American Airlines Healthmatters Screening FAQs Q: Why does American offer the Healthmatters Wellness Program? A: Our employees make a difference every day. To ensure we re giving our customers the very best, we all need to be at our very best. One way to do that is to ensure we re making smart choices and living a healthy lifestyle. We re offering Healthmatters so employees have the tools, resources and support they need to get or stay healthy. Q: Who can get a biometric screening? A: Free biometric screenings are offered to all domestic mainline employees; however, only those enrolled in the Standard, Core, Value, PPO 80/90/100 or OOA medical options are eligible to earn the Healthmatters rewards associated with completing a screening. Q: Why should I get a biometric screening? A: It s always important to keep your health and wellness top of mind. Awareness is the first step in maintaining and monitoring your health, and your biometric numbers tell a lot about your health. Getting a biometric screening helps you understand your health from the inside out and keep you informed of your risks and strengths. When you complete your biometric screening in 2015, you will receive a MyGuide to Health from Quest Diagnostics which contains a personalized guide of your results, along with an action plan based on your unique results. If you are currently enrolled in the Core, Standard, Value, PPO 80, 90, 100, or OOA plan, you are also eligible to earn $50 in Healthmatters rewards for completing your screening. You can earn an additional $100 if you meet the screening targets. Q: What are Healthmatters Rewards? A: All employees who are enrolled in Standard, Core, Value, PPO 80/90/100 or OOA medical plans can earn up to $250 in Healthmatters Rewards by completing the wellness activities below. Those rewards get deposited into a health spending account and can be used for eligible medical, prescription, dental, and vision expenses.

Q: What are the screening targets? A: If you are enrolled in the Standard, Core, Value, PPO 80/90/100 or OOA medical plans and complete your biometric screening, you will earn $50 in Healthmatters Rewards. If you meet 4 out of 5 screening targets, you can earn an additional $100: LDL cholesterol < 160 HDL cholesterol > 45 Total cholesterol < 239 Blood pressure < 139/89 Hemoglobin A1C < 7.5% Q: When and how can I access my Healthmatters Rewards? A: Your earned Healthmatters rewards will be visible on the Healthmatters Rewards Center on myactivehealth.com/healthmatters within two weeks of completing your screening. Your Healthmatters Rewards will be deposited into the YSA account associated with your medical plan within 30 days of completing your screening. You have two ways of utilizing your rewards: 1. YSA Health Card- Can use for eligible medical dental, prescription, and vision expenses at your provider s office or pharmacy (be sure to save your receipts). 2. Automatic Re-Imbursement YSA will automatically reimburse you (via check or direct deposit) whenever you have a claim for eligible medical dental, prescription or vision expenses.

Q. What is the MyGuide to Health? A: Once you complete your biometric screening, you will receive a MyGuide from Quest Diagnostics in the mail in about 2-3 weeks. MyGuide to Health uses simply-stated insights to health and risk, with highly personalized content that can help guide you to better health. It provides powerful and engaging information where the health measures you most need to focus on meets what you can do to most improve them. MyGuide to Health maximizes the value of biometric screening by focusing on your unique health risks and how you can modify their behaviors to improve them. Q: How can I get my biometric screening done? A: You have several different ways to complete your biometric screening: 1. Schedule an appointment at a local Quest Diagnostics Patient Service Center. 2. Attend an onsite event at your station. 3. Bring the physician form to your doctor s office. Q: How can I schedule my screening appointment? A: Visit we.blueprintforwellness.com: Register as a new user by entering the registration key: American. Your unique ID is: Six digit AA Employee ID + two leading zeroes (ex: 00123555). Once you have completed registration, you will be able to schedule your screening appointment. If you have scheduling questions, call 1-855-623-9355 (855-6-BE WELL). Q: What if I ve forgotten my password or I need my password reset? A: There are separate links available for username and password. If you select the forgot your username link, you will need to provide the email address associated with your account and your date of birth. As long as the email address/date of birth combination provided matches what Blueprint for Wellness has on file, you will receive an email with your username. Q:Will cost me anything to get my biometric screening done? A: No, this biometric screening is offered to all American and US Airways employees for free.

Q: What is involved in the biometric screening? A: The process takes about 15 minutes from the point of check-in to completion. The screening will measure your blood pressure, height, weight, waist circumference and also includes a blood draw for many different blood tests. These tests are Direct LDL, Total Cholesterol, Cholesterol/HDL ratio, HDL Cholesterol and Hemoglobin A1c with Estimated Average Glucose (eag). Q: Do I need to fast before screening? A: No, the screening does not require you to fast. This means you are free to eat before your screening. To ensure your results provide an accurate depiction of your health, you should engage in your typical activities and diet for a couple of weeks prior to the screening. Q: How is Waist Circumference measured? A: The waist circumference process will be done in a private area either onsite or at a Quest Diagnostics Patient Service Center. The measurement will be taken above the first layer of clothing. Q: What is an A1c test? A: Hemoglobin A1c (also known simply as A1c ) measures how your body uses glucose (blood sugar) for energy. The result of your A1c test is like getting a history of your blood sugar for the past 2 to 3 months. Hemoglobin A1c with an Estimated Average Glucose (eag) is a calculation that allows you to understand the relationship to Glucose levels over time. Q: What is the advantage of A1c versus a glucose test? A: Hemoglobin A1c reflects the average blood sugar control for the previous 2-3 months, known as long-term control. In contrast, glucose levels reflect short-term control, influenced by diet, activity, and the daily cycle of our lives. Q: Why does Quest Diagnostics use A1c testing? A: The American Diabetes Association (ADA) recommends that individuals with diabetes be tested at least twice each year for those in good control and quarterly if those whose diabetes is not well controlled or whose therapy change. In the summer of 2009, an international expert committee assembled by the American Diabetes Association (ADA), International Diabetes Federation (IDF), and European Association for the Study of Diabetes (EASD) recommends the A1c assay as the new test for the diagnosis of diabetes. (Care.DiabetesJournals.org) Q: What is a Lipid Screening? A: Lipid screening is the most common technique used to evaluate the cardiovascular system and measures the different types of fat in the body. Cholesterol (fatty material) accumulates within the artery walls, where it

eventually hardens. Arteries subsequently lose their normal elasticity and become narrow, restricting the passage of oxygen-rich blood to the heart. There are many different kinds of lipids, most of which are included in your total cholesterol level. Lipids included in total cholesterol are HDL Good Cholesterol, and a direct measurement of your LDL Bad Cholesterol. Q: What is a Direct LDL test? A: The Direct LDL accurately determines your low-density lipoprotein (LDL) cholesterol (also called bad cholesterol) in your blood. This test is used when you are not fasting. In traditional screenings that require fasting, LDL cholesterol is calculated. Since you are participating in a screening that does not require you to fast, your LDL cholesterol will be a direct measurement. An elevated direct LDL result indicates a greater risk of developing heart disease. Q: Are my results confidential? A: Yes. Quest Diagnostics and American Airlines value and understand that your privacy is very important and we have put many steps in place to assure confidentiality. All information obtained in your biometric screening is Protected Health Information (PHI) and is secured in accordance with the Health Insurance Portability and Accountability Act (HIPAA). Your employer will not have access to individual results. Q: How do I get my results? A: You will receive a MyGuide from Quest Diagnostics in the mail within two to three weeks of your blood draw. The MyGuide will contain a breakdown of your results, as well as a customized action plan based on those results. If you registered online and provided an email address, you will receive an email when your results are available to view online 3-5 days after your screening. To view your results online, go to we.blueprintforwellness.com. If you previously created a username and password, enter your user name and password. If you did not register for an appointment, click the register now link in the yellow box. You will be prompted to create a username and password to access your results online. Q: What if have a regular physician exam and want my physician to provide my laboratory results? A: You can utilize a Physician Result Form. You can get a Physician Result Form at We.Blueprintforwellness.com Then follow the below steps:

Ask your doctor to complete it using recent lab results. Your doctor may charge a fee to complete the form and you would be responsible for payment. Check to see that the form is complete with all required information Make sure your doctor faxes the Physician Results Form to the fax number on the form before October 31. You will receive your report within 3 weeks after submitting Q: What is the deadline to complete my biometric screening? A: You must complete your biometric screening before October 31, 2015. If your physician is submitting the physician form to Quest Diagnostics, it will need to be received before October 31, 2015.
