempt, you will not be rewarded and will need to start a new journey to continue with WLR. What happens if I do not achieve the goal I selected at the beginning of the cycle? The goal you select at the start of the cycle serves as a target for you to aim for. You will be rewarded based on the amount of weight you actually lose at the end of your cycle, no matter what goal level you selected at the start. Starting a new cycle I am eligible to continue onto a new cycle. How long do I have to activate the new cycle? You have 8 weeks from the end date of your cycle to activate a new cycle. If you don t activate a new cycle within this window period, you ll have to go for another verified weigh-in to update your data. You ll then receive a new recommendation to join WLR, and have to start a new WLR journey. I have just ended a cycle but have been informed I need to start a new journey why? There may be two possible reasons: 1. If you don t achieve a minimum amount of weight-loss or weight maintenance to stay in your cycle, or if you didn t go for a verified weigh-in before your cycle closes, your cycle will be considered as failed. This means you will have to start a new journey to continue with WLR. 2. You get 3 opportunities to maintain your recommended healthier weight in your final maintenance cycle. You ll need to start a new journey to continue with WLR if you fail at your third attempt of a maintenance cycle. Support in losing weight How can I get help to lose weight? Vitality believes that a healthy, balanced diet and physical activity is important in achieving and maintaining a healthy weight. We do not prescribe any particular method for weight-loss, but encourage you to choose an approach that suits your individual preferences and health needs. 10 What is safe weight loss? Clinical guidelines state that weight loss of 0.5 to 1 a week is healthy and sustainable. As such, it s best to stay active and follow a healthy, balanced diet in order to lose kilograms in a safe and sustainable way. This will increase the likelihood of maintaining a new, lower weight in the future. Completing WLR I have reached my recommended healthier weight, now what? Each cycle has the ultimate goal of getting you to your recommended healthier weight. Once you achieve your recommended healthier weight, you ll move into the final cycle of your journey. The goal for this final cycle will always be to maintain your recommended healthier weight. You get 3 opportunities to maintain your recommended healthier weight in your final maintenance cycle. If you fail at your third attempt, you won t be rewarded and will need to start a new journey. If you maintain your recommended healthier weight for your final cycle, you will have achieved your journey goal and completed your WLR journey! What happens if you fail final maintenance? You get 3 opportunities to maintain your recommended healthier weight in your final maintenance cycle. If you fail at your third attempt, you won t be rewarded and will need to start a new journey to continue with WLR. I have successfully achieved my recommended healthier weight and ended my journey. Why is the app recommending I start a new journey? This could be the case if your Body Mass Index (BMI) was greater or equal to 30 when you started your WLR journey and your recommended healthier weight helped you achieve a 10% to 15% weight loss. If your BMI is still out of range at the end of your WLR journey, you may receive a recommendation to start a new journey. If you do so, you ll get a new recommended healthier weight, which may bring your BMI in range. Ending WLR or opting out How do I end the programme if I no longer wish to or can no longer participate? If you have an active journey and want to leave, select End programme on the Discovery app at any time. Just remember that if you leave WLR, all your weight history data will be lost. So if you choose to re-enter later, you will have to restart your journey from the beginning. You can activate a new journey at any time, as long as you meet all the eligibility criteria.
