FENLAND EXERCISE REFERRAL Annual Health Professional Update 2017 Thank you for your interest in Fenland s Exercise Referral Scheme. This scheme is delivered by New Vision Fitness and managed via Fenland District Council. This update will provide you with the changes made to the scheme, a progress update for past two years and the local options available for your patients. Exercise Referral is a 12 week programme for patients referred by a Health Professional for conditions that have been proven to be improved or managed by regular physical activity. An individual programme is devised for the patient based on their referred condition and motivations to begin exercise. The patient is then supported throughout the 12 weeks by the Exercise Referral Specialist and encouraged to create a healthy habit for life within our Leisure Centres. Overall, we are pleased to announce that we have received 308 referrals over the past two years from local Health Professionals. However, we urge you to keep referring into this scheme to support us in our aim to reduce the health inequalities within Fenland. We would specifically welcome more referrals for diabetes and obesity; full referral criteria can be found later in this report. As I am sure you will be aware, there are many benefits of the scheme which can include: Improving the health and wellbeing of patients Impact upon the number of return visits to GPs and hospitals Reduce medication reliance Help control some health conditions or limit the regression of illnesses Reduced cost to the NHS For more information please email: sportsdevelopment@fenland.gov.uk ANNUAL HEALTH PROFESSIONAL UPDATE 2017 FENLAND EXERCISE REFERRAL

Headline Developments for 2017-18 We have listened to feedback and have now waivered the joining fee, saving the patient 16.99 when they sign up. The scheme still remains the cheapest way to access our Leisure Centres with a 3 month contract at 24.50 per month. Patients can use any of our Leisure Centres and a programme can include swimming and fitness classes subject to their referred condition. Performance Indicators The figures below provide data on performance that are collated and monitored annually as part of Fenland District Council s monitoring and evaluation processes. % Patient Reported Increase in Wellbeing 98% 85% 157 Number of Referrals 151 In line with the recent Public Health Profile for Fenland we would welcome more referrals for diabetes and obesity. How to make a referral All referring professionals must first complete a registration form which can be found at the back of this report or downloaded from: www.newvisionfitness.co.uk/exercis e-referral.html % Completion Rate 93% 90% 1092 141 Number of visits Number of Completions 1008 140 Once you are registered, making a referral is quick and easy. A referral form is completed by a health professional and then given to the patient who can then choose which New Vision Fitness Centre they want to begin their programme in. The differences we make What our clients and staff said about the Exercise Referral Scheme and the benefits of it. Being referred to the George Campbell Leisure Centre for extra support after my physiotherapy had come to an end was the best thing I did. It got me more mobile, confident and made me able to do so much more than I would if I didn t go. You get great care and the dedicated staff are there to help you. They offer a one to one programme to support your needs. George Campbell Exercise Referral member. The referral scheme is important to us as we have around 100 referrals per year. Most are retirees or those off work due to injuries or high blood pressure. Being able to get out and use the gym equipment enables them to socialise while improving their health. One of our greatest success stories is a gentleman that came in following an operation who could hardly walk. Our staff saw the progression he made and the vast improvements to his mobility and positive effects on this confidence. What conditions can you refer to the scheme? Please ensure your patient is eligible for a referral and at the right stage of behaviour change. You can refer for any of the following conditions Diabetes (Type 1 & 2) Obesity (BMI > 30) Cardiac Rehabilitation Hypertension Mental Health Respiratory (including Asthma) Smoking Cessation Multiple Trauma/Injury Muscular Skeletal Neurological We also had an Ex-army, 55 year old, who came to the George Campbell Centre with a degenerative hip joint problem and was also a borderline diabetic. He was overweight and at the beginning could only use the recumbent bikes. After following a personal exercise programme for 12 weeks he progressed to using a Swiss ball, treadmills and elliptical cross-trainers. After just one year he lost three stone in weight and was able to join in other aerobic fitness classes like circuits, group cycling and Boxercise. Still an active member of the gym this patient now trains up to five times a week. A total transformation, given that he had previously not undertaken any form of exercise since leaving the army. Centre Manager for New Vision Fitness We are actually very lucky to have such a great gym here in Chatteris. I have MS so mobility and movement control is sometimes a struggle. As I progress through the 12 week referral scheme I can t wait to try out some of the different workouts and I certainly won t get bored. Additional Classes and services Not everyone will be appropriate to refer into the exercise referral scheme, however exercising and leading a healthy lifestyle is beneficial for all. Below is a selection ection of options available to your patients that do NOT require a referral. Exercise Referral Scheme Health Professional Registration Form All staff involved in the Exercise Referral Scheme will protect the confidentiality of the patient and will operate within the conditions as set by the referring Health Professional (HP). The Health Professional will satisfy themselves that the patient is clinically suitable to exercise. The Health Professional is responsible for only the clinical aspect of a referral, not the exercise aspect. All Gym Consultants involved in the scheme will be comprehensively trained in Exercise Prescription and Fitness Evaluation. The cost to the patient will be 24.50 per Month on a 3-Month Commited Agreement. The Health Professional will complete, as fully as possible, a Referral for Exercise Form for each patient outlining recommendations for exercise and contra-indications.
