Caregiver Advocacy Program provides free information and consultation to caregivers of individuals age 60+ or with dementia. Also provides options for respite care, case management services, and medical equipment; fees vary. Southeast Ohio Aging and Disability Resource Network (SE Ohio ADRN) hosts the Network of Care website which features an easy to use Service Directory of local agencies and organizations, a Health Library, a listing of Assistive Devices, and a free on-line electronic personal health record. The SE Ohio ADRN helps to streamline services and supports for older adults, people with disabilities and their caregivers. http://seohioadrn.oh.networkofcare.org Project Lifesaver provides a free rapid response search and rescue service designed for individuals who wander. Individuals must have 24/7 caregivers and wear a unit that is checked monthly. Passport Program provides in-home alternatives to nursing home care for qualified low-income older adults. Services include personal care, homemaker services and respite or adult day care. Senior Housing Program provides home assessment, home mobility modifications, and minor home repair services for seniors age 60+ with annual income at or below 35% of area median income. Long-Term Care Ombudsman Program advocates for quality care and negotiates for the solution of problems that arise between providers and consumers of long-term care services, including nursing homes. Assisted Living Waiver Program pays for a variety of assisted living services for older adults who are Medicaid eligible. All participants receive a Medicaid card to assist with physician visits, nursing services, and other services.
