SDAFP Oktoberfest Member Event We are excited to announce a special event for: SDAFP Members Family Medicine Physicians, Family Medicine Residents and Medical Students! We will have several displays with our information regarding SDAFP Activities! On Saturday October 24th we will be having two SAM Sessions Hypertension and Cerebrovascular Disease (separate fee for SAM session) Our Event is purely a social event intended on bringing together our physician members for an evening of fun! We welcome our medical student and resident members to attend as our guests! Stop by and learn a little about some different micro brewery s and how to pair certain wines! We will even have some scotch sampling! They are Dr. Reid Blackwelder current AAFP Board Chair and Dr. Robert Wergin current AAFP Board President. They will give an update on the AAFP Health is Primary Campaign and be around to address member concerns regarding the AAFP. This premier event will take place at Great Bear Ski Lodge in Sioux Falls, SD on Friday October 23, 2015 from 6:30-8:30 PM. The event will include hors d oeuvres and we will be sampling and learning about a selection of Micro Brew Beers, Wines and Scotches. This will be a fundraising event with the proceeds going to sponsor student and resident activities and conference scholarships. SDAFP fees and payment: Registration fee is $200 for both sessions or $125 per session: Sessions: 9:00 Cerebrovascular Disease 1:00 Hypertension I will mail in my check (payable to the SDAFP) I will pay on site ABFM #: SDAFP/AAFP #: IMPORTANT NOTES: You may log onto the American Board of Family Medicine (ABFM) BEFORE the SDAFP SAM Group Learning Session on and pay the fee for Maintenance of Certification. The amount depends on when you recertify. You pay this fee directly to the ABFM. You must provide me with your ABFM number or your AAFP number so that I can register you for the group session. If you prefer to pay the ABFM fee after the session then when you log on to do the clinical assessment portion of the SAM it will require you to make payment to continue. The fee you pay the SDAFP is to offset venue/materials/research and speaker. You do not need to bring a laptop to this session. Lunch will be served to all registrants in attendance on site.
