MIS EDUCATIONAL COURSE OFFERING DESCRIPTIONS 4 Advanced Concepts in MIS Summit The MIS summit is an annual, two day course designed for surgeons highly experienced with MIS procedures who want to expand their MIS practice. This unique course is customized to the learner s MIS experience and areas of interests. Key Opinion Leaders will lead complex spine case presentations and discussion on variety of MIS topics including, but not limited to adult deformity, adolescent deformity, trauma, tumor, image guidance, revision surgery, complication avoidance and emerging technologies. Key learnings will be reinforced by hands-on bioskills training sessions using cadaveric specimens. Concepts in Minimally Invasive Spine Surgery Spine surgeons with little or no experience with MIS procedures in the lumbar spine and who are interested in developing their MIS skills are invited to attend this type of course program. Course participants should have a strong interest in adding MIS techniques to their armamentarium. The agenda includes a full day of hands-on training using cadaveric specimens supplemented by lectures from experienced surgeon faculty. Course lecture and bioskill session topics will include: Minimally Invasive Discectomy & Decompression, Minimally Invasive TLIF, Percutaneous Transpedicular Fixation, Management of Potential Complications in MIS and Lateral Approach for Interbody Fusion. Advanced Concepts in Minimally Invasive Spine Surgery This advanced program is designed for surgeons experienced with MIS procedures who want to learn new techniques and advance their MIS expertise and procedure confidence. The agenda focuses on case presentations and discussion, hands-on training using cadaveric specimens, and lectures given by an experienced surgeon faculty. Topics include: MIS for Adult Deformity, Trauma, Tumor, Image Guidance, Revision Surgery and Complication Avoidance. MIS Principles & Case Preparation Spine surgeons with little or no experience with MIS procedures in the lumbar spine and who are interested in adding MIS techniques to their practice are invited to attend this type of course program. This program provides a unique opportunity for spine surgeons to advance their MIS education through skill development. MIS Principles and Case Preparation courses allow for collaboration in a small group setting of 4-6 participants led by surgeon faculty and the DePuy Synthes Spine s Surgical Innovation Specialist Team. The agenda includes one full day of faculty presentations, case-based exercises & discussions and hands-on practical lab sessions. Complex MIS Peer Meetings Spine surgeons with MIS experience and whose practice is in part dedicated to treating patients with MIS techniques are invited to gain additional MIS procedural confidence by attending a Complex MIS Peer Meeting. This unique training opportunity allows surgeons to advance their MIS education and training through small group collaboration among peers, surgeon faculty and DePuy Synthes Spine s Surgical Innovation Specialist Team. The course agenda includes discussion on best-practices, faculty presentations, case-based exercises & discussions and hands-on practical lab sessions. Advanced MIS Tutorials The tutorial programs are unique training opportunities that allow extended time with thought leaders in specific areas of MIS. The two day tutorial includes live surgery observation, case and technique discussion and hands-on practical lab sessions. Course size is limited to 4-6 participants to allow for peer collaboration and a low learner to faculty ratio.

Supplementary Education Worldwide Headquarter Visits Physicians, nurses, operating room staff and associated medical professionals are invited to visit the DePuy Institute, LLC in Raynham, MA. During the visit, you will be given the opportunity to learn more about the DePuy Synthes companies and interact with our products and people. This customized visit may include a tour of the DePuy Synthes facility, interaction with DePuy Synthes senior management and/or a hands-on educational experience that we are confident will exceed your expectations. OR Visitations The Education Through Visitation (ETV) program is designed to provide the opportunity to meet experienced surgeons in either a one-on-one or small group setting. Participants attend a live surgery and discuss surgical techniques, indications and clinical results. Program agendas are customized to suit the participant s surgical area of interest. Product Evaluations Product evaluation labs are intended for spine surgeons who are familiar with one or more surgical approach, technique or competitive product. This program offers surgeons an opportunity to learn DePuy Synthes Spine products and the associated surgical techniques prior to adopting them in the operating room. Surgeon to Surgeon Consultation A Surgeon-to-Surgeon Consultation allows surgeons an opportunity to review challenging on-label cases and current technique and treatment options in an informal setting. This consultation may include an onlabel X-Ray review to discuss the safe and effective treatment options for a patient while adhering to HIPAA guidelines. All cases must be pre-submitted for regulatory review prior to discussion. Dinner Meetings A dinner meeting allows the healthcare professional an opportunity to engage in lively discussion and debate with surgeon experts in an informal setting. The instructor provides lecture and case review on current clinical issues relating to the safe and effective use of DePuy Synthes Spine products. Grand Rounds Program A grand rounds program is typically offered in support of an academic institution or hospital program. DePuy Synthes Spine is proud to support these opportunities directly when the instructor/ presenter is teaching on the safe and effective use of DePuy Synthes Spine products. Education for Allied Professionals DePuy Synthes Spine provides accredited and non-accredited learning opportunities for allied professionals. Educational courses focus on pre and post-operative care, anatomy, biomechanics, foundational spine pathology and surgical solutions for the spine. Proctorships Proctorships are designed to provide a service by which an experienced surgeon educator will travel to a less experienced surgeon s operating room to attend an on-label live surgical case. The instructor will discuss the safe and effective use of DePuy Synthes Spine products while educating the learner on surgical techniques, indications and clinical results. Proctorships may be available in limited circumstances as part of advanced surgical training. 5
