Wellness Assessment The Wellness Assessment is an evidence-based tool that gives participants a better understanding of their overall health. It collects information about individual behaviors and provides feedback to improve and maximize personal health. Immediately upon completion of the online Wellness Assessment, personal feedback is provided with customized recommendations for improvement, as needed. Entering biometric data is strongly encouraged, but optional. Participants may complete the online Wellness Assessment once per calendar year and wellness credits will be awarded automatically. Once the Wellness Assessment is submitted and saved as final, participants can no longer make changes. Wellness Credit Value: 300 Health Preventive Exam and Screenings Participants are encouraged to have their routine annual physical exams and preventive screenings with their health care provider. Exams and screenings may include annual physicals, biometric screenings, preventive screenings, blood work, or any other age/gender appropriate screenings. Upon completion of an exam and screening, the participant will complete an online affidavit form to receive wellness credits. Wellness Credit Value: 300 Vision Exam Participants who have completed their vision exam by their health care provider will receive 50 wellness credits. Upon completion of their vision exam, the participant will complete an online affidavit form to receive wellness credits. Wellness Credit Value: 50 Dental Exam Participants who have completed their dental exam by their health care provider will receive 50 wellness credits. Upon completion of their dental exam, the participant will complete an online affidavit form to receive wellness credits. Wellness Credit Value: 50 2
Healthy Living Programs (HLP) Healthy Living Programs are interactive, personalized, six-week online programs that help participants take important steps to reduce risk and improve health. All HLPs include nutrition, physical activity, and weight management components. Once a participant completes their Wellness Assessment, the top three recommended HLPs are automatically suggested based on identified health risks and the participant s stage of readiness to make lifestyle changes. Several HLP topics are available and can be completed by the participant. Wellness credits are awarded automatically upon the completion of each HLP. Healthwise Conversations Online Healthwise Conversations are engaging, interactive modules that help individuals manage important health issues. Featuring user-friendly hosts, fun graphics and clinically backed content, conversations speak directly to the participants needs. Each experience is unique, because the tool uses branch-logic technology. Participants select a health topic they are interested in learning more about. An interactive animated conversation will play and questions are asked throughout the chosen conversation. Depending on the answer a participant provides, the conversation uses branch-logic technology to play a segment that is specifically geared to that answer. At the end, participants receive a printable summary page, based on their input including links to content within the portal. Credits are awarded automatically upon completion of each conversation. 2013 Healthwise Conversations: About Healthy Living Preventing Falls Healthy People Get Active Healthy Eating Healthy Thinking Quitting Smoking Again Healthy Weight Dealing with Stress Sleeping Well About Diseases & Conditions Asthma and Controller Medications Beta Blockers and Your Heart Daily Aspirin to Prevent Heart Attacks Diabetes and A1c Testing Low Back Pain Medicines to Treat Depression Statins for People with Diabetes or Heart Problems Wellness Credit Value: 10 credits per each completed Online Healthy Conversation Healthy Worksite Activities/Local Events Participants will be able to earn credits for participating in a local wellness event designated by their local wellness coordinator. Each group is encouraged to select or create activities that have special appeal to local employees. Criteria will be established for what activities qualify. Examples: Mini Challenges; Stress Management Classes; Nutrition Classes; Safety Classes; Ergonomic Classes; Group Exercise Classes; Special Events; etc. Employees are also encouraged to contribute to the well-being of the community by participating in community events that involve physical activity. This is a great opportunity to promote group activities. Some examples include Komen Foundation Walk/Run, American Heart Association Heart Walk, March of Dimes Walk, Habitat for Humanity, and various fun runs, bicycling events, triathlons, walk-a-thons, etc. Upon completion of the online affidavit form, the system will automatically award wellness credits. Wellness Credit Value: 25 (Maximum 200 wellness credits) 4
Wellness Challenges Wellness Challenges are designed to give participants the opportunity to practice healthy behaviors. There is abundant research data that show these behaviors to enhance the health and quality of life for individuals that include them in their daily lifestyle. The key behaviors focus on physical activity, nutrition, weight management, and stress management. Challenges require the participant to record daily behaviors to meet the criteria of the challenge. Upon reaching the challenge goal, credits are awarded automatically. To assist with daily tracking, participants in the challenge activities may choose to use a manual paper-tracking tool. Ultimately, the participant will be required to transfer their tracking to WellwithBlue SM wellness portal in order to receive wellness credits. Daily activity is tracked as points, and points roll up into credits. Get Heart Smart Challenge February 1 March 8 The Get Heart Smart Challenge is designed to help the participant build two heart smart activities into their life: physical activity and a healthy diet. Physical activity can reduce or eliminate many risk factors such as high blood pressure, diabetes and excess weight. A healthy breakfast gives people the energy they need to handle the physical and mental stress of the day ahead. In addition, breakfast eaters tend to meet their recommended targets for nutrients and they manage their weight better than those that skip breakfast. Participants can earn bonus points for having their blood pressure checked during this challenge. Wellness Credit Value: 100 for Challenge completion (Participant must earn at least 45 points to complete the challenge and receive 100 wellness credits.) Bodies in Motion Challenge April 15 May 17 The Bodies in Motion Challenge is a challenge that encourages physical activity. Regardless of the participant s current activity level, this challenge focuses on creative ways to incorporate more physical activity into daily routines. It emphasizes behaviors like taking more steps, carrying groceries instead of using a cart, standing instead of sitting, parking farther out in the parking lot, etc. Moderate amounts of physical activity can help you feel better both physically and emotionally. Wellness Credit Value: 100 for Challenge completion (Participant must earn at least 65 points to complete the challenge and receive 100 wellness credits.) 5
Healthy Lunchbox Challenge September 9 October 11 The Healthy Lunchbox Challenge is designed to help the participant eat a nutritious lunch on a daily basis. Behavioral changes and educational opportunities on nutrition are the center of this challenge. The participants focus on making time in their workday for a healthy lunch, including low-fat protein, whole grains, fruits, and vegetables. Goals are for the participant to benefit from healthy nutrition; such as increased energy, concentration, and optimized work performance. Wellness Credit Value: 100 for Challenge completion (Participant must earn at least 20 points to complete the challenge and receive 100 wellness credits.) Rest & Relaxation Challenge November 1 November 29 Stress management skills work best when they are used regularly, not just when the pressure is on. This includes getting enough rest and taking time to relax. This challenge is designed to encourage practicing techniques that will help participants cope with the effects of a hectic, stressful lifestyle. Wellness Credit Value: 100 for Challenge completion (Participant must earn at least 35 points to complete the challenge and receive 100 wellness credits.) Participant Satisfaction Survey An online survey is administered toward the end of the program year to measure the perceptions and satisfaction of participants. Credits are tracked automatically upon submission of the survey. Wellness Credit Value: 50 6
How to Get Started First Time User Login 1. Type in your browser: www.wellwithbluemt.com 2. Click on the Go to Login/Registration Page button. 4. Enter your Member ID: Use your BCBSMT insurance card and enter the numbers only (drop the letter pre-fix). Example: Your ID reads: YDH987654321 Only enter: 987654321 5. Select your gender and enter your date of birth. 6. Click the Validate Button. *If you do not have BCBSMT insurance, you can still access our wellness website. Contact your Human Resources Department to get your unique Member ID. 7. Verify your information and create your Login ID and Password. Keep in mind that your email address will become your Login ID. 3. Select your affiliation in the drop down box underneath Register then click the Register button.
