2 SJCG Strategic Plan 2016-2020

MISSION St. Joseph s Care Group is a Catholic organization that identifies and responds to the unmet needs of the people of Northwestern Ontario, as a way of continuing the healing mission of Jesus in the tradition of The Sisters of St. Joseph of Sault Ste. Marie. VISION St. Joseph s Care Group is a leader in client-centred care. CORE VALUES CARE St. Joseph s Care Group will provide quality care for our clients body, mind and spirit, in a trusting environment that embraces diversity. COMPASSION St. Joseph s Care Group will demonstrate dignity and respect for those in need, accepting people as they are, to foster healing and wholeness. COMMITMENT St. Joseph s Care Group is committed to our community, the people we serve, the people we employ, and our faith-based mission through a continued pursuit of excellence. SJCG Strategic Plan 2016-2020 3

FOCUSING ON OUR CLIENTS, OUR PARTNERS, OUR PEOPLE, AND OUR FUTURE Our Strategic Plan 2012-2015 ushered in one of our largest periods of growth since The Sisters of St. Joseph of Sault Ste. Marie first opened our hospital in 1884. Some called the plan overly ambitious, but we are proud to have accomplished much of what we set out to do. Our construction projects are certainly the most visible of our changes over the past four years with the completion of Sister Leila Greco Apartments, the Hogarth Riverview Manor Expansion, and the new East Wing of St. Joseph s Hospital, set to open in Fall 2017. More important are our evolving models of client-centred care. We are continuing to advance a self-management approach to care, and we are involving our clients more closely in their own care decisions to achieve the health outcomes that are important to them. We are using technology to help overcome challenges to healthcare including the sheer size of Northwestern Ontario to bring care closer to the people we serve. In other words, we are accomplishing our Mission of responding to the unmet needs of our clients throughout Northwestern Ontario, and improving our level of care all the time. This Strategic Plan 2016-2020 sets out our strategy to build upon that success over the next five years. Tracy Buckler Linda Pauluik 4 SJCG Strategic Plan 2016-2020

After this period of rapid growth, we are focusing on the delivery of our new commitments and maintaining our excellent level of care. We are on the right track. Our staff and volunteers are proud to work at St. Joseph s Care Group, and our clients tell us how every day our people go above and beyond expectations, and live our values of Care, Compassion, and Commitment. We will continue to set the bar higher. Our single most important goal is to enhance the client-centred care we provide today and ensure we continue to meet the needs and expectations of Our Clients, Our Partners, Our People, and Our Future. Tracy Buckler President and Chief Executive Officer Linda Pauluik Chair, Board of Directors President Our staff and volunteers are proud to work at St. Joseph s Care Group, and our clients tell us how every day our people go above and beyond expectations, and live our values of Care, Compassion, and Commitment. Tracy Buckler and Chief Executive Officer SJCG Strategic Plan 2016-2020 5

ST. JOSEPH S CARE GROUP OUR STRATEGIC DIRECTION St. Joseph s Care Group is proud to continue providing client-centred care in a way that is sustainable and meets the needs of the people that we serve: we are here for you when you need us. Our role is to deliver high quality services in addictions & mental health, rehabilitative care & chronic disease management, and seniors health, and to support our local and regional partners in these areas. As we advance our vision of becoming a leader in client-centred care, we plan through the lens of Quality, Safety and Risk. Our Strategic Plan 2016-2020 will focus on four key Strategic Directions: ST. JOSEPH S CARE GROUP IS HERE FOR YOU WHEN YOU NEED US: QUALITY Here for our clients Here for our partners SAFETY Here for the future Here for our people 6 RISK

WHAT WE INTEND TO DO OUR STRATEGIC PRIORITIES FALL UNDER FOUR KEY STRATEGIC DIRECTIONS: This Strategic Plan lays out our directions and priorities over the next five years to ensure we continue to focus on our unique role in healthcare, meeting the needs of our clients and our partners. That includes recognizing the unique partnerships we must continue to develop with Indigenous Peoples in Northwestern Ontario. We will also focus on our people and the resources that will enable us to continue to provide excellent care and leadership in the future. Here for Our Clients We will address unmet needs through our programs and services Here for Our Partners We will collaboratively define our role in the provision of care for our communities Here for Our People We will work to advance our culture and continue our caring mission with our staff and volunteers Here for Our Future We will plan for continued financial sustainability SJCG Strategic Plan 2016-2020 7

HERE FOR OUR Clients WE WILL BE HERE FOR OUR CLIENTS AND ADDRESS UNMET NEEDS THROUGH OUR PROGRAMS AND SERVICES BY: ALIGNING OUR SERVICE OFFERINGS WITH THE NEEDS OF OUR COMMUNITIES Develop and implement clinical services plans for Addictions & Mental Health, Rehabilitative Care & Chronic Disease Management, and Seniors Health Develop and implement health human resources plan for clinical and non-clinical staff Improve access to quality services across Northwestern Ontario EVOLVING OUR CLIENT-CENTRED CARE APPROACH Involve clients and caregivers in care process and organizational planning 8 SJCG Strategic Plan 2016-2020

Our clients and their families are our partners in care throughout their healthcare journey. Through innovative technologies and partnerships, we are finding ways to bring care to the region, closer to the comforts of home. Janet Sillman, Vice President Addictions & Mental Health SJCG Strategic Plan 2016-2020 9

HERE FOR OUR Partners WE WILL WORK COLLABORATIVELY WITH OUR PARTNERS TO DEFINE OUR ROLE IN CARING FOR OUR COMMUNITIES BY: DEFINING OUR REGIONAL ROLE Work with our partners to identify our regional role for Addictions & Mental Health, Rehabilitative Care & Chronic Disease Management, and Seniors Health CLARIFYING EXPECTATIONS AND FOCUSING ON CAPACITY BUILDING FOR SPECIALIZED SERVICES AND SUPPORTS Bring regional health care providers together to understand existing services, identify gaps in service, and find ways to meet those gaps Collaborate on opportunities to build capacity through shared knowledge 10 SJCG Strategic Plan 2016-2020

The success of partnerships will be realized through improved health outcomes across Northwestern Ontario, because we will have found ways to increase capacity and share expertise in a way that meets the needs of clients. Kathleen Lynch, Vice President Rehabilitative Care & Chronic Disease Management SJCG Strategic Plan 2016-2020 11

HERE FOR OUR People WE WILL BE HERE FOR OUR PEOPLE, AND WILL WORK TO ADVANCE OUR VISION AND CONTINUE OUR MISSION BY: INVESTING IN OUR PEOPLE Provide the tools, skills and supports to keep staff and volunteers safe and well Build a comprehensive staff and volunteer development plan to support people to grow their career and experience with St. Joseph s Care Group PROMOTING OUR VALUES-BASED CULTURE OF CARING Be deliberate in promoting our values Help leaders to be Mission Managers 12 SJCG Strategic Plan 2016-2020

Our staff and volunteers are essential to providing client-centred care and maintaining our unique culture of Care, Compassion, Commitment. We seek feedback on our culture from staff, clients and families to make sure that we are living our values. Myrna Holman, Vice President People, Mission and Values SJCG Strategic Plan 2016-2020 13

HERE FOR OUR Future WE WILL BE HERE FOR OUR FUTURE THROUGH THE RESPONSIBLE USE OF RESOURCES BY: PLANNING FOR CONTINUED FINANCIAL SUSTAINABILITY Align our care and services to meet provincial funding expectations OPTIMIZING FUNDING Continue to find efficiencies within existing programs and services Explore options including streamlining, integration, and reducing duplication in the system FOCUSING ON OUR ROLE AND HAVING THE RIGHT RESOURCES IN PLACE Evaluate our programs and services relative to our Mission Work efficiently and collaboratively to transition services when more appropriate providers of service are identified 14 SJCG Strategic Plan 2016-2020

While we have always had to effectively manage our finances, now more than ever our funders are insisting on increased efficiency and accountability. We want to make sure that we are remaining true to our Mission while continuing to provide safe, high-quality programs in a way that is sustainable now and for our future. Scott Potts, Vice President Infrastructure & Planning SJCG Strategic Plan 2016-2020 15

HOW WE DEVELOPED OUR PRIORITIES A strategic plan is a complex document. It requires an understanding of where we are as an organization, where we want to be in five years, what challenges we are likely to face, and how best to overcome those challenges. That s why research and engagement are so important. St. Joseph s Care Group completed an environmental scan to better understand the future needs of the people that we serve and trends in healthcare delivery, as well as provincial strategy and funding approaches. None of the data are meaningful until we understand what is important to our clients, our tenants, our residents, our staff, our volunteers, and the populations that we serve. That is why St. Joseph s Care Group conducted multiple interviews, focus groups, surveys, meetings, and planning sessions. We spoke with our clients, volunteers, direct care staff, clinical leaders, professional staff, management team, board of directors, service delivery partners, and our local and regional stakeholders. We gained insight into the things we do that are valued, how data are viewed through the lens of local experience, and about opportunities to meet unmet needs. We would like to extend our heartfelt thanks to all for sharing your candid feedback, experiences, and your hopes for the future. Seeking input from clients and families on this Strategic Plan was a very natural thing for us. It is an extension of how we work together whether we re developing a care plan or developing a new building and environment for service. We are here because of you. 16 SJCG Strategic Plan 2016-2020
