Cornea overview Dr. Sarita Tuladhar MD, Ophthalmology Gandaki Medical College Embryology CORNEA: n Corneal epithelium is derived from surface ectoderm n Corneal stroma, descement memb, bowman s layer, endothelium is derived from fibrous layer of mesoderm lying anterior to the optic cup. CORNEA n Transparent, avascular tissue n Anterior 1/6 of the outer fibrous coat of the eyeball n 11-12mm horizontally, 10-11mm vertically n Corneal thickness at the center 0.52mm, at the periphery is 0.7mm n Ant. & posterior radius of curvature is 7.8mm & 6.7mm respectively. n Net refractive power - +43D, refractive index 1.37 HISTOTOGY Epithelium n Composed of stratified squamous epith. n 5% of total corneal thickness (50-90micro m thick) n 5-6 layers of cells, deepest is basal cells, next 2-3 layers is wing or umbrella cells & most superficial is surface cells n Entire epith. is replaced by 6-8 days 1

n Basal layers n tall columnar polygonal shaped cell n palisade like manner n forms the germinal layers of the epith n firmly joined by desmosomes & maculae occludentes n tight intercellular junctions accounts for epith. & its barrier function n Wing cells: 2-3 layers of polyhedral shaped cells n Surface cells: n most superficial, desmosome & maculae occludens are more numerous n ant. cell wall has microvilli which plays an import role in tear film stability Bowman s membrane: n Consists of acellular mass of condensed collagen fibrils n 8-14micro m thick n Resistance to infection & injury. Once destroyed it doesn t regenerate n 0.5mm thickness Stroma n Constitutes 90% of total thickness n Consists of collagen fibrils (lamella) & cells embedded in hydrated matrix of proteoglycans n Lamellae Becomes continuous with scleral lamellae at the limbus Parallel arrangement of lamellae allows easy dissection & also implicated in the corneal n Cells keratocytes, macrophages, histocytes, lymphocytes Descemet s membrane: n Strong homogenous layer n It is made up of collagen, glycoprotein & no elastic fibers visible by electron microscope n 3microm at birth & 10-12 microm in young adults n Resistant to infection, injury, chemical agents n Unlike bowman s memb when destroyed it can regenerate n Appears to end at anterior limit of the trabecular meshwork as Schwalbe s line 2

Endothelium n Single layers of flat polygonal cells, appears as mosaic n Cell density of endoth. is 6000cells/mm 2 at birth, 2400-3000 cells in young n Defect left by dying cells is filled by polymegathism of remaining cells n Corneal decompensation occurs when cell counts becomes <500 cells/mm 2 BLOOD SUPPLY: n Endothelium contains an active pump mechanism & is involved in active secretion & protein synthesis n Next to the photorecepter, endothelial cells contains the highest no.of mitochondria n Avascular n Small loop derived from the ant.ciliary vessels invade its periphery for about 1mm & provide nourishment NERVE SUPPLY: n Derived from long ciliary nerves branch of nasociliay nerve n It connect with conjunctival nerves to form a pericorneal plexus of the nerves n 60-80 myelinated trunks from the pericorneal plexus enters the cornea & in the stroma it lose their mylein sheath & form a stromal plexus n Most passes anteriorly and forms a subepithelial plexus n Fibers penetrate the bowman s memb. lose their schwan s sheath, divide under basal layer of epith & extend bet cells of all layers of epith.& forms a intraepith plexus n Highest near the centre & decreases towards periphery n Absent nerves in post part of cornea, descemet memb, endothelium 3

Physiology of cornea n Two primary physiological functions of cornea n To act as a major refracting medium n To protect the intraocular contents Corneal n Transparency is the quality or state of transmitting light without appreciable scattering so that bodies lying beyond are entirely visible n It is of paramount importance for the cornea to satisfy the basic physiologic function as a major refracting medium, for a clear retinal image Factors affecting corneal Physical/anatomical factors v Uniform and regular arrangement of the corneal epithelium v Peculiar arrangement of the stromal lamellae v Corneal avascularity Physiological factors v Relative state of corneal dehydration Cellular factors Corneal epithelium and tear film n Normal epithelium is transparent due to homogeneity of its refractive index n The basal cells are firmly joined laterally to each other and anteriorly to the wing cells by desmosomes and maculae occludentes n This accounts for its and barrier function n Normal precorneal tear film also plays an important role in maintaining the n Conditions associated with tear film abnormalities and/or corneal epithelial abnormalities may result in loss of its Arrangement of the stromal lamellae - Two theories n Maurice theory (1957) Uniform collagen fibrils are arranged in a regular lattice so that a scattered light is destroyed by the mutual interference As long as these fibrils are regularly arranged in a lattice, seperated by less than a wavelength of light (4000 to 7000 Å), the cornea will remain transparent Theory of Goldman et al.(1968) n Lattice arrangement is not a necessary condition for stromal n Cornea is transparent because the fibrils are small in relationship to the light and do not interfere with the light transmission unless they are larger than 1 ½ of a wavelength of light (2000Å) Loss of will result if this regular arrangement is altered by stromal edema or mechanical stress 4

Physiological factors v Relative state of corneal dehydration maintained by barrier effects of epithelium and endothelium and the active bicarbonate pump of the endothelium Cellular factors affecting n Corneal fibroblast (keratocytes) are important in maintaining as they are source of stromal collagen, and proteoglycans n Most of the enzymes required for the changes in the matrix are present in them Source of nutrients n Solutes ( glucose and others) enter cornea by either simple diffusion or active transport through aqueous humor and by diffusion from perilimbal capillaries n Oxygen is derived directly from air through the tear film by epithelium Lacrimal gland n Is formed from a no. of buds from the upper angle of conjunctival sac Lacrimal sac & nasolacrimal duct n are derived from nasolacrimal furrow Extra ocular muscles n Derived from mesenchyme 5
