Every bit of information will be used by the Faculty Council to assist with the fair consideration of your candidacy during the selection process. As this is a student organization, students applying for selection should be the ones to complete this form. Please print with blue or black ink ONLY. VI. Letter of Intent: Please address a formal letter to the members of the Faculty Council who are selecting you for membership. Your letter should address all of the following key ideas concerning why NJHS will benefit from having you as a member: - How do you demonstrate respect, responsibility and pride both in and out of school? - How much time are you willing to commit to your membership with NJHS? Will you be comfortable making service and leadership a priority among your other middle school obligations? How will you balance your time and energy? - What do you hope to gain by being a member in NJHS? How will membership help you grow (personally and/or academically) in middle school? - Finally, why did you choose to complete the process of becoming a member of NJHS? Letters must be submitted as a student-written, hand-written copy on this Student Activity Information Form only. Letter may be no longer than the provided form.

NATIONAL JUNIOR HONOR SOCIETY 2017-2018 CHAPTER BY-LAWS CENTENNIAL MIDDLE SCHOOL REQUIREMENTS TO BELONG TO THE CENTENNIAL CHAPTER: A. Maintain a 3.85 grade point average, or higher, each quarter. B. Demonstrate character, leadership and service - the characteristics by which you were selected. C. Participate in club service projects. D. Complete minimum requirements for individual service each school year. E. Attend all Chapter Meetings. One excused missed meeting per year is allowed. More than one excused missed meeting will result in his/her membership being revoked. CONSEQUENCES FOR BREAKING THE CHAPTER BY-LAWS: A. FAILURE TO MAINTAIN GRADE POINT AVERAGE: 1. Automatic probation a minimum of one quarter. The student has one quarter to achieve the required gpa for membership. A letter will be sent home to notify parents. 2. If while on probation the student does not achieve the required gpa by the end of one quarter, his or her membership will be revoked. A letter will be sent home to notify parents. B. FAILURE TO DEMONSTRATE CHARACTER: 1st offense: Written warning and/or probation 2nd offense: Letter of probation/dismissal 3rd offense: Dismissal C. PROBATION: Probation is effective for nine weeks from the date of the probation letter. Members on probation are still required to participate in all NJHS meetings, individual service hour commitments, and group service projects. D. DISMISSAL The student who is notified of dismissal is required to appear before the Advisor, Principal and/or Assistant Principal, and a member of the Faculty Council at which time the student may give an explanation. Parents may accompany a student to this hearing. Any student dismissed from NJHS may never be a member of another chapter.

YEARLY MEMBERSHIP FEE: A yearly fee of $50.00 is considered a tax credit donation. Please download the NJHS tax credit donation form on the NJHS Forms Page and submit it along with the membership fee at the August 31 st Mandatory Chapter Meeting. The Centennial chapter of NJHS strives to have members pay for as little as possible for participation in our honorary. The membership fee supports Centennial s NJHS including the membership t-shirt and pin, and the cost of the National Membership Affiliation. SERVICE: A. All members will be required to complete 16 hours of individual service hours to the school and/or community during the school year. At least 8 of the 16 hours MUST be completed at a Kyrene School District site. B. Parent(s) are encouraged to sponsor or assist with service projects. Students participating in service projects are expected to behave as they would at a school function and to follow directions set by teachers or parents/sponsors. CHAPTER MEETINGS: A. Chapter Meetings are held one morning every month from 7:30-8:15 in the Centennial Library. Dates and times are listed on the NJHS Website Calendar. Members are expected to attend all meetings. One excused missed meeting per year is allowed. More than one excused missed meeting will result in his/her membership being revoked. If you sign up to receive NJHS Remind texts, be aware that it is a courtesy reminder and that technical problems may arise. You may not use not seeing/not receiving a Remind text message as an excuse for missing a mandatory chapter meeting. Always refer to the NJHS Website calendar for meeting dates and times.
