Endometrial biopsy/scratch Why? Several studies have indicated that endometrial injury may induce changes in the endometrium that improve reproductive outcomes in women undergoing ART cycles How is this done? Most commonly with a pipelle the endometrial injury is performed between day 7 of the previous cycle and day 7 of the ET cycle versus no intervention or a sham procedure How does this work? - exact mechanism unclear however theories include: injury to the endometrium induces decidualisation which prepares the uterus for pregnancy induces a wound healing response, which involves recruitment of immune cells thought to be important in implantation Slows endometrial maturation which is thought to be too fast in some ART cycles Cochrane Database of Systematic Reviews 2015

Endometrial biopsy/scratch: Although current evidence suggests some benefit of endometrial injury, we need further evidence from welldesigned trials that avoid instrumentation of the uterus in the preceding three months, do not cause endometrial damage in the control group, stratify the results for women with and without recurrent implantation failure (RIF) and report live birth rates.
